Abstract

Objective: The aim of this study was to compare menstrual pain levels, attitudes toward menstruation, health literacy, and physical activity levels among women from different generations. Methods: This cross-sectional and comparative study included a total of 1,220 women aged between 18 and 55 years. Participants were categorized into three groups according to their birth years: Generation Z (n=380), Generation Y (n=440), and Generation X (n=400). Menstrual pain levels were assessed using the Visual Analog Scale (VAS), while attitudes toward menstruation were evaluated using the Menstrual Attitude Scale (MAS). Health literacy was measured with the Turkish Health Literacy Scale (TSOY-12), and physical activity level was determined based on daily step counts obtained via a smartphone-based pedometer application. Data were analyzed using the Kruskal–Wallis test, Dwass–Steel–Critchlow–Fligner post-hoc test, Spearman correlation analysis, and linear regression analysis. Results: Menstrual pain levels differed significantly across generations (p<0.001), with the highest levels observed in Generation Z. Attitudes toward menstruation also varied between generations, with Generation Z demonstrating significantly higher MAS scores compared to the other groups (p=0.003). In terms of health literacy, Generation X had significantly lower scores than Generations Z and Y (p0.05). Correlation analyses revealed a weak positive relationship between step count and menstrual pain across all generations. Additionally, the relationship between attitudes toward menstruation and menstrual pain was significant only in Generation Y. Conclusion: The findings suggest that menstrual pain, attitudes toward menstruation, and health literacy may vary across generations. Menstrual health should be considered a multidimensional process influenced not only by biological factors but also by generational characteristics and psychosocial factors.
