Inflammation Markers at the Beginning and After the Manic Episode in Bipolar Disorder Patients

Abstract

Backround: This study aimed to compare blood-based inflammatory marker levels during manic episodes and early remission in patients with bipolar disorder (BD) and their relationship with the Young Mania Rating Scale (YMRS). Materials and Methods: This retrospective clinical study included 61 patients diagnosed with BD manic episodes who were treated between January 1, 2016, and June 1, 2024, along with 64 healthy control subjects. The control group consisted of individuals aged 18 to 65 years who had undergone a general health check-up at the family medicine outpatient clinic. BD diagnosis was made based on DSM-V criteria. Hemogram values, CRP, and albumin levels were measured within 24 hours of hospital admission and again at discharge. Results: BD patients showed higher levels of CRP, CAR, NEU, and MPV compared to healthy controls (p<0.05), while albumin levels were lower (p<0.000). The monocyte and SIRI values significantly increased after treatment in female but not in male patients. A negative correlation was identified between the YMRS scores and inflammatory markers in females for albumin and monocytes, while in males, a negative correlation was identified for CRP and CAR. Conclusions: These findings highlight the role of systemic inflammation in BD and suggest gender-specific differences in disease characteristics and treatment responses, warranting further investigation.
