This study aimed to assess PrEP acceptance and associated factors in this population. Methods : A cross-sectional survey was conducted from September to December 2024 using multistage sampling. Data on demographics, PrEP knowledge, and vaccination willingness were collected from 229 staff across Guiyang's pet hospitals. Logistic regression analyses identified key determinants. Results : A total of 241 questionnaires were collected, with 229 valid questionnaires and a validity rate of 95.0 %. The willingness rate of pre-exposure prophylaxis was 68.1%, but the rate of actual vaccination was only 9.8%. The results of one-way c² test showed that the differences between groups of vaccination willingness were statistically significant for monthly income (P<0.001), whether they had been vaccinated with pre-exposure prophylaxis (P=0.003), and whether they thought that rabies vaccine could be given before exposure to prevent rabies (P<0.001). Multifactorial logistic regression analysis showed that those with a monthly income of $6,000 or more (OR=19.349, 95% CI=2.054-182.245), those who thought it was not possible to get rabies vaccine before the injury (OR=7.375, 95% CI=1.765-30.82), or those who did not know (OR=11.448, 95% CI=1.996-65.655 ) had a low willingness to vaccinate against rabies. The willingness to recommend rabies vaccination for rabies prevention (OR=2.936, 95% CI=1.218-7.076) was lower among those who believed that rabies vaccination for rabies prevention (OR=2.936, 95% CI=1.218-7.076) was not allowed before injury (OR=2.198, 95% CI=1.044-4.628) except for other positions of veterinarians, and were aged 25-30 years (OR=2.779, 95% CI=1.237-6.224). Conclusion : Targeted interventions should address economic barriers and knowledge gaps, particularly the prevalent "no exposure, no vaccination" misconception. Rabies Willingness To Vaccinate Pre-Exposure Prophylaxis Influencing Factors Pet Hospital Introduction Rabies is an acute infectious disease caused by a viral infection, with a mortality rate of up to 100 % in the absence of effective prevention or treatment [1] - [3] . According to the World Health Organization (WHO), globally about 59,000 people die of rabies every year, and more than 95% of these cases are caused by transmission from dogs. China used to be an endemic area for rabies, but with the implementation of a series of preventive and control measures (e.g., dog immunization, post-exposure treatment, etc.), the incidence of the disease has been declining year after year. [4] - [5] . However, there is still a gap between the World Health Organization's (WHO) goal of ‘eliminating dog-to-human rabies by 2030’ and the goal of ‘eliminating dog-to-human rabies by 2030.’ [6] Currently, effective rabies vaccination is an important way to prevent rabies, and its preventive vaccination strategy is mainly divided into pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis ( PEP). Pre-Exposure Prophylaxis of rabies refers to the prophylactic human rabies vaccination of individuals at risk of rabies infection before rabies exposure occurs [7] .Although it is generally accepted that vaccinating pets against rabies is the most cost-effective method of preventing rabies in humans, current pet vaccination coverage is far from 70%, making it difficult to form an effective immuner barrier [8] .Currently, post-exposure prophylaxis is the main way to prevent rabies in the population, but in the case of a short incubation period and severe exposure, the exposed person needs to be injected with expensive rabies immune globulin or monoclonal antibody at the same time as the first dose of vaccine, or he/she may still be at risk of life-threatening diseases [9] .In contrast, after completion of rabies PrEP, re-exposed persons only need to receive 1-2 doses of vaccine to rapidly stimulate an immune response without the need for additional rabies immunoglobulin or monoclonal antibody injections. This has the advantages of simplifying the disposal process, reducing the number of medical visits, lowering economic costs, and significantly improving the effectiveness of prevention [10] .WHO recommended rabies PrEP for people at high risk of rabies exposure in the 2018 Rabies Vaccine Position Paper, which includes veterinarians and other veterinary hospital staff [11] - [12] .Studies have shown that an individual's perception is an important factor influencing vaccination decisions [13] .Pet hospital staff's perceptions and attitudes toward rabies PrEP not only affect their own health, but may also influence the health behaviors of their family members and people in the community. Based on this, this study investigated the knowledge, vaccination intention, and influencing factors of rabies PrEP among pet hospital staff in Guiyang City, Guizhou Province, aiming to provide a scientific basis for improving the rabies PrEP vaccination strategy and increasing the vaccination rate, and thus contributing to the goal of eliminating canine-to-human rabies by 2030. Materials and methods 1. Subjects A cross-sectional survey was conducted between September 2024 and December 2024, using staff who are in contact with susceptible animals (e.g., cats, dogs) in pet hospitals in Guiyang City as subjects. Inclusion criteria: staff aged 18 years or older working in pet hospitals in six districts of Guiyang City (Yunyan, Baiyun, Guanshanhu, Huaxi, Nanming, and Wudang) between September 2024 and December 2024. Exclusion criteria: (1) those with cognitive impairments affecting autonomous behavior; (2) those who refused to sign the informed consent form; and (3) those who went out during the survey or refused to cooperate with the survey. The required survey sample size was calculated according to the cross-sectional survey sample size formula n = (Z 1- ɑ /2 ) 2 P (1- P )/ d 2 . According to the pre-survey results, the knowledge rate P is 63.0%, the permissible error (d) is 0.15P, and α is 0.05. Considering the sampling method using a multi-stage sampling DEFF value of 1.5, and taking into account the 10% non-response rate and invalid questionnaires, the final calculation is that at least 167 valid samples are needed. The multistage sampling method was used to select the respondents. 2. Research content and methods The study designed its own questionnaire based on relevant references and used an anonymous questionnaire for data collection. (See annex for details)Before the survey began, the investigators were uniformly trained, and after the training was completed, the investigator distributed the questionnaire to the staff of the pet hospitals, and the staff could voluntarily fill in the questionnaire by mobile phone or paper version. Before the survey began, the investigator explained in detail to the respondents the purpose and significance of the study and the anonymity of the questionnaire and emphasized that the data would only be used for academic research. The questionnaire survey included 3 parts: basic information of the respondents, knowledge of rabies PrEP, and willingness to vaccinate. 3. Statistical analysis The data were collated and statistically analyzed using Microsoft Excel 2019 and Jamovi 2.4.14 statistical software to calculate the rabies PrEP knowledge rate (= number of cases with knowledge/total number of cases surveyed × 100%), rabies PrEP vaccination rate (= number of cases vaccinated/total number of cases surveyed × 100%), willingness rate (= number of cases who had not been (= number of vaccinated cases/number of unvaccinated cases × 100%), and willingness to recommend (= number of cases willing to recommend vaccination/total number of surveyed cases × 100%). The chi-square test was applied to compare the groups, and the unconditional binary logistic regression was applied to analyze the influencing factors of vaccination rate, willingness to vaccinate, and willingness to recommend. The test level α = 0.05. Results 1. Basic Characteristics of Survey Respondents A total of 241 questionnaires were collected, of which 229 were valid, with a valid response rate of 95.0%. The gender distribution of the survey respondents was 80 males (34.9%) and 149 females (65.1%), with a gender ratio of 1:1.9. The age was mainly concentrated in 18-30 years old (78.5%), followed by 30-40 years old (16.6%), and 41 years old and above only accounted for 0.04%. Educational level was mainly bachelor’s degree or above, accounting for 94.3%, and other education accounted for 5.7%; job positions were mainly assistant and veterinarian, accounting for 80.3%, and other positions accounted for 19.7%; The distribution of years of experience was as follows: less than 1 year accounted for 15.3%, 1-3 years accounted for 49.3%, 4-6 years accounted for 18.8%, 6-10 years accounted for 6.6%, and more than 10 years accounted for 10.0%; and staff members with occupational exposure experience accounted for 88.6% of the staff and 11.4% of those with no exposure experience; the frequency of injuries was dominated by 1-3 times/year, which accounted for 63.1%, 4-5 times accounted for 12.7%, and more than 5 times accounted for 20.1%. 2. Rabies PrEP Awareness, Vaccination, and Cost Coverage Knowledge of rabies PrEP as a form of rabies prevention among the staff of pet hospitals in Guiyang City was 62.0%, which was lower than the knowledge of post-exposure prophylaxis (88.6%). Among them, 46.3% of the survey respondents believed that they were at risk of infection, and 72.1% recognized the effectiveness of pre-exposure vaccination. Among the 229 veterinary hospital staff, the rabies vaccination rate was 76% (17/229), of which 90.2% (157/174) were post-exposure prophylaxis, and only 9.8% (17/174) performed pre-exposure prophylaxis. Regarding cost bearing, 68.8% of the respondents indicated that the cost of vaccination was borne by the hospital. Main reasons for vaccination/willingness to vaccinate and reluctance/hesitation to vaccinate The results of the survey showed that of the 156 respondents who had been vaccinated or were willing to be vaccinated by PrEP, the main reasons were the belief that the vaccination would be effective in preventing rabies (94.2%), reducing anxiety (59.0%), reducing the time taken for the vaccination (51.3%), reducing the severe pain of the injection of rabies immune globulin (51.9%), and reducing the cost (46.2%). Of these, 22.4% of the vaccination costs were borne by the hospital. Among the 73 respondents who were reluctant or hesitant to vaccinate, the main reasons were that they thought they did not need to vaccinate if they were not injured (69.4%), high economic cost (41.7%), high time cost (43.2%), safety of the injured animal (had not gone out or the animal had been vaccinated) did not need to be vaccinated (22.2%), worried about the safety of the vaccine (37.5%), worried about the effectiveness of the vaccine (29.2%) , do not know can be injured before vaccination for prevention (11.1%) and do not know where to go for vaccination (5.6%). 4. Analysis of factors influencing willingness to vaccinate and willingness to recommend 4.4.1 One-way analysis The results of the one-way chi-square test showed statistically significant differences between groups of willingness to vaccinate in terms of monthly income ( P <0.001), whether or not they had been vaccinated with pre-exposure prophylaxis ( P =0.003), and whether or not they thought that pre-exposure could be done with rabies vaccine to prevent rabies ( P <0.001). ( Table 2) 76.8% of survey respondents were willing to recommend others for vaccination, with the highest willingness to recommend (85.7%) in the younger age group (18-25 years). Willingness to recommend was significantly associated with age ( P <0.001), position ( P =0.014) and PrEP perception ( P =0.038). (Table 4) Using the survey respondents' willingness to recommend people around them to receive rabies PrEP vaccine as the dependent variable (0=yes, 1=no), and including the factors that were statistically significant in the one-way analysis in the unconditional logistic regression analysis, the results showed that: the willingness not to recommend for age 26-30 years was 2.779 times higher than that of age 18-25; the willingness not to recommend that it was not possible to take rabies vaccine to prevent rabies before injury was 7.375 and 11.448 times higher than that of the opinion that it was possible to do so. rabies was 2.936 times more likely than the belief that it could, respectively; and the position of veterinarian was 2.198 times more likely than the other positions. Currently, in the national rabies elimination campaign phase, pet hospital workers, as a highly exposed population, have a positive effect on rabies elimination goals by increasing awareness and vaccination rates of rabies PrEP in this group. The results of this survey showed that the knowledge of rabies prevention methods among the staff of pet hospitals in Guiyang City was 60%-80% . This may be attributed to the strengthened rabies prevention and control publicity in urban communities in recent years, as well as the knowledge acquired by most of the staff through post-exposure prophylaxis in the course of their medical practice. However, pet hospital staff, as an occupational group and a high-risk group for rabies, are still insufficient. Meanwhile, although the knowledge rate of rabies PrEP among pet hospital staff (62. 3% ) was higher than that of the study conducted by He Ling et al. [17] for pet trading market staff, it was still insufficient compared to the knowledge rate of post-exposure prophylaxis (88. 6% ). This suggests that pet hospital staff have insufficient knowledge of the importance of proactive pre-exposure prophylaxis, believe that they do not need to be vaccinated if they have not been injured, take a chance that they can be exempted from rabies vaccination because the dogs and cats they come into contact with are fine, and have a misunderstanding of the economic and time costs of PrEP. In fact, the economic and time costs of PrEP are much lower than post-exposure prophylaxis for high-risk groups, a finding that emphasizes the need to strengthen education and outreach efforts on the knowledge of rabies PrEP in this group. The 9.8% PrEP vaccination rate lags behind the 32-45% reported in European veterinary cohorts, highlighting systemic barriers in China's occupational health system. Our finding that 69.4% staff endorsed 'no injury, no vaccination' mirrors cultural attitudes observed in Southeast Asian studies (OR=2.15, p<0.01). Numerous studies have shown that rabies PrEP vaccination rate is related to survey respondents' knowledge of rabies, and its associated risks [18] , but the vast majority of the staff in this study believed that disinfection after rinsing could be used as a preventive method of rabies and more than half of the staff believed that they did not have a risk of rabies, a finding that is also in line with the results of other domestic and international studies, that is, the knowledge of pet industry workers about rabies prevention knowledge is still deficient [19] , which may be related to the single daily publicity and lack of knowledge training. In addition, this study found that although most of the staff ( 76% ) had been vaccinated against rabies, only 9. 8% had received pre-exposure prophylaxis, a result that is consistent with the findings of Ou Zhimei et al. [20] and He Ling [17] . The low PrEP uptake may reflect reliance on post-exposure prophylaxis coverage through employer-purchased commercial insurance (reported by 68.8% respondents). This resulted in the majority of staff (68. 8% ) preferring post-exposure prophylaxis after injury. However, the success rate of post-exposure prophylaxis is influenced by a number of factors, such as timeliness of disposition, wound severity and viral load, all of which increase the risk of disease. Therefore, pre-exposure prophylaxis is still recommended for this high-risk group. It is noteworthy that the proportion of males vaccinated in this study was lower than that of females, which may be related to the higher tolerance of health risks and lower acceptance of vaccines among males. In this study, age, monthly income, and knowledge of rabies PrEP were found to be the main factors influencing the willingness to vaccinate against rabies PrEP. This result is consistent with previous studies that individual perceptions and economic factors are important factors influencing vaccination decisions [21] . In addition, some staff members had misconceptions about rabies exposure, such as believing that ungated pets or pets vaccinated against rabies do not carry the rabies virus and that minor bites or scratches do not require rabies vaccination. These misperceptions may have further reduced PrEP vaccination rates. Meanwhile, this study showed that staff with low income were more willing to receive rabies PrEP vaccination than those with high income, which may be due to the fact that the low-income group valued the economic benefits of rabies PrEP more. It is recommended to promote pet hospitals or the government to subsidize PrEP vaccination for low-income groups in high-risk occupations to lower the economic threshold. It is worth noting that the young group (18-25 years old) had a high willingness to recommend of 85.7% and the new media platform was their main source of information (72.1%), which provided a breakthrough for precise intervention. Meanwhile, the willingness to recommend was significantly correlated with age (P<0.001), position (P=0.014) and PrEP perception (P=0.038). Combined with the vaccination strategy for high-risk occupational groups proposed by WHO (2018), it is recommended to utilize new media platforms and promotional brochures, combined with the characteristics of high recommendation willingness of the young group (18-25 years old), combine PrEP promotion with short video platforms and vocational training courses, and through case simulation and cost-benefit comparisons, strengthen the "prevention is better than treatment Through case simulations and cost-benefit comparisons, it reinforces the core concept of "prevention is better than cure. The limitation of this study is that it relies only on subjective willingness surveys and does not incorporate actual vaccination records or behavioral observations, so there may be a "willingness-behavior discrepancy". At the same time, because the staff who volunteered to participate in the study might be more concerned about rabies prevention and control and their own health, which led to an overestimation of the overall willingness to be vaccinated, and some high-risk employees (e.g., those who frequently come into contact with stray animals) might have refused to participate due to their busy work schedules, which resulted in a response bias. Conclusion In summary, increasing the knowledge and vaccination rate of rabies PrEP among pet hospital staff is crucial to achieving the goal of eliminating dog-to-human rabies by 2030. It is recommended to increase the level of knowledge of rabies PrEP and the learning of rabies exposure disposal practices among pet hospital staff through regular trainings, online courses, and pamphlets, with a special focus on young, low-income staff, and those with low knowledge of rabies PrEP. Meanwhile, future studies should further explore effective strategies to increase rabies PrEP vaccination rates through policy and practice interventions. In addition, some staff members had misconceptions about rabies exposure, such as believing that ungated pets or pets vaccinated against rabies do not carry the rabies virus and that minor bites or scratches do not require rabies vaccination. Meanwhile, this study showed that staff with low income were more willing to receive rabies PrEP vaccination than those with high income, which may be due to the fact that the low-income group valued the economic benefits of rabies PrEP more. Data on demographics, PrEP knowledge, and vaccination willingness were collected from 229 staff across Guiyang's pet hospitals. Logistic regression analyses identified key determinants.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults\u003c/strong\u003e: A total of 241 questionnaires were collected, with 229 valid questionnaires and a validity rate of 95.0 %. The willingness rate of pre-exposure prophylaxis was 68.1%, but the rate of actual vaccination was only 9.8%. The results of one-way c² test showed that the differences between groups of vaccination willingness were statistically significant for monthly income (P\u0026lt;0.001), whether they had been vaccinated with pre-exposure prophylaxis (P=0.003), and whether they thought that rabies vaccine could be given before exposure to prevent rabies (P\u0026lt;0.001). Multifactorial logistic regression analysis showed that those with a monthly income of $6,000 or more (OR=19.349, 95% CI=2.054-182.245), those who thought it was not possible to get rabies vaccine before the injury (OR=7.375, 95% CI=1.765-30.82), or those who did not know (OR=11.448, 95% CI=1.996-65.655 ) had a low willingness to vaccinate against rabies.
