Evaluation of ovarial reserve in patients with ovarian endometriosis and infertility

The study evaluated ovarian reserve in 100 patients with ovarian endometriosis and infertility (main group), using ultrasound counts of antral follicles and, in 55 of these patients, serum anti-Müllerian hormone (AMH) levels; results were compared with 70 infertility patients without endometriosis (control group). Women with ovarian endometriosis and infertility had fewer antral follicles than controls in both right and left ovaries, with values that decreased with age and were lower in both primary and secondary infertility. AMH levels were similar between groups (3.5 ng/mL vs 3.3 ng/mL), and no correlation was found between AMH level and the “endometrioma” variable. The authors conclude that antral follicle counts are not a reliable marker of ovarian reserve in this setting, attributing this to difficulties counting follicles due to altered ovarian anatomy from endometriomas.
