Effects of different drugs and medication times on recurrence of endometriosis after laparoscopy

Abstract

Objective: To explore the effects of different drugs and medication times of triptorelin and gestrinone on recurrence of ovarian endometriosis after laparoscopic conservative surgery. Methods: A total of 345 patients with ovarian endometriosis underwent laparoscopic conservative surgery,and then they were divided into five groups according to postoperative medication situations: control group( 92 patients),three- month-GnRH- a treatment group( 77 patients),six- month- GnRH- a treatment group( 70 patients),three- month-gestrinone treatment group( 53 patients),and six- month- gestrinone treatment group( 53 patients). The recurrence situations and times in the five groups were observed and compared,the annual recurrence rates and accumulative recurrence rates were calculated in the five groups. Results: The accumulative recurrence rates on the first year,the second year,and the third year after surgery were 8. 69%, 20. 58%,and 23. 12%,respectively; the mean accumulative recurrence rate in treatment groups was 17. 78%,which was statistically significantly lower than that in control group( 38. 04%)( P 0. 05); the three- year accumulative recurrence rates in three-month-GnRH-a treatment group( 22. 07%),six- month- GnRH- a treatment group( 10. 00%),and six- month- gestrinone treatment group( 11. 32%) were statistically significantly lower than that in control group( P 0. 05); there was no statistically significant difference in the three- year accumulative recurrence rate between three- month- gestrinone treatment group( 28. 30%) and control group( P 0. 05); the three- year accumulative recurrence rate in six- month- GnRH- a treatment group was lower than that in three- month- GnRH- a treatment group, and the three- year accumulative recurrence rate in six- month- gestrinone treatment group was lower than that in three- month- gestrinone treatment group,there were statistically significant differences( P 0. 05). The postoperative recurrence times in six-month-GnRH-a treatment group and six- month- gestrinone treatment group were( 20. 00 ± 4. 61) months and( 19. 17 ± 4. 07) months,respectively, which were statistically significantly lower than that in control group [( 13. 43 ± 3. 01) months]( P 0. 05); the postoperative recurrence times in three- month- GnRH- a treatment group and three- month- gestrinone treatment group were( 15. 88 ± 5. 83) months and( 15. 20 ±5. 43) months,respectively,compared with control group,there was no statistically significant difference( P 0. 05); the postoperative recurrence time in six- month- GnRH- a treatment group was lower than that in three- month- GnRH- a treatment group,and the postoperative recurrence time in six- month- gestrinone treatment group was lower than that in three- month- gestrinone treatment group, there were statistically significant differences( P 0. 05). Conclusion: The mean application times of GnRH- a and gestrinone after surgery less than three months can't delay the recurrence,but GnRH- a can reduce the recurrence rate,gestrinone can't reduce the recurrence rate; applying GnRH- a and gestrinone for six months after surgery can reduce the recurrence rate and delay recurrence to a certain degree.
