Endométriose et auto-formation des internes de médecine générale de l’Université de Bordeaux : analyse des portfolios d’une promotion d’interne

Introduction: endometriosis is a frequent disease with a high public health cost and a diagnosis delay of between 5 and 10 years. Numerous studies have noted a lack of knowledge on the part of general practitioners (GP) as a barrier to diagnosis and a factor limiting the proper management of patients. The aim of the study was to examine the approach to endometriosis by family medicine residents (FMR) and the problems reported. Method: cross-sectional, retrospective and descriptive study, by analysis of the portfolios of the FMR of the class of 2017 at the University of Bordeaux. Written learning traces (WLT) extracted from the computer database using the keyword "endometriosis". Data collected anonymously according to a pre-established reading grid. Results: out of the 3060 WLT of the promotion, we extracted 35 WLT after application of the exclusion criteria, i.e. 1.1% of the total. Nearly 50% of the situations were encountered in GP practices. 90% of the residents reported a lack of overall knowledge about endometriosis, including insufficient university training. One third had difficulties in relieving patients of acute and chronic pain. One in five residents felt that the official recommendations were insufficient and more than 10% were not comfortable with the clinical gynaecological examination. The research work in the literature enabled them to acquire the essential elements of the diagnosis and to learn about pain management and the overall care of patients. Conclusion: like practising GPs, nearly 90% of the FMR in our study felt that they lacked knowledge about endometriosis. They consider their university training and the official recommendations to be insufficient. It now seems essential to reinforce the teaching of endometriosis during medical studies.
