Surgical technique and the analysis of clinical outcome of laparoscopic operation to cure ovarian endometriomas

【Objective】To investigate the surgical technique and analyse clinical outcome of laparoscopic clinical operation to cure ovarian endometriomas. 【Methods】The clinical data of 488 cases of ovarian endometriomas underwent laparoscopic operation in our hospital between April 1999 and June 2008 was retrospectively analysed. 【Results】All patients were performed under laparoscopic. During operation suction cyst fluid before dissociation adhesions, and then dissection capsule wall and hemostasis was achieved with electric coagulation. Among which, 299 cases underwent one side cystectomy, 189 cases underwent bilateral cystectomy, and without any complications. 145 cases take Nemestran for 3～6 months and 178 cases were injected with GnRH-α subcutaneously for 3 months. 375 cases had been followed after operation, the follow-up rate is 76.84%. 193 cases has pelvic pain before operation, of which 34 cases were complete remission and 138 cases were basic remission. The total symptom relief rate is 90.16%, the symptom relief rate of treatment group was 93.29%, while Non-drug group was 35.71%; combined with infertility is 132 cases, among which, 76 cases is pregnancy, the rate is 57.66%, the treatment group was 93.29%, while Non-drug group was 35.71%; 70 cases were recurrence, the total recurrence rate is 18.67%, the treatment group was 93.29%, while Non-drug group was 35.71%. 【Conclusion】Laparoscopic surgery is the first choice to treat ovarian endometriomas. We can get good results and reduce recurrence rate by master surgical technique and the application combined with drugs after surgery.
