In this study, we determined the proportion of adverse fetal outcomes and identified associated factors among pregnant mothers of advanced age who delivered at Kabale Regional Referral Hospital (KRRH) in Southwestern Uganda. Methods: We conducted a cross-sectional study at KRRH’s Maternity Ward from March to August 2023. We enrolled post-delivery mothers aged ≥35 years. We collected data on socio-demographic, obstetric, and medical characteristics, as well as fetal outcomes, using interviewer-administered structured questionnaires. We defined an adverse fetal outcome as the occurrence of any of the following complications: early neonatal death, preterm birth, congenital anomalies, macrosomia, low APGAR score, or low birth weight, sustained by the fetus or neonate during pregnancy, delivery, or the immediate post-delivery period before discharge from the hospital. We performed multivariable logistic regression to identify factors associated with adverse fetal outcomes. Results: We enrolled 417 participants, of whom 206 (49.4%) were aged 35-37 years. The majority (n=273; 65.5%) were multiparous (parity ≥5). Adverse fetal outcomes occurred in 8.4% (n=36; 95% CI: 6.1-11.5%) of participants, with early neonatal death being the most common (3.6%), followed by low birth weight (2.4%), congenital anomalies (2.6%), macrosomia (1.4%), low APGAR score (1.4%), and preterm birth (1.2%). Prolonged labor (adjusted odds ratio [aOR]=3.71, 95% CI: 1.40-9.85) and a history of abortion (aOR=2.56, 95% CI: 1.17-5.60) were significantly associated with adverse fetal outcomes. Conclusion: Approximately 1 out of every 10 advanced-aged mothers surveyed experienced a poor fetal outcome, with mothers who had prolonged labor or history of abortions having increased odds of the poor fetal outcomes. Close monitoring during pregnancy and neonatal care preparedness among mothers of advanced age especially those with prolonged labour and history of abortion could mitigate risks of adverse fetal outcomes in this study population. Advanced maternal age adverse fetal outcomes perinatal mortality and morbidity Uganda Figures Figure 1 INTRODUCTION Advanced aged mothers refer to those mothers aged 35 years or older while pregnant or at time of giving birth. It’s currently observed globally that the average age at child bearing is increasing. This is made more possible with the current advent and adoption of assisted reproductive technology even in menopausal women through egg donor programs. Additional factors contributing to this trend include intentional postponement due to career commitments, prolonged career paths, delayed marriages, and the continuation of childbearing into advanced age. While more prevalent in high-income countries than in low- and middle-income nations, advanced maternal age still constitutes a substantial and increasing proportion of pregnant women globally( 1 ). A multinational study spanning 29 countries in Africa, Asia, the Middle East, and Latin America found that, on average, the prevalence of pregnant women classified as of advanced maternal age was 12.3%. This prevalence varied widely across countries, ranging from 2.8% in Nepal, 8% in Uganda, 18% in the Democratic Republic of Congo, 31.1% in Japan, to 33.4% in Norway ( 1 , 2 ). In Northern Ethiopia, the prevalence of advanced maternal age was found to be 37.5% ( 3 ). Pregnancies among advanced aged mothers are known to be associated with adverse fetal outcomes including low birth weight, premature births, birth asphyxia, admission to neonatal intensive care unit (NICU), and neonatal deaths ( 4 – 8 ). The prevalence of these adverse fetal outcomes varies across regions and studies, with rates of 29.1% in Ethiopia( 3 ), and 10.8% in Denmark( 9 ). However, most studies have focused on individual adverse outcomes rather than composite rates. Factors associated with these adverse outcomes include maternal demographics (e.g., education levels, income levels, and residence), maternal comorbidities (e.g., hypertension, diabetes, and heart disease), and obstetric parameters (e.g., parity, prenatal care, duration of labor, mode of delivery, gestational hypertension, pre-eclampsia, history of abortion, and contraception) ( 3 – 5 , 10 , 11 ). While studies on fetal outcomes among advanced aged mothers have been done elsewhere outside Africa( 3 , 6 , 7 ), and in some African countries( 4 , 6 , 11 ), to the best of our knowledge, no study has been conducted in Uganda to determine fetal outcomes and associated factors among advanced-aged mothers. These data are necessary to serve as a basis for designing and planning evidence-based preventive interventions for this category of pregnancies, thereby improving outcomes in this maternal age group and providing a foundation for further related studies in this special maternal age group. This study aimed to determine the proportion of adverse fetal outcomes and identify associated factors among mothers of advanced age who delivered at a regional referral hospital in Southwestern Uganda. METHODS Study setting, study design and study population We conducted across sectional study at the Maternity Ward of Kabale Regional Referral Hospital (KRRH), Southwestern Uganda. The hospital doubles as a teaching hospital for Kabale University School of Medicine as well as a regional referral hospital for districts in Southwestern Uganda and neighbouring countries of Democratic Republic of Congo and Rwanda. The Maternity Ward of the hospital conducts approximately 450 deliveries every month. For our study, we included all mothers aged 35 years or older at the time of delivery who were admitted to the Maternity Ward of KRRH from March to August 2023. Sample size and sampling methods The sample size was determined using Kish Leslie formular 1965 N=(Z 2 pq/d 2 ). N = Total number of participants required. Z = the critical value (it is 1.96 at 0.05 level of significance). p = proportion of adverse fetal outcomes among advanced aged mothers d = margin of error. We set our level of significance at 5% and confidence level at 95%, proportion of adverse fetal outcomes among advanced aged mothers at 50% since we lacked local prevalence. After adjusting for non- response of 10%, we included 427 women of advanced age and sampled them consecutively. Data collection and study variables Data collection involved the administration of structured questionnaires by four trained research assistants, all midwives under close supervision by the principal investigator, after obtaining participant written informed consent. The collected data encompassed independent variables (maternal socio-demographic, medical, and obstetric characteristics), while dependent variables comprised various fetal outcomes. Socio-demographic variables included maternal age, education level, occupation, and marital status. Medical characteristics included pre-gestational diabetes, chronic hypertension, preexisting renal, liver, and heart diseases, as well as long-term medications. Obstetric variables included parity, gestational age at delivery, preterm rupture of membranes (PROM), hypertensive disorders of pregnancy, gestational diabetes, duration of labor, obstructed labor, antepartum hemorrhage, and mode of delivery. Fetal outcomes, considered as dependent variables, included preterm birth, birth weight, intrauterine growth restriction, intrauterine fetal death, APGAR Score, birth asphyxia, admission to NICU, and neonatal death. Data management and analysis Data were entered in Epi Info Version 7 (CDC, Atlanta, USA) and exported to Stata version 15 (StataCorp, College Texas, USA) for analysis. We used descriptive statistics to characterize maternal socio-demographic, obstetric, and medical characteristics, as well as fetal outcomes, presenting these as frequencies and percentages. We then compared the distribution of the characteristics between advanced aged mothers with adverse fetal outcomes and those without, using either the Chi-square test or Fisher's exact test. To identify factors associated with adverse fetal outcomes, we performed logistic regression analyses at both the bivariate and multivariate levels, with odds ratios and corresponding confidence intervals, serving as measures of association. Variables with a p-value < 0.2 in the bivariate analysis were included in the multivariable logistic regression. Variables with a p-value < 0.05 in the multivariable model were considered statistically significant. RESULTS Out of the 2,700 mothers who gave birth at KRRH from March to August 2023, 436 (16.1%) were classified as advanced-aged mothers. Among these, we enrolled and analyzed data for 417 eligible women (Fig. 1 ). Characteristics of study participants Out of the 4117 participants, nearly half (49.4%) were aged 35–37 years, while only 13.7% were older than 40 years. The majority (65.7%) resided in rural areas, and 65.5% were multiparous (P5 + 0). Additionally, 94% attended at least 4 antenatal care (ANC) visits, and 77.9% used contraception before the current pregnancy. Regarding education, 36.9% had attained primary education, and 37.2% had completed secondary education. Only 3.3% reported chronic illnesses (Table 1 ). History of abortion was more common among women with adverse fetal outcomes (42.9%) compared to those without (21.7%, p = 0.005). Additionally, hypertension was significantly more prevalent among women with adverse outcomes compared to those without (7.4% vs. 0.8%, p < 0.001). No other significant differences were observed in the demographic and medical characteristics between the two groups (Table 1 ). Similarly, the proportion of mothers with antepartum hemorrhage was higher among those with adverse outcomes than those without (5.7% vs. 0.8%, p = 0.04) (Table 2 ). Obstructed labor occurred more frequently in mothers with adverse outcomes compared to those without (25.7% vs. 13.4%, p = 0.046). Additionally, the proportion of mothers with a duration of active labor > 12 hours was significantly higher among those with adverse outcomes (37.1% vs. 13.6%, p = 0.001). No significant differences were observed in other obstetric characteristics, including term PROM, chorioamnionitis, chronic hypertension, preeclampsia, eclampsia, gestational diabetes, anemia in pregnancy, postpartum hemorrhage, and mode of delivery, between the two groups (Table 2 ). Mothers experiencing prolonged labor had 3.71 times the odds of adverse fetal outcomes compared to those without prolonged labor (adjusted odds ratio [aOR] = 3.71, 95% CI: 1.40–9.85, p = 0.009). Similarly, mothers with a history of abortion had 2.56 times the odds of adverse fetal outcomes compared to those without such a history (aOR = 2.56, 95% CI: 1.17–5.60, p = 0.019) (Table 4 ). Mothers of advanced age with prolonged labor or a history of abortion had increased odds of experiencing adverse fetal outcomes compared to other advanced-aged mothers. In our study, 8.4% of participants experienced at least one adverse fetal outcome. Limited studies have examined prevalence rates across multiple adverse fetal outcomes, which makes direct comparisons challenging. However, our findings are comparable to those from a study in Denmark, where 10.8% of pregnant women aged 40 years or older experienced one or more adverse pregnancy outcomes( 9 ). A multi-site study in Ethiopia reported a much higher perinatal adverse outcome rate of 29.1% ( 3 ) than ours. The lower rate in our study could be due to the majority of participants being under 40 years old (only 14% were ≥ 40 years), unlike the Ethiopian study. Given the recognized increased risk of adverse fetal outcomes among advanced-aged mothers( 12 ), healthcare professionals could consider counseling couples intending to give birth later in life about these risks. Additionally, improving contraception uptake to reduce pregnancies among mothers of advanced age in our setting should also be considered, especially since the vast majority (approximately two-thirds) of the mothers in our study were of grand-multiparity (parity ≥ 5). In our study, advanced aged mothers with prolonged labour were more likely experience adverse fetal outcomes. Prolonged labour is inherently associated with a number of deleterious fetal outcomes including fetal distress, fetal death, ENND, fetal sepsis. Several studies have consistently found an association between prolonged labour and such adverse fetal outcomes in low-, mid-, and high-income countries of Africa, Asia ( 11 ), South Africa ( 13 ), and Greece ( 14 ). However, some studies such as those conducted in Germany ( 15 ), and Calfornia, USA ( 16 ) have showed no association between prolonged labour and adverse fetal outcomes. The difference could be attributed to the differences in the capacity of intensive care facilities in the respective study settings, as adequate neonatal care can mitigate many of these adverse outcomes. Based on our findings, implementing proper labor monitoring to prevent prolonged labor in advanced-aged mothers could significantly reduce the risk of adverse fetal outcomes. Additionally, in cases where prolonged labor is already established, ensuring adequate preparations for neonatal resuscitation for babies born to advanced-aged mothers should be considered to mitigate the risk of adverse fetal outcomes. In our study, advanced-aged mothers with a history of abortion showed a higher likelihood of adverse fetal outcomes compared to those without such a history. The impact of previous abortions on subsequent pregnancies can involve various mechanisms, including residual effects on cervical competence leading to premature births; endometrial trauma predisposing to placenta previa, and immunological factors like thrombophilias causing placental insufficiency and fetal growth restriction, potentially resulting in intrauterine fetal demise ( 17 – 19 ). Similar associations has been observed in other studies done elsewhere ( 20 – 22 ). Implementing history-based screening protocols to identify at-risk advanced-aged mothers with a history of abortions and providing them with closer antenatal follow-up could facilitate timely interventions and help reduce the risks of adverse fetal outcomes. Our study has limitations. It is a single-site study, limiting the generalizability of our findings to advanced-aged mothers in Southwestern Uganda and similar peri-urban settings in sub-Saharan Africa. The majority of participants were under 40 years old, potentially underestimating the prevalence of adverse fetal outcomes and introducing bias towards the null for the assessed factors. Additionally, the absence of very advanced-aged women (≥ 40 years) and younger mothers (< 35 years) in our study population hinders comparative analysis. We recommend future larger comparative studies for a more comprehensive understanding of the impact of advanced maternal age on perinatal outcomes in our setting. A notable strength of our study is its pioneering exploration of adverse fetal outcomes among advanced-aged mothers in the East African region. It\u0026rsquo;s currently observed globally that the average age at child bearing is increasing. This is made more possible with the current advent and adoption of assisted reproductive technology even in menopausal women through egg donor programs. Additional factors contributing to this trend include intentional postponement due to career commitments, prolonged career paths, delayed marriages, and the continuation of childbearing into advanced age. While more prevalent in high-income countries than in low- and middle-income nations, advanced maternal age still constitutes a substantial and increasing proportion of pregnant women globally(\u003cspan citationid=\"CR1\" class=\"CitationRef\"\u003e1\u003c/span\u003e). A multinational study spanning 29 countries in Africa, Asia, the Middle East, and Latin America found that, on average, the prevalence of pregnant women classified as of advanced maternal age was 12.3%. In this study, we determined the proportion of adverse fetal outcomes and identified associated factors among pregnant mothers of advanced age who delivered at Kabale Regional Referral Hospital (KRRH) in Southwestern Uganda.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethods: \u003c/strong\u003eWe conducted a cross-sectional study at KRRH’s Maternity Ward from March to August 2023. We enrolled post-delivery mothers aged ≥35 years. We collected data on socio-demographic, obstetric, and medical characteristics, as well as fetal outcomes, using interviewer-administered structured questionnaires. We defined an adverse fetal outcome as the occurrence of any of the following complications: early neonatal death, preterm birth, congenital anomalies, macrosomia, low APGAR score, or low birth weight, sustained by the fetus or neonate during pregnancy, delivery, or the immediate post-delivery period before discharge from the hospital. We performed multivariable logistic regression to identify factors associated with adverse fetal outcomes.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults:\u003c/strong\u003e We enrolled 417 participants, of whom 206 (49.4%) were aged 35-37 years. The majority (n=273; 65.5%) were multiparous (parity ≥5). Adverse fetal outcomes occurred in 8.4% (n=36; 95% CI: 6.1-11.5%) of participants, with early neonatal death being the most common (3.6%), followed by low birth weight (2.4%), congenital anomalies (2.6%), macrosomia (1.4%), low APGAR score (1.4%), and preterm birth (1.2%).
