This paper reports a 33-year-old nulliparous Malagasy woman with chronic menstrual-related pelvic pain who presented with abundant hemorrhagic ascites, and the authors sought to diagnose peritoneal endometriosis. Because access to laparoscopy was difficult, they used epidemiologic, history, clinical, morphological, and biological evidence to support a diagnosis of peritoneal endometriosis complicated by hemorrhagic ascites. The patient’s course included draining of the ascites and disappearance of chronic pelvic pain under suppressive hormonal treatment, which the authors state further supported the diagnosis. This paper is centrally about endometriosis — specifically peritoneal endometriosis revealed by hemorrhagic ascites, with the claimed novelty of being the first case from Madagascar.
