Long-term influence of transcervical resection of endometrium on ovarian function

Abstract

[Objective] To evaluate the long-term influence of transcervical resection of endometrium (TCRE) on ovarian function. [Methods] The patients were 50 cases, under 45-year-old, who underwent transcervical resection of endometrium and the other 50 cases who underwent hysterectomy with ovarian reserved during the same period because of the uterus benign lesion. Before the operation and at 6 months, 1 year and 2 years after operation, the cases were studied by measuring the serum FSH, LH, E2, P, PRL, T and were examined by transvaginal color doppler to obtain resistance index (RI) and pulsatility index (PI) from ovarian arteries and measured the anteroposterior diameter and the left-right diameter of ovary. At the same time, the cases were scored the climacteric symptom, and were determined the basal body temperature. [Results] In the TCRE group, the serum sex hormone levels, PI, RI, the anteroposterior diameter and the left-right diameter of ovary, the climacteric symptom score and the percentage of duplexing body temperature were not significantly different between pre-operation and post-operation. In the hysterectomy group, there were significant differences between pre-operation and post-operation in the level of E2, P, FSH, LH. At 6 months, 1 year and 2 years after operation, there were significant differences between the hysterectomy group and the TCRE group in the level of E2, P, FSH, LH. In the hysterectomy group, the level of E2, P gradually fell and the level of FSH, LH gradually rose. In the hysterectomy group, examined by transvaginal color doppler, PI, RI rose at 6 months, 1 year and 2 years after operation and the anteroposterior diameter and left-right diameter of ovary fell at 1 year and 2 years after operation. And the result compared with preoperation and compared with the TCRE group were significantly different. In the hysterectomy group, the climacteric symptom score rose and the percentage of duplexing body temperature fell at 1 year and 2 years after operation. And the result compared with preoperation and compared with the TCRE group were significantly different. [Conclusion] In our research, it shows no obvious influence on ovarian function after TRCE during the review time. However our research shows that hysterectomy has influence on ovarian function.
