This case report describes a patient presenting with cyclical right-sided leg pain that was ultimately diagnosed as lumbosacral plexopathy caused by sciatic endometriosis. The authors highlight that while endometrial tissue typically involves the pelvis, ovaries, or bowel, it can rarely infiltrate neurovascular bundles in the legs. This unusual manifestation demonstrates how ectopic endometrial implants can affect distant nerves and cause catamenial symptoms outside the pelvic region. This paper is centrally about endometriosis — specifically an atypical presentation involving sciatic nerve infiltration leading to lumbosacral plexopathy.
