This study aimed to provide translation, cultural adaptation, and validation of the Czech version of IPOS-r. Methods: The IPOS-r was translated and culturally adapted to Czech using cognitive interviews. Patients and staff in dialysis centers and outpatient renal clinics completed the IPOS-r. The reliability of Czech IPOS-r was tested with Cronbach alpha (for internal consistency), the intraclass correlation coefficient for total IPOS-r score, and weighted Kappa (for test-retest and inter-rater reliability of individual items). Convergent validity was tested with Spearman correlation analysis to Kidney Disease Quality of Life Survey-Short Form 1.2 (KDQOL-SF 1.2). We tested responsiveness to change using a distribution-based approach. Results: The sample included 88 patients (mean age 66 ±SD 13.8; 58% men), who were treated with hemodialysis (70.5%), home hemodialysis ( 5.5%), peritoneal dialysis (3%), and conservative management (21%).). The Cronbach alpha was 0.72; intraclass correlation was 0,84 for test-retest reliability and 0.73 for inter-rater reliability. Good convergent validity of IPOS-r correlated to KDQOL-SF 1.2 was proved with Spearman correlations between 0.4-0.8 for most of the IPOS-r items. Using IPOS-r is feasible and meantime to complete the questionnaire was 9 minutes. Patients who reported a change in health status after one month demonstrated a total IPOS-r score change of 8 points in both positive, and negative directions. Conclusions: The process of translation and cultural adaptation of IPOS-r was efficient and the Czech IPOS-r questionnaire is a reliable and valid tool. Our results recommend the use of Czech IPOS-r for the documentation of symptoms in patients with advanced chronic kidney disease. Trial registration GAUK[82121] IPOS-r Outcome measurement Validity Reliability Patient-reported outcome measure Advanced kidney disease Symptom assessment Background Patients with end-stage renal disease (ESRD) suffer from a high symptom burden, which is comparable to those living with advanced cancer. 1 , 2 Persistent physical or psychological symptoms impair functional status, well-being, and health perception, and contribute to a lower quality of life (QoL). 3 Patients treated with dialysis can live longer, but this survival benefit disappears in frail elderly patients with many comorbidities 4 , their functional status and independence in activities of daily living significantly decrease after starting dialysis. 5 Symptoms and problems recognition by healthcare staff caring for these patients is often inadequate. 2 , 6 They focus mainly on physical symptoms, rely on standard consultation and recognition of symptoms´ severity is often poor. 7 , 8 Using patient-reported outcome measures (PROMS) on regular basis can improve QoL and outcomes in kidney disease. 9 , 10 Optimal PROMS should be short, sensitive to a change of health status, easy to administer, valid, and reliable for the tested population. There are several PROMS available for renal patients, and some of them are used despite limited validation data. 11 According to the national survey made in renal clinics of Australia and New Zealand IPOS-r was used most frequently. 9 IPOS-r has been developed by the POS team in the UK as a result of demand from clinicians to merge the IPOS and the POS-r. IPOS has been validated in a population of palliative care patients with both cancer and non-cancer diagnosis. 12 The English IPOS-r shows good test-retest reliability, internal consistency, and construct validity in patients with advanced chronic kidney disease and was recommended in symptom assessment. 13 This survey offers patient- and staff-completed versions assessing the same domains, both with good psychometric properties. 13 Basic IPOS has already been translated, culturally adapted, and validated in the Czech Republic, but the survey was tested on palliative patients in hospices and hospitals, 81% of them had cancer. 14 Use of IPOS on renal patients has not yet been tested in the Czech Republic. The only validated tool used in this country in renal research is the Czech version of the KDQOL-SF 1.2 15 , which is not used clinically because of its length. IPOS-r was validated just in the English version 13 , no country-specific validated translations are available. By translation, cultural adaptation, and validation of the Czech IPOS-r we have tried to bridge this gap. Methods This was a mixed-method multicenter study conducted in 5 facilities in the Czech Republic (1 outpatient renal clinic, and 4 dialysis centers). Concepts analysis The first phase of the study was a brief literature review of all concepts used in IPOS-r followed by the second phase, which was translation and cultural adaptation of the questionnaire. Translation and cultural adaptation The second phase was based on guidelines for translation and cultural adaptation of the IPOS family instrument, available on the POS web page. 16 Forward translation of IPOS-r was made by two translators with Czech as their first language, one was a healthcare worker and the other was a professional translator. Their translations were merged by the research team and the version for cognitive interviews was created. The Czech version was then translated back into English by two translators with English as their first language, one with and one without a healthcare background, and both versions were sent to the POS team in the United Kingdom for the final check. The final corrected version was used afterward for cognitive interviews, we performed in-depth qualitative interviews to check for patients‘ and staff views on the questionnaire. Validation The third validation phase was conducted in 1 outpatient clinic (Faculty Hospital Kralovske Vinohrady in Prague) and 4 dialysis centers across the Czech Republic ( BBraun Avitum Ohradni in Prague, Fresenius Medical Care in Melnik, Fresenius Medical Care in Louny and Fresenius Medical Care in Slany). Data were collected by trained physicians, nurses, and social workers during the regular patient encounters or patients sent the survey by post. We included a convenience sample of adult patients with advanced kidney disease (eGFR < 15 ml/min/1,73m 2 ) who were treated with hemodialysis, home hemodialysis, peritoneal dialysis, or conservative management. We included all patients except those, who were cognitively impaired, didn't have the Czech language as their mother tongue or were too unwell to participate. Participants completed the questionnaire independently or with help from their families or healthcare provider. Doctors, nurses, or social workers completed their version of the questionnaire on the same day independently from the patients. Survey data were collected at three time points. Different instruments were used at each time point. At the first time point (T1), patients completed the IPOS-r patient version and KDQOL-SF 1.2, and independently healthcare staff completed the IPOS-r staff version. At the second time point (T2) patients filled in IPOS-r three days after the first questionnaire had been completed. At the third time point (T3) IPOS-r was completed one month after the first questionnaire and the patients replied if their situation had changed by answering „no“, yes, negative change“ or „yes, positive change“. During the third assessment, patients also completed the time needed to fill in the questionnaire completely. Statistical analysis Demographic data were reported using descriptive statistics. Patients who had any missing values in the IPOS-r questionnaire were excluded from the analysis. A significant p-value was set at 0,05 and all analyses were conducted within SPSS version 28.01. We tested item analysis, reliability, and validity of the Czech version of IPOS-r as follows. Item analysis For every item of IPOS-r, we computed the mean, standard deviation, and modus. We also computed Item difficulty which was calculated using item mean and converted to an interval (0;1) using the formula mean-scale min/(scale max-scale min). Correlations with total scores without a particular item were also provided. Reliability Reliability was computed using three different ways. Reliability was assessed with internal consistency using Cronbach α for the total score of IPOS-r. Test-retest reliability was determined on the first and second assessments of IPOS-r. We computed the level of agreement for every item and assessed agreement with quadratic weighted kappa. A total score of IPOS-r agreement was assessed with Intraclass correlations(ICC) and the level of ICC from 0.4 to 0.7 was considered moderate and > 0.75 was considered to represent high test-retest reliability. 17 Inter-rater reliability was assessed on patients and staff ratings on the first time point using weighted kappa, level of agreement for every item, and intraclass correlations for the total score. The level of kappa from 0.41 to 0.60 was considered moderate, 0.61–0.80 as substantial, and 0.81–1 as almost perfect 18 , 19 Responsiveness to change We also assessed the responsiveness to change in our sample using a distribution-based approach 20 We compared mean changes based on the global change rating which was assessed by patients during the third assessment after one month. Patients were divided into 3 groups – positive change, negative change, and no change. Validity For assessing the convergent validity of IPOS-r we used the KDQOL-SF 1.2 questionnaire which is the only validated measure that is used in the Czech Republic for assessing the quality of life of patients with renal disease. We expected a mid-range correlation (0.5–0.7) between items from IPOS and KDQOL. For assessing validity we used nonparametric Spearman correlations. Results Sample From March 2021 to December 2021 we collected data from 100 patients, however for 12 of them IPOS-r data were not complete and we excluded them from the final analysis. The final sample consisted of 88 patients with advanced chronic renal disease. The mean age in this sample was 66.1 (SD = 13.8), and 58% of patients were men. They were treated with hemodialysis (70.5%), home hemodialysis ( 5.5%), peritoneal dialysis (3%), and conservative management (21%). Cognitive interviews A project team member, who is a psychologist with experience in cognitive interviews, conducted 10 qualitative interviews with renal patients and 10 interviews with the healthcare staff in 2 dialysis centers and 1 hospital renal unit. We tried to assess the face and content validity of the questionnaire. The interviews covered all questions and answers to the questionnaire. We checked for their comprehensibility, appropriateness, and relevance for the interviewees and if any problematic parts were found, participants could have reformulated the IPOS-r questions. All the questions and answers of the survey were acceptable for the interviewees, we just had to add an explanation to the restless legs symptom, as the concept was not completely clear for those patients, who had never experienced it. Participants also suggested adding the „cannot answer“ option to the psychological domains of the survey. Item analysis In Table 1 we present descriptive statistics of all IPOS-r items and evaluation of each item´s difficulty and correlation with the total score. The minimum difficulty was 0.05 for vomiting, the maximum was 0.48 for family/friends anxiety finding this question the most variable. Most of the item-total correlations were higher than 0.3, only for Constipation, Diarrhea, Practical problems, and Time wasted on appointments there was a lower value. The mean total score was M = 21.8 (SD = 11.3, range 0–47). The IPOS option „ cannot answer“, which was suggested by participants of cognitive interviews for psychological domains, was not used by our participants in this study, therefore it could be left out. A level of Cronbach α above 0.7 is considered acceptable. 21 Test-retest reliability (Temporal stability) was computed for all items and also for the total score. We present the mean scores at T1 and after 3 days (T2), the level of agreement between these two ratings, and weighted Cohen’s kappa in Table 2 . Most of the kappa coefficients (22 of 24) were above 0.4 which is considered moderate test-retest reliability, only for Vomiting and Information items the value was below 0.4. The mean at the first measure was M = 21.8 SD = 11.3 and for the second measure M = 20.1, SD = 12.1. Intraclass coefficient for total score was ICC = 0.84 (95% IC = 0.76–0.9) and this value of ICC indicates high test-retest reliability. Inter-rater reliability was computed on the ratings of patients and staff which were assessed during the first time point. Weighted Cohen’s kappa was computed for every item and intraclass correlation was computed for the total score. For 11 items out of 24, we found sufficient agreement between the staff and patient assessment (weighted kappa > 0.4), with the highest level of agreement for Pain (0.66) and Changes to the skin (0.56). The lowest level of agreement was found for Anxiety (0.17). For the total score, we computed intraclass correlation coefficients which were 0.73 (IC 95% = 0.6–0.8) and this level is considered moderate agreement. Patients who reported positive change after 30 days had a mean change of the total scores of 8 points (a lower level of total score indicates better quality of life). Similarly, patients who reported negative changes had also an 8 points difference between the time points T1 and T3, meaning increasing in IPOS score, and a worsening quality of life. Table 4 Mean total IPOS-r score changes (between T1,and T3) by global change scale N Mean change (T1-T3) Yes, positive change 4 8.25 Yes, negative change 10 -8.6 No change 47 0.6 Missing data 27 Validity Convergent validity was confirmed using Spearman correlation with items from KDQOL-SF 1.2, which is the gold standard and the only validated tool used by the Czech nephrologist in research. We chose questions from the KDQOL questionnaire which were related to IPOS-r items and assessed the same concept. There was a whole team consensus on the selected items. If there weren’t any questions assessing the same concept, we chose those assessing the most similar one, still, some concepts in IPOS-r were missing in KDQOL (constipation, diarrhea, sore or dry mouth). They appreciated its clarity and shortness. The average time to complete the survey was 9 minutes, which was acceptable for all of them. Discussion PROMs are very useful tools to capture patients’ experience with disease and healthcare. Translation and validation of PROMs are needed as they are used not only in clinical practice but also in research and audit. IPOS-renal doesn’t have any validated translations except the English version. The Czech translation and cultural adaptation of IPOS-r were performed successfully, Czech IPOS-r is short and the time needed to complete it is acceptable for patients and staff. Items analysis showed that all of the items in IPOS meet the requirements for item difficulty and item-total correlation. The lowest discriminant ability was found in item Vomiting because 91% of patients did not report this symptom. This is consistent with previous results and validation of the Czech IPOS on palliative patients 14 . Another study with patients from hospitals and home-based palliative services found similar results when Vomiting, Practical matters, and Having enough information did not have a full range of responses. 12 The questionnaire showed sufficient internal consistency, excellent test-retest reliability, and sufficient agreement between the staff and patient assessment, especially in physical domains. In some of the physical domains, namely weakness, nausea, poor appetite, and sore or dry mouth, the study showed lower than moderate interrater agreement so the clinicians should focus on assessment of these symptoms, as they seem to be overlooked. Weighted kappa values for interrater reliability were in the range ‘‘poor’’ only for anxiety, lower than moderate agreement was seen in the most non-physical domains, which are more difficult to assess. Similar results were found also in the study assessing the psychometric properties of the original English version of IPOS-r. 13 Similarly, we were able to demonstrate very good convergent validity for IPOS-r when compared to the KDQOL-SF 1.2 questionnaire in most domains. The only items without sufficient correlation were family anxiety, ability to share with family, need for information, practical problems, and time wasted on appointments. These domains are not covered by KDQOL exactly so we matched them with similar concepts which could have lowered the convergent validity. Diarrhea, constipation and sore or dry mouth concepts were not present in KDQOL, so correlations were not assessed for these symptoms. IPOS-r can detect a change, negative change is consistent with decreasing QoL and higher IPOS-r total scores, and positive change is consistent with increasing QoL and lower IPOS-r total score. Because of the small number of patients reporting the relevant change in one month, we didn't calculate the Wilcoxon signed-rank test. Limitations This study has several limitations. The numbers of patients in the study were not sufficient to provide factor analysis and assess the domains of the Czech IPOS-r. The renal patients in our study were very stable, with only 14 of 100 reporting a change in their health status after one month, so we couldn´t have calculated responsiveness to change by a statistical test. Interval of the retest should be longer than one month to be able to assess responsiveness to change, on the other hand, this could increase the recall bias. IPOS-r and KDQOL, which were used as a gold standard, do not completely cover the same concepts, some domains assessed by IPOS-r are completely missing in KDQOL and vice versa, and this can lower the convergent validity of the survey. However, the validity of IPOS-r was also confirmed in cognitive interviews. Conclusions The process of translation and cultural adaptation of IPOS-r was provided efficiently and our study proved that the Czech IPOS-r questionnaire is a responsive, reliable, and valid tool. Our results recommend the use of this questionnaire for the documentation of symptoms in patients with advanced chronic kidney disease. The Czech translation and cultural adaptation of IPOS-r were performed successfully, Czech IPOS-r is short and the time needed to complete it is acceptable for patients and staff. Items analysis showed that all of the items in IPOS meet the requirements for item difficulty and item-total correlation. The lowest discriminant ability was found in item Vomiting because 91% of patients did not report this symptom. Patients and staff in dialysis centers and outpatient renal clinics completed the IPOS-r. The reliability of Czech IPOS-r was tested with Cronbach alpha (for internal consistency), the intraclass correlation coefficient for total IPOS-r score, and weighted Kappa (for test-retest and inter-rater reliability of individual items). Convergent validity was tested with Spearman correlation analysis to Kidney Disease Quality of Life Survey-Short Form 1.2 (KDQOL-SF 1.2). We tested responsiveness to change using a distribution-based approach.\u003c/p\u003e\u003cp\u003e\u003cstrong\u003eResults:\u003c/strong\u003e\u0026nbsp;The sample included 88 patients (mean age 66 ±SD 13.8; 58% men), who were treated with hemodialysis (70.5%), home hemodialysis ( 5.5%), peritoneal dialysis (3%), and conservative management (21%).). The Cronbach alpha was 0.72; intraclass correlation was 0,84 for test-retest reliability and 0.73 for inter-rater reliability. Good convergent validity of IPOS-r correlated to KDQOL-SF 1.2 was proved with Spearman correlations between 0.4-0.8 for most of the IPOS-r items. Using IPOS-r is feasible and meantime to complete the questionnaire was 9 minutes. Patients who reported a change in health status after one month demonstrated a total IPOS-r score change of 8 points in both positive, and negative directions.\u003c/p\u003e\u003cp\u003e\u003cstrong\u003eConclusions:\u003c/strong\u003e The process of translation and cultural adaptation of\u0026nbsp;IPOS-r was efficient and the Czech IPOS-r questionnaire is a reliable and valid tool.
