This retrospective single-center study used a prospectively collected database to assess how often appendectomy specimens were histologically abnormal in 717 consecutive colorectal cancer patients (2015–2021), including 342 patients undergoing left-sided resection with prophylactic appendectomy. Histology showed that 534/576 (92.7%) appendices were normal, while 42/576 (7.3%) had abnormalities including inflammatory pathology, hyperplastic polyps, appendix tumors, and specifically 2/576 (0.3%) cases of appendiceal endometriosis; among the 342 prophylactic appendectomy patients, 10 (2.9%) had a synchronous appendix neoplasm. The authors reported no complications attributable to prophylactic appendectomy.
