Hospital-Based Patterns of Esophageal and Gastric Cancer Cases at a Major Tertiary Referral Center in Kabul, Afghanistan: A Retrospective Case Series

Abstract

Abstract Background: Esophageal and gastric cancers are among the leading causes of cancer-related mortality worldwide, with a disproportionate burden in low- and middle-income countries. In Afghanistan, evidence on these malignancies is limited and largely hospital-based. This study aimed to describe the distribution, demographic characteristics, and histopathological patterns of esophageal and gastric cancers diagnosed at a major tertiary referral hospital in Kabul. Methods: This retrospective hospital-based case series included all patients diagnosed with esophageal or gastric cancer at Jumhoriat Hospital, Kabul, between October 2023 and August 2024. Demographic, socioeconomic, behavioral, and histopathological data were extracted from medical records. Descriptive statistics were used to summarize findings, and the association between sex and cancer type was assessed using the chi-square test. Results: Out of 3,250 registered cancer cases, 917 patients were diagnosed with esophageal or gastric cancer. Esophageal cancer accounted for 708 cases (77.2%), while gastric cancer comprised 209 cases (22.8%). The highest proportion of cases occurred in the 55–64-year age group for both esophageal (28.2%) and gastric cancers (32.0%). Esophageal cancer showed a relatively balanced sex distribution (53.2% male), whereas gastric cancer was significantly more frequent among males (71.2%) (χ² = 20.75, p < 0.001). Squamous cell carcinoma was the predominant histological subtype of esophageal cancer (70%), while adenocarcinoma was the most common subtype of gastric cancer (81.3%). A high proportion of patients, particularly those with esophageal cancer (85.3%), belonged to a low economic status. Conclusions: This hospital-based case series indicates that esophageal cancer constitutes the majority of upper gastrointestinal cancer cases at a major referral center in Kabul, with both esophageal and gastric cancers predominantly affecting individuals aged 55–64 years. The observed demographic and histopathological patterns are consistent with reports from other low-resource settings and provide important baseline data to inform cancer surveillance and early diagnostic strategies in Afghanistan.

Global epidemiological reviews consistently report a substantial burden of these cancers, particularly in low- and middle-income countries, where late diagnosis and limited access to specialized healthcare services remain common challenges ( 1 )( 2 )( 3 ). The epidemiology of esophageal cancer shows marked geographical variation, with high-incidence regions reported across parts of Central and South Asia. This geographic clustering, often described as an esophageal cancer belt, has been attributed to a combination of environmental, lifestyle-related, and population-specific factors rather than a uniform global distribution ( 1 )( 4 )( 5 ). Northern Afghanistan has been included in studies focusing on these high-risk regions, with hospital-based investigations reporting a notable frequency of esophageal cancer among patients evaluated for upper gastrointestinal diseases ( 6 )( 7 ). In addition to geographic variation, differences in esophageal cancer occurrence among ethnic groups living within the same regions have been reported. Studies from northern Afghanistan indicate higher frequencies among certain ethnic populations, including Uzbek and Turkmen groups, suggesting potential associations with environmental exposures and lifestyle practices specific to these communities ( 4 )( 7 ). Gastric cancer also remains a major global public health concern. Although declining trends have been observed in some regions, gastric cancer continues to contribute substantially to cancer-related mortality, particularly in Asian countries. Epidemiological reviews emphasize that late-stage diagnosis remains common and that the disease burden is still considerable in many low-resource settings ( 8 )( 9 )( 10 ). Regional evidence from neighboring countries, especially Iran, has documented significant variation in gastric cancer patterns and has highlighted the relevance of infectious, dietary, and lifestyle-related factors within similar environmental and cultural contexts ( 9 )( 11 ). Across global and regional studies, both esophageal and gastric cancers demonstrate consistent demographic patterns, with higher frequencies reported among males and older age groups. These trends have been widely documented and remain evident despite regional differences and temporal changes ( 3 )( 5 )( 12 ). In Afghanistan, particularly in Kabul, population-based cancer registry data are scarce. Available evidence on esophageal and gastric cancers is largely derived from hospital-based studies conducted in tertiary referral centers such as Jumhoriat Hospital. In the absence of comprehensive population-level data, hospital-based case series play an important role in describing local cancer patterns and informing health system planning. Therefore, this study aimed to describe the frequency and distribution of esophageal and gastric cancer cases diagnosed at Jumhoriat Hospital, a major tertiary referral center in Kabul, Afghanistan. Methodology Study Design and Setting This study was a hospital-based descriptive case series conducted to describe the frequency and distribution of esophageal and gastric cancer cases using routinely collected hospital records. This study was reported in accordance with the STROBE guidelines for observational studies. The study was carried out at Jumhoriat Hospital, a major tertiary referral hospital located in Kabul, Afghanistan. The hospital serves as a central center for cancer diagnosis and management and receives patients from Kabul and other provinces of the country. Study Population and Period The study population consisted of all patients diagnosed with cancer whose medical records were registered at Jumhoriat Hospital during the October 2023 to August 2024. From this population, cases diagnosed with esophageal and gastric cancers were identified and included in the present analysis. Data Source and Data Collection Data were collected retrospectively from patients’ medical records (dossiers) using a structured data extraction form. Extracted variables included demographic characteristics (age and sex), type of cancer, age categories, and drug use as documented in the medical records. Medical records were reviewed systematically. Data extraction was conducted consistently using the same data extraction form, and records were checked for completeness and accuracy prior to analysis. Sampling Method A census approach was applied. All available medical records of patients diagnosed with esophageal and gastric cancers during the study period were included. No sampling technique was used. During the study period, a total of 3,250 cancer records were registered at Jumhoriat Hospital. Among these, 917 records corresponded to patients diagnosed with esophageal and gastric cancers, and these cases constituted the final study sample. Statistical Analysis Data were coded, cleaned, and analyzed using IBM SPSS Statistics version 26 Descriptive statistics were used to summarize the data, including frequencies and percentages. In addition, Chi-square (χ²) test was performed to assess associations between type of cancer (esophageal vs. gastric) and sex. A p-value < 0.05 was considered statistically significant. The results were interpreted descriptively, and no causal relationships were inferred due to the case series design. Inclusion Criteria Medical records of patients diagnosed with esophageal or gastric cancer Records registered within the defined study period Records containing sufficient information for analysis Exclusion Criteria Medical records of patients with cancer types other than esophageal or gastric Incomplete or missing medical records Records registered outside the study period Records with missing key clinical or histopathological data Ethical Considerations This study was conducted after obtaining official administrative permission from Kabul University of Medical Sciences (Abu Ali ibn Sina) and the National Cancer Control Program to review medical records at Jumhoriat Hospital, Kabul, Afghanistan (permission letter No. 344). The study involved a retrospective review of patient file, and no direct patient contact occurred; therefor, informed consent was waived. All extracted data were anonymized prior to analysis to ensure confidentially. Methodological Limitations As a hospital-based case series, the findings of this study may not be generalizable to the wider population. Additionally, reliance on secondary data may have resulted in incomplete documentation for some variables. Results During the study period, a total of 917 patients diagnosed with esophageal and gastric cancers were included in this hospital-based case series. Of these, 708 cases (77.2%) were diagnosed with esophageal cancer, while 209 cases (22.8%) had gastric cancer. The baseline demographic characteristics of the study population, including age, sex, marital status, ethnicity, and province of residence, are presented in Table 1 . The highest proportion of cases was observed among patients aged 55–64 years, followed by those aged 45–54 years and 65–74 years, while younger age groups constituted a relatively small proportion of the study population. A comparable age-related pattern was observed for both cancer types. Detailed age-specific distributions are shown in Table 2 . Most patients were either self-employed or unemployed, and all cases were classified as having a weak economic status. A notable proportion of patients reported drug use, and the majority had a documented history of chronic disease. Body mass index (BMI) data were available for a subset of patients, with normal and underweight categories being the most frequently recorded among those with available measurements. Squamous cell carcinoma was the predominant histological subtype among esophageal cancer cases, whereas adenocarcinoma was the most commonly observed subtype among gastric cancer cases. Other histopathological types were identified in smaller proportions. The histopathological characteristics of esophageal and gastric cancers are presented in Table 4 . The results demonstrated a statistically significant association between sex and type of cancer (χ² = 20.75, df = 1, p < 0.001), with gastric cancer occurring more frequently among males, while esophageal cancer showed a more balanced distribution between sexes. The detailed distribution and results of the Chi-square analysis are shown in Table 5 . In our study, esophageal cancer constituted the majority of cases (77.2%), while gastric cancer accounted for 22.8%. A similar predominance of esophageal cancer has been reported in hospital-based studies from other low- and middle-income countries, where referral bias and late presentation are common ( 13 )( 14 ). In contrast, population-based data from high-income regions often report a higher or comparable burden of gastric cancer, highlighting regional heterogeneity ( 15 )( 16 ). The age distribution in our cohort showed a clear peak in the 55–64-year age group for both esophageal (28.2%) and gastric cancers (32.0%), with very few cases below 35 years of age. This pattern is consistent with international evidence indicating that upper gastrointestinal cancers predominantly affect older adults, reflecting cumulative exposure to risk factors and delayed diagnosis ( 15 )( 17 )( 18 ). Comparable age trends have also been described in regional studies from Asia and Africa ( 13 )( 19 ). Sex distribution differed between cancer types in our study. Esophageal cancer showed a relatively balanced distribution between males (53.2%) and females (46.8%), whereas gastric cancer was significantly more common among males (71.2%), with a statistically significant association between sex and cancer type (χ² = 20.75, p < 0.001). Male predominance in gastric cancer has been consistently reported in international and regional studies ( 19 )( 20 )( 16 ), while a more balanced sex distribution for esophageal cancer has been observed in some high-risk regions, particularly where environmental and household exposures may affect both sexes ( 17 )( 18 ). Histopathological findings in our study further support known regional patterns. Squamous cell carcinoma was the dominant histological type of esophageal cancer (70%), whereas adenocarcinoma was the predominant subtype of gastric cancer (81.3%). The predominance of esophageal squamous cell carcinoma aligns with evidence from Africa and Asia, where this subtype remains the most frequent, in contrast to Western countries where adenocarcinoma is more common ( 15 )( 17 )( 16 ). Similarly, the dominance of gastric adenocarcinoma in our cohort is consistent with reports from hospital-based studies in comparable settings ( 20 )( 21 ). Socioeconomic characteristics showed that a large proportion of patients, particularly those with esophageal cancer (85.3%), belonged to a low economic status. This finding is in agreement with studies linking upper gastrointestinal cancers to social deprivation, poor nutritional status, and limited access to preventive and diagnostic services ( 13 )( 19 )( 18 ). The high prevalence of reported substance use, especially among patients with gastric cancer (71.3%), is also comparable to observations from regional studies, although causal inferences cannot be drawn due to the descriptive nature of the data ( 14 )( 20 ). In summary, this study demonstrates that esophageal and gastric cancers in a major referral hospital in Kabul predominantly affect individuals aged 55–64 years, show a marked male predominance in gastric cancer, and are characterized by squamous cell carcinoma of the esophagus and adenocarcinoma of the stomach. When compared with all referenced studies, the findings are highly consistent with patterns reported from other low-resource and high-risk regions. These results underscore the importance of strengthening early diagnostic capacity and cancer surveillance systems in Afghanistan, while providing robust baseline data for future multicenter and population-based studies. Conclusion In conclusion, this hospital-based case series demonstrates that esophageal and gastric cancers represent a substantial oncological burden among patients attending a major tertiary referral hospital in Kabul. The observed demographic and histopathological patterns are consistent with regional and global evidence and reflect the clinical reality in a low-resource setting. Although population-level inference is not possible, these findings provide valuable baseline data to inform cancer service planning and underscore the need for strengthened cancer registry systems and coordinated multicenter research in Afghanistan. This study aimed to describe the distribution, demographic characteristics, and histopathological patterns of esophageal and gastric cancers diagnosed at a major tertiary referral hospital in Kabul.\u003c/p\u003e\u003ch2\u003eMethods:\u003c/h2\u003e \u003cp\u003eThis retrospective hospital-based case series included all patients diagnosed with esophageal or gastric cancer at Jumhoriat Hospital, Kabul, between October 2023 and August 2024. Demographic, socioeconomic, behavioral, and histopathological data were extracted from medical records. Descriptive statistics were used to summarize findings, and the association between sex and cancer type was assessed using the chi-square test.\u003c/p\u003e\u003ch2\u003eResults:\u003c/h2\u003e \u003cp\u003eOut of 3,250 registered cancer cases, 917 patients were diagnosed with esophageal or gastric cancer. Esophageal cancer accounted for 708 cases (77.2%), while gastric cancer comprised 209 cases (22.8%). The highest proportion of cases occurred in the 55\u0026ndash;64-year age group for both esophageal (28.2%) and gastric cancers (32.0%). Esophageal cancer showed a relatively balanced sex distribution (53.2% male), whereas gastric cancer was significantly more frequent among males (71.2%) (χ\u0026sup2; = 20.75, p\u0026thinsp;\u0026lt;\u0026thinsp;0.001). Squamous cell carcinoma was the predominant histological subtype of esophageal cancer (70%), while adenocarcinoma was the most common subtype of gastric cancer (81.3%). A high proportion of patients, particularly those with esophageal cancer (85.3%), belonged to a low economic status.\u003c/p\u003e\u003ch2\u003eConclusions:\u003c/h2\u003e \u003cp\u003eThis hospital-based case series indicates that esophageal cancer constitutes the majority of upper gastrointestinal cancer cases at a major referral center in Kabul, with both esophageal and gastric cancers predominantly affecting individuals aged 55\u0026ndash;64 years.
