September 2004 Publication Date: 23 December 2004 (online) Zusammenfassung Die Symptomatik zyklusabhängiger Unterbauchbeschwerden bei Frauen im gebärfähigen Alter lässt immer an eine Endometriose als mögliche Ursache denken. Wir berichten über eine Patientin im Zustand nach vaginaler Hysterektomie mit Morcellement des Uterus, die sich mit vierwöchentlich auftretenden stärksten Schmerzen im Unterbauch vorstellte. Die Diagnostik ergab einen soliden, an der vorderen Bauchwand adhärenten Tumor, der sich bei der operativen Abklärung als Restuterus mit intramuraler Endometriose herausstellte. Im Anschluss an die „Komplettierungs-Hysterektomie“ war die Patientin beschwerdefrei. Abstract Endometriosis is a possible and likely cause of periodically recurrent pelvic pain in premenopausal women. We report on a patient with a history of a vaginal hysterectomy with morcellation of the uterus who presented with monthly recurrent heavy lower abdominal pain. A solid tumour adherent to the lower anterior abdominal wall was diagnosed. Surgery revealed a significant portion of the uterus that showed intramural endometriosis. After “completion hysterectomy” the patient was completely free of pain.
