Abstract

症例は38歳女性．月経に一致した左気胸を発症し当センターを紹介された．月経22日後に行った胸腔鏡下手術にて，肺瘻の部位や気腫性嚢胞は確認できなかったが，左胸腔内の臓側，壁側胸膜にびまん性に多発する褐色の結節を認めた．病理学的に臓側，壁側胸膜ともに子宮内膜組織を認め，月経随伴性気胸と診断した．術後第3病日に月経が開始し右気胸を併発したが胸腔ドレナージにより改善した．後にホルモン療法を施行し，術後1年の経過では気胸の再発を認めていない．本症例は月経直前に手術を施行したため脱落する前の病変が観察されたと考えられた．発症機序は腹腔からの子宮内膜組織の侵入と臓側胸膜病変の脱落による気胸発症と推測された．壁側胸膜に子宮内膜組織を認めることはまれであり，月経随伴性気胸の発症機序を考えるうえで，本症例は興味深く貴重な症例と思われた．
