Pregnant women should be provided with relevant and useful information to cope up with this specific period of their lives. Excessive worry can affect maternal physiological and psychological state that influences the pregnancy outcomes. Access to reliable pregnancy related information from the right sources reduces pregnancy related worries. This study assessed pregnancy related worry and associated factors among pregnant women attending ante natal care. Methods A cross-sectional study using a systematic random sampling method was conducted on 422 pregnant women who came for antenatal care follow up at Abebech Gobena MCH hospital from June 1, 2023 to September 30, 2023. The socio-demographic and obstetric characteristics, health information status, health care service utilization and social factors were obtained and pregnancy related worries were measured using Cambridge Worry Scale. Descriptive statistics was used to see frequency of independent variables. Simple logistic regression analysis was computed and all independent variables with p-value less than 0.25 were entered in multivariate logistic regression. Multivariate logistic regression was fitted to identify the factors associated with pregnancy related worries. Result From total of 422 pregnant women using Cambridge worry scale, 54.7% of the respondent women had pregnancy related worries and the rest 45.3% had no worries. The odds of pregnancy related worry among pregnant women were higher in women from rural area [AOR = 2.78; 95% CI: 1.24, 6.25], who were unable to read and write [AOR = 0.30 95% CI: 0.10, 0.92], who had previous pregnancy complication [AOR = 0.39; 95% CI: 0.16, 0.96], women living in more than five km distance [AOR = 2.45; 95% CI: 1.22, 4.90], who used digital source of information [AOR = 6.98; 95% CI: 2.00, 24.40] and low social support [AOR = 0.08; 95% CI: 0.01, 0.44]. Conclusion and recommendation pregnancy related worries were prevalent in women attending ante natal care and significantly associated with women’s place of residence, educational status, previous pregnancy complication, distance from the health facility, source of trusted source of information, and social support. Pregnant women are preoccupied with fetal growth and future duties, making them vulnerable to a variety of psychological issues such as mood swings, exhaustion, emotional disorders, and mixed anxiety-depressive disorder. Pregnancy-related anxiety is a common problem among women during and after pregnancy and negatively affects women’s physical and mental health and their children’s cognitive, emotional, and behavioral development. Thus, it is linked to a wide range of physical, emotional, and social changes ( 1 )( 2 ). Pregnancy is a time in a woman's life when she is filled with the joyful anticipation of having a baby, which can be influenced by a variety of psychological pressures. According to research on the psychological status of pregnant women, each stage of pregnancy offers potential stresses that might cause a variety of concerns ( 3 ). Some studies found higher levels of worry in the first and third trimesters of pregnancy, while others found that each facet of a pregnant woman's worries fluctuated throughout the pregnancy ( 4 ). Several studies have found that anxieties about the chance of losing the baby, the infant's health, and childbirth are widespread among pregnant women. Aside from this type of anxiety, there are additional sources of stress in women's daily lives (for example, worry about money, employment, housing, health, and marital relationships). Most women have modest worries throughout pregnancy; however, some women may have pathological worries, which are defined as uncontrollable negative thoughts and excessive fear about future occurrences in pregnancy that can cause anxiety. Worries are a cognitive component of anxiety that have a significant positive impact on pregnancy. Foetal health, foetal loss, childbirth, the mother's well-being, body image, parenting and child care, financial, family, and social support are all cognitive dimensions of pregnancy-related worry ( 5 ). Pregnant women who like to receive required information from a health care professional in person can significantly contribute to reducing worries. Although information from health care professionals is the most important source of information, many women demand more information due to the prolonged gap between antenatal care visits ( 6 ). Being exposed to several sources may increase the possibility of receiving contradictory information. This is compounded by the fact that different sources of pregnancy-related information might lack the core knowledge and skills for proper navigation and interpretation ( 7 ). On the other hand, there remains doubt about the quality and reliability of pregnancy-related health information. This could also cause concerns or feelings of worry, and pregnant women ought to be able to cope with such feelings ( 8 ). The Cambridge Worry Scale (CWS) ( 9 ) is a validated tool designed to measure pregnant women's fears and concerns. The woman rates each of the 16 items on the measure on a scale of 0 to 5, where 0 indicates little worry and 5 indicates significant worry ( 10 ). Psychometric validation of the Cambridge Worry Scale has been achieved in research from the UK ( 10 ), Germany ( 11 ), Spain ( 12 ) and Turkey ( 13 ). Most research has shown that women are most concerned about giving birth, the possibility that something is wrong with the unborn child, and the possibility of miscarriage. In the Spanish study, having an unplanned pregnancy and expecting a child for the first time were linked to increased anxiety ( 12 ). Pregnancy is a period of physical change in a woman’s life when health-related behaviours are most vital because of the lives of the mother and the foetus; hence, the pregnant woman requires a continuous flow of maternal information. As a result, maternal health information is a fundamental building block of women’s health during pregnancy, childbirth, and the postpartum period. It can help identify existing health risks in pregnant women and foil potential health troubles for women and their children( 14 ). Exploring pregnancy-related worries and associated factors among pregnant women has become of interest to the health system, organizations working on maternal health, and researchers. This study aimed to investigate pregnancy-related worries and associated factors among pregnant women and to provide new insights regarding the pregnancy-related worries of women attending an antenatal clinic, which would enable them to make more informed decisions. Furthermore, the findings of this study would be used to make efficient information delivery programs available to women attending antenatal clinics, which would enable them to have easy access to maternal information relating to their information needs . 2. Methods and materials A cross-sectional study design was conducted on 422 pregnant women aged above 18 years and attending ANC follow-up from June 1, 2023, to September 30, 2023, at Abebech Gobena MCH Hospital, Addis Abeba, Ethiopia. Ethical clearance was obtained from the Institutional Review Committee of Yekatit 12 Hospital Medical College. After the purpose and objective of the study had been informed, verbal consent was obtained from each study participant. All participants were also informed that participation was voluntary and they could withdraw from the study at any time if they were not comfortable with the questionnaire. Source and study population The study recruited all pregnant women aged over 18 and attending ANC follow-up at Abebech Gobena MCH Hospital during the study period. The study took place between June 1, 2023, and September 30, 2023. Sample size determination The sample size was determined by using a single population proportion formula with the following statistical assumptions: P (50%, proportion of pregnant women who had pregnancy-related worries since there was no previous study), d (the permissible margin of error of 5%), and Zα/2 (the value of the standard normal curve score corresponding to the given confidence interval = 1.96) corresponding to a 95% confidence level. n = n = \(\:\frac{\left(\mathbf{Z}\varvec{\alpha\:}∕2\right)2\:\mathbf{x}\:\mathbf{p}(1-\mathbf{p})}{\left(\mathbf{d}\right)2}\) n = \(\:\frac{\left(1.96\right)2\:\mathbf{x}\:\left(0.5\mathbf{x}0.5\right)}{\left(0.05\right)2}\:\) = 384 After adjusting for the total study population and 10% of the potential non-response rate, the final sample was 422. The sample size employing factors would not be computed because there were no studies carried out in Ethiopia or other related locations. Sampling procedures A study participant was selected using a systematic random sampling method. Based on the ANC registration, on average, every month, about 1033 pregnant women attended ANC follow-up at Abebech Gobena MCH Hospital. In the present study, the data collection took place over four months. As a result, the sampling interval “K” value was calculated as K = N/n, considering N = the expected number of pregnant women attending ANC at Abebech Gobena MCH Hospital per four months = 4132 and n = sample size = 422; K = 4132/422 ≈ 10, and this gives a sampling interval of ten. Thus, study subjects were selected at ten intervals, and the first participant was selected by the lottery method. Variables of the study Pregnancy-related worries are our dependent variable, while socio-demographic factors (age, sex, marital status, residence, income, educational status, and occupational status), obstetric-related factors (gravity, trimester, history of abortion, stillbirth (IUFD), complications in previous pregnancies), health service factors (accessibility of service, health worker behaviour, and health service quality), healthcare service utilization (ANC use, counselling during ANC, information given at counselling), and behavioural factors (social support) Data collection procedures The data was collected using an interview-administered, pretested, and structured questionnaire prepared by the investigator after reviewing different literature (3, 6, 9, 35). The data collection tool was prepared in English and translated into the local language, Amharic, by the language expert, and then translated back to English by language experts to check for consistency. The translated Amharic tool was used for data collection. The questionnaire was composed of seven sections, including socio-demographics of the participants, obstetric-related history, health service utilization and related factors, behavioural factors, sources of information, and the Cambridge worry scale. Four BSc midwives and two physicians took part in data collection and supervision, respectively. Two days of intensive training were given on the overall objective of the study, ethical issues (getting consent, privacy, confidentiality, and voluntary participation), interview techniques, and the objective of each question and related probes. During the training, the data collector performed a small demo. A pre-test was done on 5% of the total sample size at St. Peter Hospital. Operational definitions Pregnancy the period in which a foetus develops within the female body or uterus. Pregnancy-related worries are measured using the summation of 17 items on the Cambridge worry scale and taking the mean value as the cut-off point for determining low or high pregnancy-related worries. Pregnancy-related information are information sources used by women during pregnancy to meet their information needs regarding pregnancy. Pattern of information source classified as professional and informal (conventional and digital) sources of information. Professional sources midwives, obstetricians, general practitioners, leaflets from care providers, websites from midwives and hospitals, and information meetings organized by midwives and hospitals. Informal sources are divided into conventional sources (family and friends, peers, books, and journals) and digital sources (apps, websites about pregnancy and childbirth, forums and blogs, social media, and TV programs) ( 15 ). The Cambridge Worry Scale (CWS) is a validated instrument for measuring women’s concerns or worries during pregnancy. The instrument consists of 17 items, and the woman estimates each aspect of worry on a scale ranging from 0 to 5, where zero is no worry and five is major worry ( 12 ). Data quality assurance To ensure the quality of the data, two-day training was given to the whole research team before the actual fieldwork with adequate practice, during which the final refinement of the tools was made. In addition, a pre-test was done at St. Peter Hospital on 5% (21 subjects) of the sample size before actual data collection. A necessary amendment was made upon identification of the ambiguity of the questionnaire. Data analysis procedures Data was entered into Statistical Package for Social Sciences (SPSS) version 26, and then data cleaning and coding were done to make it ready for analysis. Descriptive statistics were used to see the frequency distribution, mean, and standard deviation. Multi-collinearity was checked using variance inflation factors (VIF). Bivariate logistic regression analysis was computed, and all independent variables with a p-value less than 0.25 were entered into multivariate logistic regression. Multivariate logistic regression was fitted to identify the factors associated with pregnancy-related worries. Variables with a p-value less than 0.05 at a 95% confidence interval were considered statistically significant. 3. Result Demographic and obstetric characteristics of pregnant women A total of 422 respondents completed the questionnaire. The respondents were aged between 18 and 43 years, with most of the respondents being between 26 and 30 years old (35.8%) and 21 and 25 years old (33.2%). More than half of the respondents reported their marital status as married (60.4%), and their religion was orthodox (59.5%). Around 29.6% of the respondents had completed primary school, and nearly half (44.8%) of the respondents were housewives (Table 1 ). Nearly half of the respondents (41.9%) were in their third trimester of pregnancy, and 26.3% of the respondents reported having a pregnancy-related complication in their previous pregnancy. (Table 2 ) Sources of health information More than half (53.3%) of the women had less frequent media use (listening to radio, watching television, reading newspapers, and using social media) as sources of pregnancy-related information. Around two-thirds (64.1%) of the respondents depend on a professional source for information, followed by a digital source (22.8%), while the least common source of information is a conventional source (13.1%). More than half (56.6%) of the respondent women had high information-seeking status. Around 33.9% of the women in this study needed information on pregnancy complications and danger signs, while 24.9% needed information on foetus health and development, and 14.7% needed information on medicines in pregnancy (Table 3 ). Health service factor and health care service utilization More than half of the women (55.5%) live less than 5 km from a health facility. 43.8% of the women responded that health service quality was good, and 33.4% responded that service quality was very good. More than half of the women (57.8%) expressed their health professional approach as welcoming, and 28.9% as very welcoming (Table 4 ). Almost two-thirds of the respondents, women (60.2%), had less than three ANC visits during the time of the questionnaire, and two-thirds of the women got counselling at their ANC visit. From health information given at counselling, more women receive information about complications in pregnancy (36.0%). (Table 5 ) Pregnancy-related worries From a total of 422 pregnant women, 54.7% of the respondents had pregnancy-related worries, and the rest, 45.3%, had no worries (Fig. 1 ). In Table 6 , a detailed presentation of the percentage of scores on the 17 items of CWS is shown. The highest concern was the possibility of something being wrong with the baby, with a mean score of 2.87 (2.49–2.87, 95% CI). The other highest worries were being with a partner at birth (mean 2.70), giving birth (mean 2.68), and money problems (mean 2.64). The lowest mean scores were problems with the law, relationships with family and friends, and relationships with partners. Factors associated with pregnancy-related worry In the bivariate logistic regression, place of residence, marital status, religion, educational status, living arrangement, history of stillbirth, previous pregnancy complications, distance from a health facility, perceived quality of health service, source of trusted information, social support, and number of ANC visits were significant variables at a p-value cut-off point less than 0.25. In multivariable logistic regression, place of residence, educational status, previous pregnancy complications, distance from the health facility, source of trusted source of information, and social support were factors significantly associated with pregnancy-related worry among pregnant women attending ANC at Abebech Gobena MCH Hospital. The odds of pregnancy-related worry among pregnant women from rural areas were 2.78 times higher compared with pregnant women living in urban areas [AOR = 2.78; 95% CI: 1.24, 6.25]. Pregnant women with diplomas and higher educational status have 70% lower odds of pregnancy-related worry than pregnant women who were unable to read and write [AOR = 0.30, 95% CI: 0.10, 0.92]. Pregnant women who have no history of previous pregnancy complications had 61% lower odds of pregnancy-related worry as compared to their counterparts [AOR = 0.39; 95% CI: 0.16, 0.96]. The odds of pregnancy-related worry among pregnant women living more than 5 kilometres from the health facility were 2.45 times higher compared with pregnant women living less than 5 kilometres from the health facility [AOR = 2.45; 95% CI: 1.22, 4.90]. The odds of pregnancy-related worry among pregnant women who used digital sources of information as trusted sources were 6.98 times higher compared with pregnant women who used professional sources of information [AOR = 6.98; 95% CI: 2.00, 24.40]. Pregnant women who have high social support had 92% lower odds of pregnancy-related worry as compared to pregnant women who have low social support [AOR = 0.08; 95% CI: 0.01, 0.44]. 4. Discussion The current study investigated pregnancy-related worries among expectant women and associated factors in Abebech Gobena Hospital, Addis Abeba. The main finding of this study revealed that more than half of pregnant women in this study had pregnancy-related worries, and women’s worries during pregnancy were mostly related to pregnancy and birth, social factors, pregnancy-related health information, and health service factors. Using the Cambridge worry scale (CWS), this study showed that the highest worries of expectant women were the possibility of something being wrong with the baby, giving birth, money problems, and being with a partner at birth. In line with this study, several studies have found that concerns about the possibility of losing the baby, the baby's health, and childbirth are common among pregnant women. The finding of this study is similar to other studies using CWS, where worries about a baby's health and giving birth ranked as the first and second worries ( 12 ), ( 16 ), ( 17 ), and ( 17 ). Unlike this, a cross-sectional study done in Iran assessing CWS revealed the most prevalent causes of worries in pregnant women were having nobody in the delivery ward ( 5 ). In addition to this type of concern, women also worry about money, going into labour early, and housing. The lowest worries in this study were problems with the law, relationships with family and friends, and relationships with partners. This is also similar to some studies using CWS ( 18 ), ( 19 ), and ( 17 ). This study showed that women need a variety of health information during the term of their pregnancy. The most frequently reported information needs were related to pregnancy complications and danger signs, information on foetus health and development, and medicines in pregnancy. In line with this study, a study conducted in Kerman, Iran, stated that the most frequent information needs among mothers are care of the foetus, development and growth of the foetus during different stages of pregnancy, nutrition, and special tests during pregnancy ( 11 ). This is also supported by a study done among women attending antenatal care clinics in Lagos State, Nigeria, which was evidenced by the number of women claiming to seek out information on foetus growth and development, childbirth delivery, nutrition during pregnancy, and health and wellness ( 20 ). This might be due to the fact the fact that pregnancy can be a stressful phase of a woman's life. So to maintain some level of certainty, many of them need information about the progression of their current pregnancy and the health of their foetus. More than half of the women in this study depend on professional sources for information, followed by digital sources, while the least common source of information is conventional sources. In line with this, a study at Royal Women's Hospital, Melbourne, Australia, found that, for the majority of pregnant women, the discussion with midwives was the source of information ( 21 ). A study done among Dutch women and Khayelitsha Health District in South Africa revealed that the most commonly used information sources are professional sources, followed by family or friends, and then websites ( 22 )( 15 ). This could be due to women obtaining pregnancy-related information through a variety of channels; however, formal sources like a healthcare provider are the most common. This is because most women find professional information sources trustworthy and useful, and as these people are trained professionals, it will be accurate and reliable medical information. Two other common sources of information among pregnant women were conventional sources (family members, friends, and books) and digital media such as websites. In this study, pregnancy-related worries among pregnant women who used digital sources of information as trusted sources were higher compared with pregnant women who used professional sources of information. In contrast to this study, in other studies, most of the women used the internet as their main information source before or during their pregnancy ( 15 )( 23 ). The main reason was to find information and read about people in the same situation, as well as because of its convenience, ease of accessibility, and direct availability ( 6 ). This difference might be due to the fact that the fact that online search algorithms were not developed to accommodate an individual’s threshold for fear and worry and the vast amount of generic information with little or no clarification being offered ( 24 ). In this study, maternal educational status was significantly associated with pregnancy-related worry. Pregnant women with diplomas and higher educational status had lower odds of pregnancy-related worry than pregnant women who were unable to read and write. This result is supported by studies done in Debremarkos, Ethiopia (( 25 ), and China ( 26 ). These might be justified by the association of a lower level of education with lower socioeconomic status, unemployment, and more daily troubles that decrease women’s participation in social activities, information attainment, and getting appropriate care ( 27 ). The findings of this study showed that pregnant women who had no history of previous pregnancy complications had fewer worries than those who had a history of previous pregnancy complications. This might be due to the fact that complications of pregnancy involve the mother's health, the baby's health, or both. The fear of the outcome of a pregnancy complication and the fear of experiencing the complication in a current pregnancy also increase worries in these women. This study revealed that pregnant women living at a distance of a distance of more than 5 kilometres from the health facility had higher worries compared with pregnant women living at a distance of less than 5 kilometres from the health facility. This might be due to the accessibility of transportation and financial capacities that influence women to get to the health care facility, especially in times of pregnancy emergencies. Social support is the interaction of subjective and objective support from people in many elements, such as knowledge, tools, and emotional support, from a variety of sources, such as family, friends, neighbours, colleagues, and groups. This study showed that pregnant women who have high social support had lower odds of pregnancy-related worry as compared to pregnant women who have low social support. The possible reason could be that mothers who have poor social support might not be happy with their family and have no good interaction with their social environment, and this may lead to social withdrawal, less psychological and coping ability, and eventually more worry ( 28 ). Strengths and limitations of the study The strength of this study lies in the use of standardized instruments to measure pregnancy-related worry and other factors among pregnant women. The limitation of this study was that it relied on self-report; there could be potential recall and social desirability biases. Secondly, this study was conducted among expectant women visiting the ANC clinics in government hospitals. Hence, the results might not be generalizable to other expectant women seeking ANC services in the private health sector. Thirdly, the study design was cross-sectional in nature, so the temporality of the studied variables could not be established. Conclusion and recommendation This study showed that pregnancy-related worries were prevalent among pregnant women attending ANC at Abebech Gobena Hospital, Addis Abeba. Pregnant women’s place of residence, educational status, previous pregnancy complications, distance from the health facility, trusted source of information, and social support had an effect on having pregnancy-related worries. Based on the findings of our study, we recommend future longitudinal studies are needed to know how pregnancy-related worry changes across the course of pregnancy. To encourage pregnant women to increase their social network by establishing a self-help group throughout their routine ANC follow-up, health practitioners should ensure easy accessibility to more information and additional reading material, and concerned public organizations like women and youth air and other governmental organizations (NGO) should work on educating women on preventing pregnancy-related worries. This can be done by giving out brochures, flyers, sticking posters, and more community outreach. This study assessed pregnancy related worry and associated factors among pregnant women attending ante natal care.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethods\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003eA cross-sectional study using a systematic random sampling method was conducted on 422 pregnant women who came for antenatal care follow up at Abebech Gobena MCH hospital from June 1, 2023 to September 30, 2023. The socio-demographic and obstetric characteristics, health information status, health care service utilization and social factors were obtained and pregnancy related worries were measured using Cambridge Worry Scale. Descriptive statistics was used to see frequency of independent variables. Simple logistic regression analysis was computed and all independent variables with p-value less than 0.25 were entered in multivariate logistic regression. Multivariate logistic regression was fitted to identify the factors associated with pregnancy related worries.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResult\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003eFrom total of 422 pregnant women using Cambridge worry scale, 54.7% of the respondent women had pregnancy related worries and the rest 45.3% had no worries.
