Machine Learning-based Mortality Prediction for Pediatric Fulminant Myocarditis Using Cytokine Profiles

This retrospective study analyzed 21 pediatric fulminant myocarditis cases from two tertiary centers (2012–2022) to identify mortality-associated features by combining admission cytokine profiles (37 cytokines) with 14 clinical variables using partial least squares discriminant analysis and leave-one-out cross-validation. Among 51 features, 23 had high variable importance, and six cytokines (TNF-α, M-CSF, MIP-1α, IL-8, IL-6, and IL-15) were both statistically significant and highly important, while elevated CK-MB and lactate and lower pH were also linked to poor outcomes. The model showed robust discrimination (AUC 0.92) with sensitivity of 92.9% and specificity of 71.4%, with significance control using Benjamini–Hochberg FDR 0.05. The study’s key limitation is its small sample size and retrospective design.
