We report an exceedingly rare case of low-differentiated apocrine adenosquamous carcinoma with sarcomatoid differentiation presenting on the right face of an 86-year-old female. The tumor was managed with a two-stage surgical approach: first-stage wide local excision followed by second-stage wound reconstruction with skin grafting. Pathological examination confirmed deep adipose invasion with negative margins. This case highlights the clinical challenges, pathological features, and multidisciplinary management of this aggressive cutaneous malignancy.
