Résultats à long terme après cryoablation percutanée des nodules d'endométriose de la paroi abdominale

Abstract
Objective: to retrospectively evaluate the long term outcomes after percutaneous cryoablation (CA) of abdominal wall endometriosis (AWE). Method: in this IRB approved retrospective observational study, forty patients (median age, 37 years; interquartile range [IQR], 32-40 years) presenting with 52 symptomatic nodules, treated with CA between January 2013 and May 2022 and with at least 12 month follow-up were reviewed. Thirteen patients (32%) presented with deep endometriosis. Clinical pain outcomes were assessed using the visual analog scale (VAS). Adverse events were analyzed and graded according to the CIRSE classification. Results: the median follow-up was 40.5 months (IQR, 26.5-47.2 months). Median VAS score before CA was 8 (IQR, 7-9). Five patients (12.5%) had residual pain and three patients (7.5%) had a recurrence of symptoms more than 12 months after CA. Five patients underwent a second CA, one patient surgery and two patients did not wish to undergo further treatment as they considered the pain reduction adequate. The secondary efficacy of CA to control pain was 87.5% (35/40). Complete symptom relief was recorded in 80% (32/40) of patients from the first CA at the time of last follow up. The median pain-free survival rates were 89.2% [95% CI, 70.1-96.4] at 36 months, and 76.8% [55.3-83.8] after 60 months, respectively. Conclusion: CA of AWE is safe and effective to achieve long-term pain control.
