Methods CDC WONDER Multiple Cause of Death database (1999-2020) was used to identify deaths in adults aged ≥65 years with influenza and/or pneumonia (ICD-10: J09–J18) as contributing causes. Age-adjusted mortality rates (AAMRs) per 100,000 were calculated using the 2000 U.S. standard population. Joinpoint regression estimated annual percentage changes (APCs) with 95% confidence intervals (CIs), stratified by sex, race/ethnicity, region, urbanization, place of death, and state . Results From 1999 to 2020, 3,462,340 deaths were recorded. Most occurred in medical facilities (65.45%) and nursing homes (23.7%). AAMR declined from 585.69 to 473.44, with a significant drop from 1999-2018 (APC: -4.97), followed by a sharp rise from 2018-2020 (APC: 38.14), reflecting early COVID-19 impact. Males (463.84) had higher AAMR than females (316.04). NH Americans (408.47) had the highest burden; NH Asians (301.78) lowest. Geographically, the Midwest (381.65), Kentucky (506.63), and rural (NonCore) areas (445.68) had the highest AAMR; the South (368.75), Florida (235.6), and urban (Large Fringe Metro) areas (348.09) lowest. Conclusion After decades of decline, influenza-and pneumonia-related mortality rose sharply post-2018 likely due to COVID-19. Findings highlight widening disparities and the urgent need for target prevention in Midwestern, NH American Males of rural and high-burden states.
