Abstract

Abstract Home care is the face-to-face delivery of medical care in the patients' homes. In recent years, home health care has been growing rapidly due to an increase in the elderly population, and due to people who have chronic illnesses, or are underprivileged. In order to obtain sustainability of health, it is important to use home care resources efficiently, but the dynamic nature of hospital operations such as limited available personnel, high no-shows and cancellation rates of treatments reduce the productivity and efficiency of hospitals. To overcome difficulties arising due to continuously changing operational conditions, we propose a dynamic patient assignment system for home care physiotherapy and rehabilitation hospital operations. The proposed framework is a rolling-horizon algorithm that iteratively solves an integer linear model for assigning home care physiotherapy patients to operators. Our objectives are ensuring the continuity of care and balancing operator workloads while keeping patient waiting times low. Based on historical data from one of the largest rehabilitation hospitals in İstanbul, the resulting dynamic patient assignment framework was tested under several simulated scenarios. The results show the effectiveness of the proposed system, especially in terms of decreased waiting times.
