Clinical research of uterine adenomyosis treatment by bilateral graded uterine artery embolization

Abstract
Objective To investigate the clinical therapeutic effects and safety of bilateral graded uterine artery embolization(UAE) in treatment of adenomyosis(AM).Method Total 34 patients with AM were treated by bilateral graded UAE with polyvinyl alcohol particles(PVA) and fresh gelfoam particles.The degree of dysmennorrhea,amount of menorrhea,uterine and f ibroid volume,sex hormone levels were investigated before operation and on 1,3,6,12 months after operation.Results The results of bilateral graded UAE were successful,no serious complications occurred.Total 34 patients had dysmennorrhea before the operation.Two patients accepted hysterectomy due to ineffective treatment or recurrence of dysmenorrhea.For 32 patients,the symptom of dysmennorrhea disappeared in 1-12 months after the operation.In the 27 patients with hypermenorrhea,the amount of menorrhea returned to normal in 1-2 menstruations after operation.The 7 patients with normal menorrhea,the menorrhea amount did not change after the operation.The average hemoglobin of total 34 patients had regained from(75+9.5)g/L to(123+7.9)g/L,3 months later after the operation(P0.01).The median uterine volume reduced 42.7%(P0.01),and the f ibroid size decreased 60%(P0.01) on average.Sex hormone had no signif icant difference before and after treatment.Conclusion Bilateral graded UAE treatment of AM has good clinical eff icacy and safety.
