Abstract

Introdução: A doença inflamatória pélvica (DIP) constitui importante causa de morbidade ginecológica e reprodutiva, podendo resultar em infertilidade tubária, gestação ectópica, dor pélvica crônica e abscesso tubo-ovariano. Objetivo: Analisar as principais atualizações relacionadas à epidemiologia, etiologia, diagnóstico, tratamento, prevenção e complicações da doença inflamatória pélvica. Metodologia: Trata-se de uma revisão narrativa da literatura realizada nas bases SciELO e PubMed, incluindo estudos publicados entre 2016 e 2026, em português, inglês e espanhol. Foram identificadas 39 publicações na SciELO e 57 no PubMed. Após a aplicação dos critérios de elegibilidade, análise dos textos selecionados e exclusão de estudos não pertinentes, 28 estudos compuseram a amostra final. Resultados: O diagnóstico da DIP permanece predominantemente clínico, enquanto exames laboratoriais e de imagem apresentam papel complementar, sobretudo na investigação de complicações e diagnósticos diferenciais. Evidenciou-se etiologia polimicrobiana, com participação de Chlamydia trachomatis, Neisseria gonorrhoeae, Mycoplasma genitalium, anaeróbios e microrganismos associados à vaginose bacteriana. O tratamento empírico precoce e de amplo espectro permanece fundamental, destacando-se a associação entre ceftriaxona, doxiciclina e metronidazol. O rastreamento de infecções sexualmente transmissíveis, o tratamento das parcerias e a prevenção de reinfecções são essenciais para reduzir recorrências e sequelas. Conclusão: O manejo da DIP exige elevada suspeição clínica, tratamento oportuno e acompanhamento adequado. Novos testes moleculares, biomarcadores e estratégias direcionadas à resistência antimicrobiana apresentam potencial para aprimorar o diagnóstico e individualizar a terapêutica.
