Abstract

to compare the diagnostic accuracy of sonovaginography and NMR in the diagnosis of recto-vaginal endometriosis. alongitudinal prospective study in which women with recto-vaginal endometriosis suspected with medical history and/or pelvic examination were enrolled. The study group underwent in the same session transvaginal ultrasonography and sonovaginography, the latter performed as previously described. After the examination in a mean period of 60 days patient were scheduled to perform a pelvis NMR. In the moment all the diagnostic tools were performed, patient underwent laparoscopic surgery to enucleate the endometriosic lesion which was measured and sent for pathologic examination. sixty-seven patients were enrolled in the study as showed recto-vaginal endometriosic lesions. The sensitivity of sonovaginography was 92.1%, specificity 75.2%, PPV 96% and NPV 61.1%, receiving operator characteristic (ROC) curve was of 0.88, higher respect to NMR which sensitivity was 74%, specificity 61%, PPV 90% and NPV 34.1%. ROC curve was of 0.68. The procedure was well tolerated with a median visual analogue scale of 2 (range 0-8). Sonovaginography is a well tolerated procedure, costless and with a higher accuracy in the detection of recto-vaginal endometriosis respect of NMR.
