VP62.01: Ultrasound diagnosis of endometriosis and its coexistence with malignant and non‐malignant tubo‐ovarian masses

Abstract
To determine the prevalence of endometriosis in surgical ovarian specimen, its relationship with malignant and non-malignant tubo-ovarian tumours and its ultrasound diagnosis. Retrospective analysis of anatomopathological ovarian endometriosis, its coexistence with other tumours (malignant and non-malignant) and its ultrasound diagnosis studied in a tertiary referral centre during 2017–2019. During the study period we performed 450 ovarian surgical procedures (cystectomies, oophorectomies or adnexectomies), 103 of them were related to ovarian endometriosis (prevalence: 22.89%). In 27.18% of cases (N:28) endometriosis affected both ovaries. In 10 cases (9.70%) ovarian endometriosis coexisted with other ovarian masses: 4 of them were malignant (2 cases of borderline bilateral endometrioid ovarian cancer; ovarian mixed adenocarcinoma endometrioid and clear cell; and clear cell adenocarcinoma) and 6 of them were benign (2 serous and 1 mucinous cystoadenoma; 2 cystic corpus luteum; 1 hemorragic follicle). All malignant cases were suspected during ultrasound scan. Tubal resection was made in 82 cases. In 42.68% of cases (N:35) we found pathology in fallopian tubes: 24.39% (N:20) had endometriosis and 18.29% (N:15) had chronic or acute inflammation signs. Only the 3 cases of hydrosalpinx were diagnosed during ultrasound scan. Surgery of ovarian endometriosic cysts is a prevalent condition in the context of ovarian surgical procedures. Ovarian endometriosic cysts may coexist with other ovarian benign features that should be looked for in ultrasound scans before surgery. Malignant conditions related to endometriosis are rare but must be ruled out. Fallopian tube pathology must be taken into consideration when studying ovarian endometriosis.
