Results At follow-up, 28.2% of patients had developed postoperative complications. The most frequent complication was surgical site infection (SSI) (25.4%) followed by, surgical site hematoma (4.2%) then cutaneous sinus tract (2.8%).Sensitivity analysis showed that defect size, visceral fat volume, and subcutaneous fat volume can significantly predict postoperative complications with cut-off values 2.9 cm, 125 cm 3 and 301 cm 3 with sensitivity 100%, 79%, 73.7% and specificity 79%, 65%, and 65% respectively. Conclusion Visceral fat volume (VFV) was significantly associated with postoperative complication following hernioplasty operation. This study suggests VFV as a risk assessment tool for patients undergoing hernioplasty. ( 8 ) A high body mass index (BMI) increases the likelihood of postoperative complications and hernia recurrence following abdominal wall operations. However, BMI doesn't give precise details on the amount and distribution of adipose tissue ( 2 ). Studies that have been published on the relation between visceral obesity and postoperative results have shown that visceral obesity is a better indicator of complications after a gastrectomy than body mass index. ( 6 ). Furthermore, according to ( 11 ), visceral obesity (VO) is associated with worse outcomes following colorectal and pancreatic cancer surgery. . ( 12 , 15 ), suggesting that visceral fat could be a significant clinical factor to consider while evaluating various types of surgeries. ( 2 ). Since body mass index (BMI) is based on anthropometric measurements, it cannot reveal details about the topographic distribution of adipose and lean tissue ratios. An increased risk of incisional hernia (IH) formation following colorectal surgery is linked to visceral obesity (VO), which can be quantified as the visceral fat volume (VFV) or the visceral fat area (VFA) from a single CT scan through the trans-umbilical plane. ( 1 , 17 ). Methods Study population and design We prospectively enrolled 71 patients who were candidates for hernioplasty operation. Patients had a mean age of 48 ± 14 years, males represented most of this population accounting for 77.5%. The study was performed at the CT unit at Kasr Al-aini hospital, during the period from May 2023 to December 2023. Approval of the Research Ethics Committee and informed consent were obtained from all participants in the study. Privacy of all patient data was guaranteed. The full socio-demographic and clinical data including age, sex, (BMI), smoking, medical history and past history of related significance. Informed consent Including procedure description and benefits was obtained. The exclusion criteria included: Insufficient CT quality, absolute contraindication for CT scan, e.g. pregnancy and patients refusing to participate. Imaging procedure Patient’s Preparation The aim of the patient preparation is to familiarize the patient with the procedure, to make the procedure as comfortable as possible and to stress the importance of minimizing the motion Position is supine (feet first) and asked to hold his breath during the examination Equipment: MSCT scan was performed by using 16 channels MSCT helical “GE revolution Evo 128 slice” machine in Radiology department, Faculty of medicine, Cairo university hospitals. Low dose MSCT scan is obtained with 1.25 mm slice thickness and 1.5 mm slice gap. Post processing was performed by using “GE AW volume share 7” workstation CT Imaging Protocol Non contrast enhanced CT scan of the abdomen and pelvis, including an acquisition at third lumbar vertebrae. In some cases the oral contrast material was administrated as a part of their preoperative assessment. Imaging analysis The morphological CT features including site, type, content, defect size and associated hernia complications. All these criteria were independently reviewed by two experienced radiologists for the documentation of these findings. Post processing Post processing of the volume axial CT images is then performed on the workstation without need for further patient stay in the CT machine. Post processing entangles manual delineation of the edges of visceral fat as well as subcutaneous fat (after exclusion of bowel, vessels, anterior abdominal wall muscles (anteriorly) as well as vertebral body and paraspinal muscles (posteriorly)) on every slice along L3 vertebral body using 3D reconstructed images, then visceral and subcutaneous fat volume are calculated automatically after applying regions of interest (ROI) on “GE A`W volume share 7” workstation. (Fig. 1 ) Post-operative patient evaluation Patients were followed up for postoperative 30-days duration for any early postoperative complications including surgical site infection, seroma or cardiopulmonary complications. Clinical and imaging data of the complicated cases were collected. Statistical Analysis Statistical analysis was conducted using SPSS 27th edition, categorical variables were presented in count and percent, and compared between groups using Chi2 test. Quantitative variables were presented in mean, standard deviation, minimum and maximum, while it was compared using Mann Whitney U test after normality testing by Shapiro Wilk test. Sensitivity analysis was conducted to estimate the optimum cutoff point for prediction of postoperative complications along with its sensitivity, specificity and area under the curve, logistic regression model was used to estimate the odds ratio for development of postoperative complications among the included patients. Any p value < 0.05 was considered significant. Results Socio- demographic and clinical data Among all study population 55 (77.5%) males, 16 (22.5%) females, their ages ranged between 34–62 years (mean age 48 ± 14 years). ( Fig. 2 ) Hypertension was the commonest reported comorbidity accounting for 31% followed by 9% cardiac diseases, and 7% diabetes. Smoking was reported by 57.7% of patients. Inguinal hernias were the most common type accounting for 66.2%, 12 (16.9%) patients had previous hernia, while 17 (23.9%) patients had previous abdominal surgeries. Complications were associated with higher defect size (2.5 ± 0.77 vs 4.17 ± 1.72 cm), higher visceral fat content (122 ± 85 vs 368 ± 272 cm3), and higher subcutaneous fat content (305 ± 219 vs 539 ± 368 cm3) with p values < 0.001, < 0.001, and < 0.001 respectively. The defect size, and visceral fat volume are independent predictors for postoperative complications with odds ratio 4.317 (95% CI 1.245–14.967), and 1.01 (95% CI 1-1.017) after adjustment for BMI and subcutaneous fat content. ( Table 6 ) they can significantly predict postoperative complications with cutoff values 2.9 cm, 125 cm 3 , and 301 cm3 with sensitivity 100%, 79%, 73.7% and specificity 79%, 65%, and 65% respectively, with p values < 0.01 and AUC 0.884, 0.856, and 0.75 respectively. ( 14.18) Traditionally, body mass index (BMI) is used to measure obesity. Researches has linked elevated BMI to hernia recurrence following AWR. ( 7 ). Since BMI is an anthropometric measure, it cannot reveal information on the topographic distribution of adipose and lean tissue ratios.( 2 ) As a reliable substitute for BMI, computed tomography (CT) measures accurately body composition. (4,14.18) Visceral obesity (VO), measured by either the visceral fat area (VFA) from a single CT slice through the trans-umbilical plane or as the visceral fat volume (VFV) with an increased risk of IH formation after colorectal surgery. ( 1 , 17 ) From May 2023 to December 2023 our cohort study involved 71 patients who patients underwent hernioplasty operation. The mean age was 48 ± 14 years, males represented most of this population accounting for 77.5% of the included patients. All patients underwent a preoperative non-enhanced abdominal CT scan as a part of the preoperative planning. Measurements were done by manual delineation of the visceral fat as well as subcutaneous fat on every axial slice along 3rd lumbar vertebral body (L3) using 3D reconstructed images. Patients were followed for 30 days for detection of early post-operative complications including surgical site infection, seroma and venous thromboembolism. Clinical and imaging data of the complicated cases were collected. In our study, the incidence of early (30 day) postoperative complications was (28.2%), the most frequent complication was SSI (n = 18) followed by, surgical site hematoma (n = 3) then cutaneous sinus tract (n = 2) In agreement with ( 3 ) which found that (26%) of total included patients developed a postoperative complication following Incisional Hernia Repair in Abdominal Transplant Population within the first 30 days. In the present study we found that visceral fat is a strong predictor of postoperative complications following herniplasty while it is the only predictor of postoperative complications following abdominal wall reconstruction surgeries using cutoff value 125 cm 3 with sensitivity 79%, while specificity 65% in hernioplasty as well as abdominal wall reconstruction surgeries with cutoff value 445 cm3 with sensitivity 81.3%, and while specificity 80%, respectively. Concordant with ( 2 ), that demonstrated positive association between increased VFV development of SSO after AWR. These results are also matched with ( 6 ) reported that visceral obesity (VO) to be superior to BMI in the prediction of complications after laparoscopic gastrectomy. And also with ( 11 ) which found that visceral obesity is associated with worse outcomes after surgery for pancreatic, indicating that VO may be an important clinical parameter in the preoperative assessment of several types of surgery. In the present study we found that complications in the hernioplasty groups was more prevalent among hypertensive patients (p 0.031), incisional hernias (p < 0.001) previous hernias (p 0.031), and previous abdominal surgeries (p 0.002). While In abdominal wall reconstruction group we detected that complications was more prevalent among diabetes, hypertension, and smoker patients ,also incisional hernias, previous hernia and previous abdominal surgeries p values(< 0.001) (0.002), (< 0.001), (0.024) ,( 0.014) and (0.004) respectively. In concordance with (Cheema et al., 2021) which reported that history of diabetes and open abdomen are associated with SSO/SSIs, while hypertension is associated with recurrence in abdominal organ transplant patients. This is consistent with prior literature that patient selection plays an important role in hernia surgery. Also in agreement with ( 13 ) which found that a significant relationship between diabetes mellitus and smoking and the occurrence of complications in patients with incisional hernia repair Our results demonstrated that no association between BMI and occurrence of early postoperative complications following hernioplasty– probably due to low BMI among the included participants. In acceptance with ( 2 ) which demonstrated that there is no such association between BMI and occurrence of post-operative complications or hernia recurrence following abdominal wall surgeries ( AWR) Mismatched with ( 3 ) which concluded (BMI) > 35 kg/m2, was associated with surgical site occurrence / infection (SSO/I) (P < .05) Our study also found that the defect size of abdominal wall hernia, can significantly predict postoperative complications following herniopasty using cutoff value 2.9, with sensitivity 100% and specificity 79%. In agreement ( 5 ) which reported that defect width larger than 10 cm and reoperation within 90 days were factors independently associated with operation for hernia recurrence following incisional hernia repair, as well as ( 13 ) which found a significant relationship between defect size and the occurrence of complications in patients with incisional hernia repair. We detected that subcutaneous fat contents can also be used as a predictor postoperative complications following hernioplasty using cutoff value 301 cm3 with sensitivity 73.7%, while specificity 65%. In agreement with ( 16 ) which revealed subcutaneous fat volume per 1000 cm3 (p = 0.034, OR = 0.26) was significant predictor of SSI following complex ventral hernia repair. Limitations of this study First, our study has some limits such as being a small study cohort, done at single-institution. Second, we only had partial follow-up data for a short time; therefore, long-term outcomes, such as hernia recurrence, could not be determined. Also, the small sample size, which may have caused selection bias. Furthermore the Limited area of measurement of body fat volume, along L3 vertebral body (due to lack of automated protocols in our institute), which is sometimes affected with structural pathology (i.e.: fracture). Finally, relative inaccessible some of intra-operative and post-operative recovery data as well as Lack of documentation in our institution. Conclusion We concluded that visceral fat volume (VFV) was significantly associated with postoperative complication following hernioplasty operation. This study suggests VFV as a risk assessment tool for patients undergoing hernioplasty.
