Our study aimed to assess the knowledge, attitude and associated factors towards tuberculosis(TB) in the general population of shanghai. Materials and methods A multistage random sampling was used to collect data at 2020 with a self-designed questionnaire from 6022 whole people living in Shanghai, and 5604 questionnaires were effective, with an effective rate of 93.1%. We also compared the investigation with 2013 and 2015. Univariate and multiple linear regression analyses were used to analyze the factors associated with TB knowledge. Results Initially, a total of 16153 samples were included analysis in this study, yielding a response rate of 91.1%. There were 5003 in 2013, 5,546 in 2015 and 5,604 in 2020, and the total knowledge rate of the respondents was 71.2%, 81.3% and 83.6%, separately. The official account of WeChat with high acceptance by participants emerged to publicize TB prevention and control knowledge in 2020. In 2020, occupation (medical staff: OR = 6.20, p 10000 RMB: OR = 4.21, p = 0.001, 95%CI: 1.85 ~ 9.59). were associated with knowledge of TB. Conclusions Knowledge of TB among respondents was improved as time as go. New media will play an important role in health promotion. Except medical staff, other people needed more health education to improve the awareness level of tuberculosis. Though some countries have already reduced their disease burden of TB to fewer than 10 cases and less than one death per 100,000 population per year 1 , TB is still a serious health problem and one of the leading causes of death worldwide. It is estimated that 10.0 million people globally were newly diagnosed with TB in 2019 2 . China is one of the countries with a high burden of tuberculosis in the world. The World Health Organization tuberculosis report estimated that the number of tuberculosis cases in China in 2019 was about 83.3 thousand 2 . The Chinese government attaches great importance to the prevention and control of tuberculosis. National Tuberculosis Prevention and Control Plan (2011–2015) clearly pointed out that through strengthening health education and popularizing prevention and control knowledge, the awareness rate of the core information on tuberculosis prevention and control should reach 85% 3 . Promoting knowledge about TB is an important measure to prevent the spread of pulmonary TB disease 4 . A higher level of knowledge contributes to better attitudes and behaviors. The knowledge gap regarding the signs and symptoms of TB could affect the identification of the infection and delay treatment and increase the risk of transmission to other members of the community 5 . A study conducted that 53% of participants had a low level of knowledge and only 63% had a favorable attitude regarding TB 6 . Health education through spreading knowledge and guiding the audience the health behavior habit are needed to rapidly reduce the occurrence of infectious diseases 7–9 . This study aimed to assess the 3 stage status and associated factors of TB knowledge among people in Shanghai City, China. Materials and methods Study design and setting A community-based intervention study was carried out in 2020 in four districts of Shanghai, including Xuhui, Changning, Minhang and Songjiang districts. The number of monitoring points was determined according to the population of the four districts, ensuring that each district had at least one monitoring point. Taking the street as a sampling unit, the random number method was used to identify the street to be investigated. Random cluster sampling was conducted by using the random number method to determine the neighborhood committees that should be investigated. During the implementation of this study, comprehensive intervention measures were implemented, integrating communication resources such as Internet, news broadcasting and interpersonal communication network, and establishing a health communication network composed of the media, science and technology associations, cultural venues, large screen broadcasting units and each street or town to provide appropriate and effective TB health education and services tailored to the needs of different groups of people in different settings. After interventions, questionnaire of Shanghai Public Knowledge and Beliefs about TB Prevention and Control Behavior (Community) was used to evaluate the effectiveness of interventions. Sample size and participants The participants in this study were permanent residents who were aged \(\:\ge\:\) 15 years and had lived in the community for more than six months. The number of samples was calculated according to the sample size formula of the prevalence survey, with 5% type 1 error ( p < 0.05). According to the National Survey of Public Knowledge and Attitude about Tuberculosis Control Behavior, the knowledge rate about tuberculosis of the baseline population in Shanghai will be set at 0.5, necessitating a minimum of 4330 participants. Considering financial, material, human and time factors, while increasing the rate of loss to follow-up by 10%, the minimum sample size for each district was 1250. Questionnaire design The questionnaire covered three main themes: ( 1 ) Demographics and general information about the participants, including sex, age, education, marriage, occupation and average monthly household income, ( 2 ) Basic knowledge of TB control, ( 3 ) Receipt of health education, including the main channels for obtaining information on TB control and the degree of acceptance of media materials for health education on TB control. Statistical Analysis Data were entered by Epi Data 3.1, and statistical analysis was carried out using SAS 9.4. Categorical variables were presented as counts and percentages in the tables. We used Chi-square test to explore variables influencing participants' knowledge of tuberculosis. Variables identified with a P-value less than 0.25 in univariate analysis were included in the multivariate logistic regression analysis. A 95% confidence interval (95%CI) and a 5% level of significance were used to interpret the statistical significance. Statistical significance was considered if p -value < 0.05. Results 1.1 Socio-demographic characteristics Table 1 presents the characteristics of the study participants. The mean age was 52.95 years in 2013, 51.23 years in 2015, 46.11 years in 2020 and 48.9%, 45.9% and 39.5% were male for 3 different stages. There were statistical differences in age, sex, education, marital status, occupation and average monthly income of the respondents in different survey periods (all p < 0.05). Core messages corresponding to questions in the questionnaire were listed in Table 2 . The total knowledge rate of TB core information was calculated by the percentage of the total number of items that correctly answered the questions corresponding to the core information to the total number of items that answered the questions corresponding to the core information. The knowledge rate of single TB core information was defined as the percentage of people who answered the core information correctly to the total number of people that correctly answered the same core information. The comparison chart of core knowledge awareness in previous surveys was presented in Fig. 1 . In 2013, the total knowledge rate of respondents before intervention was 71.2%. After the intervention, in 2015, the total knowledge rate of the respondents was 81.3%. In 2020, the total knowledge rate of the respondents was 83.6%. Can tuberculosis be transmitted? 2 Coughing or producing phlegm for more than 2 weeks or sputum with bloodshot should be suspected of tuberculosis. How long should patients with symptoms of coughing or producing phlegm be considered for TB? 3 Those suspected of tuberculosis should go to designated hospitals for further examination and treatment. Where is the best place to seek medical attention if patients suspect tuberculosis? 4 Patients who are examined and treated for tuberculosis at designated hospitals are entitled to the reduction policy for tuberculosis patients in Shanghai. Are there any reduction policies for tuberculosis patients to examine and treat? 5 Most tuberculosis patients can be cured with regular treatment. Can tuberculosis be cured? 1.3 Status of receiving health education 1.3.1 Overall status of receiving health education Participants' overall status of receiving health education was shown in Fig. 2 . The proportion of participants who had seen programs about TB on TV in the past year was respectively 41%, 59%, 41% in 2013, 2015 and 2020. While the proportion of participants who had heard programs about TB on the radio was 33%, 46% and 23%, respectively. In the past years, promotional materials on tuberculosis prevention that have been provided to participants mainly consisted of leaflets, folding, publicity pamphlets and pamphlets. About 90% of those who were given these promotional materials had read them. 1.3.2 Analysis of ways to access health education The higher percent participants that did not remember or had not received tuberculosis health education. Medical staff was the major deliverers of tuberculosis knowledge or policies. The proportion of participants who received health education from teachers, classmates and private doctors was low. (Fig. 3 ) 1.3.3 Willingness to receive health education Most participants were willing to accept health education on tuberculosis. 52% of them would to participate the activity if they have time and 45% were willing to participate actively in 2020. Figure 4 showed the willingness to receive health education. 1.3.4 Type of TB publicity materials preferred by participants About 27% of participants said any type of publicity materials were acceptable. Andio-visual product, text-based material and drawing material were preferred by participants. With the popularity of the Internet, the official account of WeChat emerged to publicize TB prevention and control knowledge. Figure 5 illustrated the type of TB publicity materials preferred by participants. 1.4 Associated factors with knowledge of TB among participants. We consider that the respondents answer 3 core question corrected about tuberculosis knowledge are know the knowledge. The univariate study showed that factors associated with participants' awareness of TB in 2020 included occupation (medical staff: OR = 6.40, p < 0.0001, 95%CI: 3.08 ~ 13.31; farmer: OR = 0.56, p = 0.029, 95%CI: 0.33 ~ 0.94; business: OR = 0.67, p = 0.048, 95%CI: 0.45 ~ 1.00) and income (3000 ~ RMB: OR = 2.70, p = 0.016, 95%CI:1.20 ~ 6.07; 5000 ~ RMB: OR = 4.16, p = 0.001, 95%CI: 1.86 ~ 9.29; >10000 RMB: OR = 5.18, p < 0.0001, 95%CI: 2.31 ~ 11.66). After adjusting for possible confounding factors, multivariate analysis showed that the factors associated with participants' awareness of tuberculosis included occupation (medical staff: OR = 6.20, p 10000 RMB: OR = 4.21, p = 0.001, 95%CI: 1.85 ~ 9.59). The knowledge rate was also higher than some areas in china 10–13 and similar to BeiJing 14 . Through the "four networks" publicity network based on the Internet, news and broadcasting network, telecommunication network and interpersonal communication network, health education on tuberculosis was carried out to maintain the knowledge rate at a high level. However, further exploration is needed to increase the knowledge rate. The knowledge of the core information of residents was different. The knowledge rate of questions 2 and 4 was low, indicating that public awareness of the suspected symptoms of tuberculosis and the national reduction policies for screening and treatment was lacking, which should be strengthened in the future. In the past year, promotional materials on tuberculosis prevention that have been provided to participants mainly consisted of leaflets, brochures, posters and pamphlets, while magnetic tapes, compact discs and videotapes had been gradually replaced. Most of those who were given these promotional materials had read them. It suggested that, with the increasingly developed new media, it may be a trend conformed to the development to make good use of new media publicity, such as TikTok and official accounts of WeChat for science promotion 15–17 . Doctors were the main deliverers of tuberculosis knowledge or policies, while the proportion of knowledge acquired by the participants from teachers and classmates was relatively low, Kandasamy’s article [18] also reveal the healthcare students’ knowledge is better than non-healthcare counterparts. Therefore, more teachers and classmates need to be trained as volunteers to participate in TB prevention and control to contribute to end TB strategy. The survey found that only about 5% of the participants indicated that they were unwilling to participate in the tuberculosis publicity activities organized by the work unit or community, There seems to be enthusiasm among participants for TB publicity. Thus we need to hold more diverse forms activities to attract the public engagement. The knowledge rate of medical staff was high, but low among workers and farmers 19 . Income also influenced the knowledge rate, with higher income groups likely to have a higher demand for health-related knowledge than lower income groups 20 . After adjusting for possible confounding factors, multivariate analysis showed that occupation and income were associated with participants' awareness of tuberculosis. Improving the cultural literacy of the whole people is the cornerstone of improving health. Cultural was related to the occupation, especially the low knowledge rate of workers and farmers. Therefore, it is necessary to expand the popularization of TB knowledge to the masses. Low-income people with low knowledge rate are at high risk of TB, so targeted health education activities should be carried out for them 21–23 . We believe that our study has some limitations. First, as it was a cross-sectional study, it is not possible to determine the cause and effect relationships of the associations. Second, with the prevalence of new media, platforms including TikTok and official accounts of WeChat are important means for residents to obtain information. However, there were limitations in the design of relevant questions in this study, resulting in the validity of this approach not being verified, which can be further improved and adjusted in the following research. In conclusion, the knowledge rate of TB prevention and control information of residents has been improved compared with that before. Therefore, targeted publicity activities focused on the suspected symptoms of tuberculosis and the national policies should be further conducted out. The combination of traditional media and new media makes the publicity more diversified. Strengthening health education for workers, farmers and low-income groups to make health education more precise and further improve the awareness level of tuberculosis.
