The study fine-tuned an interpretable ECG foundation model to predict severe or complete stenosis of each of the four major coronary arteries on coronary CT angiography (CCTA), using internal and external validation sets and assessing performance across demographic and acquisition-time subgroups. Reported discrimination (AUCs) was moderate to high, with internal AUCs of 0.794 (RCA), 0.818 (LM), 0.744 (LAD), and 0.755 (LCX), and external AUCs of 0.749, 0.971, 0.667, and 0.727, respectively; performance also remained stable in a clinically normal-ECG subset. The authors report robustness and use risk-stratification incidence thresholds with calibration and cumulative event curves, while interpretability analyses identified waveform differences between high- and low-risk groups, highlighting electrophysiological regions associated with stenosis. A major caveat explicitly stated is that this is a preprint that has not been peer reviewed. This paper does not explicitly discuss endometriosis or adenomyosis; it was included in the corpus via a keyword match in the upstream search index.

Abstract

Abstract Coronary artery disease (CAD) remains a major global health burden. Accurate identification of the culprit vessel and assessment of stenosis severity are essential for guiding individualized therapy. Although coronary CT angiography (CCTA) is the first-line non-invasive modality for CAD diagnosis, its dependence on high-end equipment, radiation exposure, and strict patient cooperation limits large-scale use. With advances in artificial intelligence (AI) and the widespread availability of electrocardiography (ECG), AI-ECG offers a promising alternative for CAD screening. In this study, we developed an interpretable AI-ECG model to predict severe or complete stenosis of the four major coronary arteries on CCTA. On the internal validation set, the model's AUCs for the right coronary artery (RCA), left main coronary artery (LM), left anterior descending artery (LAD), and left circumflex artery (LCX) were 0.794, 0.818, 0.744, and 0.755, respectively; on the external validation set, the AUCs reached 0.749, 0.971, 0.667, and 0.727, respectively. Performance remained stable in a clinically normal-ECG subset, indicating robustness beyond overt ECG abnormalities. Subgroup analyses across demographic and acquisition-time strata further confirmed model stability. Risk stratification based on vessel-specific incidence thresholds showed consistent separation on calibration and cumulative event curves. Interpretability analyses revealed distinct waveform differences between high- and low-risk groups, highlighting key electrophysiological regions contributing to model decisions and offering new insights into the ECG correlates of coronary stenosis.

Accurate identification of the culprit vessel and assessment of stenosis severity are essential for guiding individualized therapy. Although coronary CT angiography (CCTA) is the first-line non-invasive modality for CAD diagnosis, its dependence on high-end equipment, radiation exposure, and strict patient cooperation limits large-scale use. With advances in artificial intelligence (AI) and the widespread availability of electrocardiography (ECG), AI-ECG offers a promising alternative for CAD screening. In this study, we developed an interpretable AI-ECG model to predict severe or complete stenosis of the four major coronary arteries on CCTA. On the internal validation set, the model's AUCs for the right coronary artery (RCA), left main coronary artery (LM), left anterior descending artery (LAD), and left circumflex artery (LCX) were 0.794, 0.818, 0.744, and 0.755, respectively; on the external validation set, the AUCs reached 0.749, 0.971, 0.667, and 0.727, respectively. Performance remained stable in a clinically normal-ECG subset, indicating robustness beyond overt ECG abnormalities. Subgroup analyses across demographic and acquisition-time strata further confirmed model stability. Risk stratification based on vessel-specific incidence thresholds showed consistent separation on calibration and cumulative event curves. Interpretability analyses revealed distinct waveform differences between high- and low-risk groups, highlighting key electrophysiological regions contributing to model decisions and offering new insights into the ECG correlates of coronary stenosis.","manuscriptTitle":"Fine-tuning an ECG Foundation Model to Predict Coronary CT Angiography Outcomes","msid":"","msnumber":"","nonDraftVersions":[{"code":1,"date":"2025-12-08 07:58:44","doi":"10.21203/rs.3.rs-8234977/v1","editorialEvents":[{"type":"communityComments","content":0},{"type":"decision","content":"Revision requested","date":"2025-12-31T16:17:27+00:00","index":"","fulltext":""},{"type":"editorInvitedReview","content":"","date":"2025-12-16T21:40:21+00:00","index":"hide","fulltext":""},{"type":"editorInvitedReview","content":"","date":"2025-12-16T14:27:51+00:00","index":"hide","fulltext":""},{"type":"reviewerAgreed","content":"118326094863625122294230889794569648469","date":"2025-12-12T08:36:31+00:00","index":"hide","fulltext":""},{"type":"editorInvitedReview","content":"","date":"2025-12-06T03:14:07+00:00","index":"hide","fulltext":""},{"type":"reviewerAgreed","content":"295830624552428702944649408914652826924","date":"2025-12-06T01:38:33+00:00","index":"hide","fulltext":""},{"type":"reviewerAgreed","content":"36014143736712074667810059427897607663","date":"2025-12-05T14:16:38+00:00","index":"hide","fulltext":""},{"type":"reviewerAgreed","content":"225766478194460863788876306899594417109","date":"2025-12-05T10:08:31+00:00","index":"hide","fulltext":""},{"type":"reviewersInvited","content":"","date":"2025-12-05T06:07:48+00:00","index":"","fulltext":""},{"type":"editorAssigned","content":"","date":"2025-12-04T00:56:27+00:00","index":"","fulltext":""},{"type":"checksComplete","content":"","date":"2025-12-03T12:19:58+00:00","index":"","fulltext":""},{"type":"submitted","content":"npj Digital Medicine","date":"2025-11-29T06:53:45+00:00","index":"","fulltext":""}],"status":"published","journal":{"display":true,"email":"[email protected]","identity":"npj-digital-medicine","isNatureJournal":false,"hasQc":true,"allowDirectSubmit":false,"externalIdentity":"npjdigitalmed","sideBox":"Learn more about [npj Digital Medicine](http://www.nature.com/npjdigitalmed/)","snPcode":"41746","submissionUrl":"https://submission.springernature.com/new-submission/41746/3","title":"npj Digital Medicine","twitterHandle":"","acdcEnabled":true,"dfaEnabled":true,"editorialSystem":"stoa","reportingPortfolio":"NPJ","inReviewEnabled":true,"inReviewRevisionsEnabled":true}}],"origin":"","ownerIdentity":"588422b8-6456-4fb5-8f71-e0d5f022ab4c","owner":[],"postedDate":"December 8th, 2025","published":true,"recentEditorialEvents":[],"rejectedJournal":[],"revision":"","amendment":"","status":"under-review","subjectAreas":[{"id":59187169,"name":"Health sciences/Cardiology"},{"id":59187170,"name":"Biological sciences/Computational biology and bioinformatics"},{"id":59187171,"name":"Health sciences/Diseases"}],"tags":[],"updatedAt":"2026-05-20T03:09:04+00:00","versionOfRecord":[],"versionCreatedAt":"2025-12-08 07:58:44","video":"","vorDoi":"","vorDoiUrl":"","workflowStages":[]},"version":"v1","identity":"rs-8234977","journalConfig":"researchsquare"},"__N_SSP":true},"page":"/article/[identity]/[[...version]]","query":{"redirect":"/article/rs-8234977","identity":"rs-8234977","version":["v1"]},"buildId":"8U1c8b4HqxoKbykW_rLl7","isFallback":false,"isExperimentalCompile":false,"dynamicIds":[84888],"gssp":true,"scriptLoader":[]}
