Abstract

症例は38歳女性.5回目の右気胸で当院に入院した.いずれの気胸も月経の前後3日以内に発症しているため月経随伴性気胸を強く疑い, 根治目的に胸腔鏡下手術を施行した.横隔膜ドーム中央に, 周囲に点状の褐色色素沈着を伴った欠損孔を認め横隔膜欠損による気胸と診断, 同部位を胸腔鏡用自動縫合器で切除した.組織学的にも子宮内膜症と診断した.術後, ホルモン療法に関しては患者希望もあり行わず経過観察としたが2ヵ月後に気胸の再発が認められ, BUZERELIN 900μg/dayの投与を開始し2ヵ月後の現在再発なく外来通院中である.本症の組織像から月経随伴性気胸の発生機序を推察した.また, 治療として外来療法だけでなくホルモン剤による全身療法が必要と考えられた.
