Evidence on caregiver burden in psychiatric inpatient settings in Nigeria remains limited. This study assessed the burden and QoL of caregivers of psychiatric inpatients at the Federal Neuropsychiatric Hospital Enugu (FNHE) in Southeast Nigeria. Methods A descriptive cross-sectional study was conducted among 214 caregivers of psychiatric inpatients selected through consecutive sampling. Caregiver burden and QoL were assesses using structured questionnaires adapted from the Zarit Burden Interview and the WHOQoL-BREF. Data were analyzed using IBM SPSS Statistics version 20. Frequencies and percentages were used to summarize categorical responses, while the chi-square test was used to assess associations between caregiver burden, quality of life, and socio-demographic variables. Statistical significance was set at p < 0.05. Results Most caregivers were aged 25–44 years (43.9%), female (57.0%), married (53.3%), and self-employed (63.1%). Nearly half had tertiary education (43.9%), and siblings constituted the largest proportion of caregivers (44.9%). Approximately 40.7% had provided care for more than three years. Overall, 45.8% reported little or no burden, 37.9% experienced mild to moderate burden, and 0.9% reported severe burden. Financial strain (43.9%) and emotional exhaustion (29.9%) were commonly reported challenges. Regarding QoL, 43.9% of respondents reported moderate QoL, 33.6% reported good QoL, and 11.2% reported poor QoL. Satisfaction was relatively high in some domains, with 76.2% satisfied with their ability to perform daily activities and 58.9% satisfied with personal relationships; however, 43.9% expressed dissatisfaction with their financial situation. Caregiver burden was significantly associated with QoL (p < 0.001), with higher burden linked to poorer QoL. Among socio-demographic factors, only caregiving duration was significantly associated with caregiver burden (p = 0.001). Conclusions Caregivers of psychiatric inpatients demonstrated notable resilience despite experiencing financial and emotional challenges. Increased caregiver burden was associated with poorer QoL, highlighting the need for targeted psychosocial and economic support interventions to improve caregiver well-being in psychiatric care settings. Background Mental health conditions are a major and growing public health concern globally, affecting individuals across all demographic groups and contributing substantially to morbidity and disability [ 1 ]. They encompass a range of disorders and psychosocial disabilities characterized by significant distress, impaired functioning, and elevated risk of self-harm. Globally, mental disorders accounted for approximately 14% of disability-adjusted life years (DALYs) in 2019, affecting nearly 970 million people, most commonly through anxiety and depressive disorders [ 2 , 3 ]. Severe mental illness can reduce life expectancy and heighten vulnerability to human rights violations, while the economic impact is largely driven by productivity losses [ 4 , 5 ]. In low- and middle-income countries such as Nigeria, this burden is intensified by stigma, inadequate mental health services, and limited treatment resources [ 6 ]. Psychiatric inpatients often require extensive support due to impaired capacity to perform daily activities [ 7 ]. This responsibility frequently falls on informal caregivers, whose critical role exposes them to emotional strain, financial hardship, and social isolation—collectively termed caregiver burden [ 8 – 10 ]. Burden can manifest as psychological distress, disrupted relationships, and reduced quality of life (QoL), with implications for both caregiver well-being and patient outcomes [ 11 , 12 ]. QoL, defined by the WHO as an individual’s perception of their position in life within the context of culture and value systems, is often compromised among caregivers due to prolonged caregiving demands, stigma, and limited support [ 13 , 14 ]. In Nigeria, caregivers frequently shoulder these responsibilities with minimal formal assistance [ 15 ]. Although prior Nigerian studies have examined aspects of caregiver burden and QoL, notable gaps remain. Most studies focused on specific disorders such as schizophrenia or bipolar disorder [ 10 , 16 ], emphasized financial and social stressors without exploring broader psychosocial dimensions [ 17 ], or reported high burden and distress without examining variations across caregiver subgroups [ 15 ]. Few studies have investigated the relationship between caregiver burden and QoL in a general inpatient psychiatric population, or considered how sociodemographic factors and caregiving duration influence this interaction. The Federal Neuropsychiatric Hospital, Enugu, serves a diverse population of individuals with severe mental illness who rely heavily on caregivers [ 18 ]. Despite their essential role, caregivers in this setting remain understudied. This study therefore assessed the burden and quality of life of caregivers of psychiatric inpatients, and examined the association between caregiver burden, QoL, and socio-demographic characteristics. Findings from this study provide evidence to inform the development of targeted psychosocial and economic support interventions that enhance caregiver well-being and improve patient care outcomes. 2. Methods 2.1 Study Area The study was conducted at the Federal Neuropsychiatric Hospital, Enugu State, Nigeria, a major tertiary mental health facility serving the southeastern region. The hospital provides specialized psychiatric care and attracts caregivers from diverse socioeconomic and cultural backgrounds. This setting offers a relevant context for examining caregiver burden and quality of life (QoL). Enugu State is located in the South-East geopolitical zone of Nigeria and is characterized by a rapidly growing population with diverse cultural and economic activities [ 19 ]. As a referral center within this region, the hospital provides an appropriate environment for assessing caregiving challenges influenced by limited mental health resources and sociocultural expectations. 2.2 Study Design The study adopted a descriptive cross-sectional design using a quantitative approach. 2.3 Study Population The study population comprised primary caregivers of psychiatric inpatients admitted to the hospital, defined as individuals who assumed principal responsibility for the daily care of the patient. Caregivers were eligible if they were aged 18 years or older, had provided care for at least three months, and gave written informed consent. Paid or professional caregivers, as well as caregivers receiving psychiatric treatment or those with cognitive impairments that could hinder participation, were excluded from the study. 2.4 Sample Size Determination The minimum sample size was calculated using Fisher’s formula for cross-sectional studies: $$n=\frac{{Z}^{2}*p(1-p)}{{d}^{2}}$$ Where n – minimum sample size required Z – Z-score corresponding to 95% confidence level (1.96) p – Prevalence of burden of caregivers of mentally ill patients attending an outpatient psychiatric clinic at a Nigerian tertiary health institution, set at 12.7% [ 15 ] d – Margin of error tolerated (0.05) Hence, To account for potential non-response and incomplete questionnaires, a 20% adjustment (34 participants) was added, resulting in a total sample size of 204, which was rounded up to 215 for convenience. 2.5 Sampling Technique A consecutive sampling technique was adopted in the study. All eligible caregivers who met the inclusion criteria and were present during the data collection period were recruited until the required sample size was reached. 2.6 Instrument for Data Collection Data were collected using a structured, interviewer-administered questionnaire comprising three sections. Section A captured demographic and socioeconomic characteristics, including age, sex, marital status, education, employment, income, relationship to the patient, and duration of caregiving. Section B assessed caregiver burden using items adapted from the Zarit Burden Interview, rated on a 5-point Likert scale. Section C measured quality of life using domains adapted from the WHOQOL-BREF, rated on a 5-point satisfaction scale. The questionnaire was reviewed and validated by a public health expert to ensure content validity. Internal consistency of the adapted scales was assessed using Cronbach’s alpha, yielding values of 0.772 for the caregiver burden scale and 0.733 for the quality of life scale, indicating acceptable reliability. 2.7 Data Collection Procedure Data were collected over a four-week period by a trained research assistant. Eligible caregivers were approached during patient admissions, clinic visits, and follow-up sessions. Written informed consent was obtained prior to participation, and questionnaires were administered through face-to-face interviews to accommodate participants with varying literacy levels. Participant confidentiality and anonymity were strictly maintained throughout the study. 2.8 Data Analysis Data were analyzed using IBM SPSS version 20. Variables were summarized using descriptive statistics. Chi-square tests assessed associations between categorical variables, with statistical significance set at p < 0.05. 3. Results 3.1 Socio-demographic characteristics of caregivers A total of 214 caregivers participated in the study (Table 1 ). The largest age group was 25–44 years, accounting for 43.9% (n = 94) of respondents. Females comprised 57.0% (n = 122), and over half were married (53.3%, n = 114). Regarding education, 43.9% (n = 94) had completed tertiary education, while the majority were self-employed (63.1%, n = 135). Most caregivers were siblings of the patients (44.9%, n = 96). Monthly income was predominantly within the ₦20,000–₦50,000 range (29.0%, n = 62). In terms of caregiving experience, 40.7% (n = 87) had provided care for more than three years. The monthly income is in the Nigerian Naira (₦) 3.2 Caregiver burden of psychiatric inpatients Assessment of caregiver burden (Table 2 ) revealed varied experiences among respondents. Over half of caregivers (56.1%, n = 120) reported that caregiving had never adversely affected their physical health. Emotional strain was experienced occasionally by 32.2% (n = 69) of respondents, while social isolation was uncommon, with 89.3% (n = 191) indicating they never felt isolated from family or friends. Time-related demands were notable: 28.0% (n = 60) sometimes and 25.7% (n = 55) always perceived caregiving as consuming excessive time. Financial burden was substantial, with 43.9% (n = 94) reporting that caregiving always affected their financial stability. A lack of support was consistently reported by 24.8% (n = 53), and emotional exhaustion was evident, with 29.9% (n = 64) always feeling emotionally drained. Despite these challenges, the majority of caregivers (87.4%, n = 187) never felt resentment toward the patient. A small proportion (0.9%, n = 2) experienced severe burden. Over half of respondents (53.7%, n = 115) were very satisfied with their overall health, and a substantial majority (76.2%, n = 163) reported high satisfaction with their physical ability to perform daily activities. Emotional well-being was more varied: 44.9% (n = 96) were very satisfied, while 20.6% (n = 44) expressed dissatisfaction. Personal relationships were largely strong, with 58.9% (n = 126) very satisfied. Financial stability remained a key concern, as 43.9% (n = 94) reported being very dissatisfied. Regarding stress management, 41.6% (n = 89) were very satisfied. Although 20.6% (n = 44) reported a lack of external support, the majority indicated satisfaction with available support. Finally, more than half of caregivers (52.8%, n = 113) reported high satisfaction with their caregiving role. Smaller proportions reported poor (11.2%, n = 24) or very poor QoL (0.9%, n = 2). H₁ There is a significant association between caregiver burden and the quality of life (QoL) of caregivers of psychiatric inpatients. Chi-square analysis (Table 6 ) revealed a statistically significant association between caregiver burden and quality of life among psychiatric inpatient caregivers (χ²(12) = 113.562, p < 0.001). Caregivers experiencing little or no burden predominantly reported good (49.0%, n = 48) or very good QoL (18.4%, n = 18), whereas those with moderate to severe burden were more likely to report moderate (57.6%, n = 19) or poor QoL (33.3%, n = 11). Chi-square analyses (Table 7 ) indicated that, among all socio-demographic variables examined, only the duration of caregiving was significantly associated with caregiver burden (χ² = 26.918, df = 9, p = 0.001). Other variables, including age, sex, marital status, education, employment, relationship to the patient, and monthly income, were not significantly associated with burden (p > 0.05), although monthly income approached borderline significance (p = 0.055). Discussion The socio-demographic profile of caregivers in this study highlights a predominantly young and economically active population. The largest age group (25–44 years) aligns with findings in Ethiopia, where the mean caregiver age was approximately 38 years [ 20 ]. This stage of life often involves balancing employment, parenting, and caregiving, which may amplify both psychological and economic strain. Females constituted the majority of caregivers, consistent with studies by Abayomi et al. [ 17 ], Jeyagurunathan et al. [ 21 ], and Souza et al. [ 22 ], reflecting the disproportionate caregiving burden assumed by women. This pattern mirrors traditional gender roles in African and global contexts, where women are expected to provide emotional and physical support within families. In Nigeria, the absence of formal support systems may further exacerbate this gendered burden. More than half of the caregivers were married, similar to the 65.2% reported by Jeyagurunathan et al. [ 21 ]. While marital status may offer additional emotional or financial support, it may also introduce competing family obligations, creating a dual burden. Notably, almost half had tertiary education, higher than levels reported in some previous studies, possibly reflecting better educational access among urban Nigerian caregivers [ 22 , 23 ]. A majority were self-employed, indicative of Nigeria’s informal economic structure, where small-scale business and daily earnings predominate. As highlighted by Okafor and Emedoh [ 16 ], lack of structured work benefits such as paid leave or health insurance may intensify stress when caregiving interferes with income-generating activities. Interestingly, siblings represented the largest group of caregivers, diverging from studies where parents, particularly mothers, are the primary caregivers [ 21 , 22 ]. This may reflect evolving familial roles in Nigeria due to parental migration, aging, or economic incapacity. Most caregivers earned between ₦20,000 and ₦50,000 monthly, highlighting significant financial constraints. Similar low-income patterns have been associated with higher caregiver burden and reduced QoL in other studies, likely compounding caregiving stress and limiting access to support or respite care [ 20 , 24 ]. A substantial proportion had been caregiving for more than three years, reflecting the chronic nature of psychiatric conditions. This aligns with previous findings which emphasize that prolonged caregiving is associated with emotional exhaustion, financial strain, and declining health outcomes [ 15 , 25 , 26 ]. The findings of this study highlight the complex and multifaceted nature of caregiver burden among those caring for psychiatric inpatients. 45.8% of caregivers reported little or no burden, 37.9% experienced mild to moderate burden, and only 0.9% reported severe burden, indicating predominantly moderate stress levels. This pattern aligns with previous studies [ 27 , 28 ], which similarly identified mild to moderate burden as the most common experience among psychiatric caregivers, reflecting the cumulative stress of long-term engagement with unpredictable mental illness. Emotional and financial burdens emerged as the most prominent stressors, while physical strain and social isolation were comparatively lower. Although more than half reported no adverse impact on physical health—a contrast to the more physically taxing effects noted by Von Kardorff et al. [ 26 ]—29.9% felt emotionally drained consistently, and 32.2% experienced intermittent stress. These findings are consistent with previous studies, all of which documented high levels of psychological distress, anxiety, and depression among caregivers in emotionally demanding psychiatric settings [ 15 , 21 , 25 ]. Financial strain was particularly pronounced, with almost half indicating caregiving always affected their financial stability. This corroborates prior research emphasizing the economic impact of caregiving, especially among self-employed or low-income caregivers [ 17 , 20 , 22 ]. Limited resources may restrict access to medications, transportation, or adequate nutrition, further exacerbating stress and reducing caregiver well-being. In addition, 24.8% of caregivers reported consistently lacking sufficient support, echoing findings from Ndikuno et al. [ 24 ] and Stanley et al. [ 25 ]. Insufficient family or institutional support intensifies emotional and logistical strain and may undermine sustained caregiving. Despite these challenges, majority of caregivers reported never feeling resentment toward the patient, reflecting cultural and moral expectations in collectivist societies such as Nigeria. However, suppressed resentment may mask underlying psychological strain [ 16 ]. These findings underscore the urgent need for targeted interventions, including financial assistance, psychosocial support, and counseling, as recommended in previous studies, to mitigate the cumulative effects of long-term caregiving for psychiatric inpatients [ 25 , 27 ]. The findings indicate that caregivers of psychiatric inpatients generally reported a moderately positive quality of life (QoL). These outcomes reflect the complex interplay between resilience, socio-economic status, emotional strain, and available support. The results align partly with Ndikuno et al. [ 24 ] and Hsiao et al. [ 27 ], who identified moderate to poor QoL among caregivers, although the present study shows a comparatively higher proportion of positive outcomes. This difference may stem from variations in healthcare infrastructure, cultural expectations, or internal coping mechanisms among Nigerian caregivers. Domain-specific analysis revealed that physical ability and satisfaction with the caregiving role were relatively high, whereas emotional and financial well-being were notable sources of concern. Specifically, 20.6% reported dissatisfaction with emotional well-being, and 43.9% were very dissatisfied with financial stability. These findings echo Abayomi et al. [ 17 ] and Souza et al. [ 22 ], emphasizing financial strain as a leading contributor to caregiver distress. Given that most caregivers were self-employed earning less than ₦50,000 monthly, economic pressures likely exacerbate stress and limit access to supportive resources. Satisfaction with personal relationships and the caregiving role suggests strong intrinsic motivation and a cultural sense of familial duty, consistent with Von Kardorff et al. [ 26 ]. However, emotional outcomes were mixed: while almost half were very satisfied with emotional well-being, 20.6% were dissatisfied, and 41.6% reported high satisfaction with stress management, reflecting the heterogeneity of caregiver experiences as noted by Jeyagurunathan et al. [ 21 ]. These findings underscore the importance of individualized psychosocial interventions and structured support systems—including peer networks, counseling, and psychoeducation—to enhance coping capacity and QoL, as advocated by Stanley et al. [ 25 ] and Hsiao et al. [ 27 ]. Despite a generally moderate-to-good QoL, financial and emotional challenges remain substantial, highlighting the urgent need for interventions aimed at alleviating burden and sustaining caregiving capacity for psychiatric inpatients. The present study demonstrated a strong and statistically significant association between caregiver burden and quality of life (QoL), highlighting the profound impact of caregiving stress on overall well-being. Caregivers reporting little or no burden indicated higher QoL. In contrast, those experiencing moderate to severe burden were more likely to report “moderate” or “poor”, emphasizing the negative ripple effects of caregiving demands. These findings are consistent with prior research. Srivastava et al. [ 28 ] identified a significant negative correlation between caregiver burden and QoL among dementia caregivers, while Ukpong and Ibigbami [ 10 ] reported similar trends among caregivers of patients with schizophrenia and bipolar disorder. Psychological distress stemming from caregiving burden has been shown to predict lower QoL across physical, emotional, and social domains [ 21 , 25 ]. Among socio-demographic factors, only duration of caregiving was significantly associated with caregiver burden. Other factors—including age, sex, marital status, education, employment, income, and relationship to the patient—were not statistically significant, although monthly income approached borderline significance (p = .055). Interestingly, longer caregiving duration correlated with lower perceived burden, contrary to studies such as Hsiao et al. [ 27 ], which reported higher burden with prolonged caregiving. This discrepancy may reflect the influence of cultural norms in Nigeria, where extended caregiving fosters adaptive coping and communal support, mitigating stress over time. The lack of significant associations between caregiver burden and sex, education, employment, or patient relationship contrasts with previous findings [ 17 , 20 , 22 , 23 ]. This may be explained by the more balanced gender distribution in the current sample, equitable sharing of caregiving responsibilities, or homogeneity in educational and employment backgrounds. Although monthly income did not reach statistical significance, financial strain emerged as a major stressor, with nearly 44% of caregivers reporting that caregiving always affected their financial stability. This aligns with prior studies highlighting income and financial hardship as key contributors to caregiver burden [ 16 , 17 ]. The borderline significance observed may reflect limitations in income stratification or sample homogeneity, suggesting that future research should employ more granular income measures to better capture its impact. These findings underscore that caregiver burden is a multidimensional construct influenced by both situational and contextual factors, and its mitigation requires culturally tailored interventions, including psychosocial support, financial assistance, and strategies to enhance adaptive coping over the course of caregiving. Limitations of the Study This study adapted and modified items from the Zarit Burden Interview and WHOQOL-BREF to fit the local context, rather than using the full standardized instruments. While this approach may limit direct comparability with other studies using the full scales, internal consistency for both adapted measures was acceptable (Cronbach’s alpha = 0.772 for caregiver burden and 0.733 for quality of life), supporting the reliability of the findings. 5. Conclusions This study examined burden and quality of life (QoL) among caregivers of psychiatric inpatients at the Federal Neuropsychiatric Hospital, Enugu State, Nigeria. Findings indicate that while nearly half of caregivers experienced little or no burden, a substantial proportion reported mild to moderate burden, with financial strain, emotional exhaustion, and time-related demands being particularly prevalent. Quality of life varied across domains: caregivers generally reported satisfaction with physical health and personal relationships, yet dissatisfaction was notable in financial stability and emotional well-being. A statistically significant association between caregiver burden and QoL was observed, confirming that higher burden is linked to lower quality of life. Among socio-demographic factors, only the duration of caregiving showed a significant association with burden, underscoring the impact of prolonged caregiving on well-being. These results highlight the influence of contextual, cultural, and systemic factors on caregiver experiences and emphasize the need for targeted, culturally sensitive interventions. To improve caregiver well-being, mental health institutions should establish structured support programs, including caregiver support groups, peer counseling, and psychoeducation, to help manage psychological and emotional demands. Financial assistance and subsidies, particularly for caregivers from low-income households, could alleviate economic strain. Periodic mental health screenings and counseling services would address stress, anxiety, and emotional exhaustion, while part-time or temporary respite care could provide caregivers with opportunities to recover from physical and emotional fatigue. Policymakers should recognize caregivers as essential members of the care team, integrate their needs into national mental health strategies, and promote public awareness campaigns to reduce stigma and strengthen social support networks. Interventions should also be sensitive to the duration of caregiving, providing intensive orientation and guidance for new caregivers and sustained emotional, psychosocial, and financial support for long-term caregivers. Evidence on caregiver burden in psychiatric inpatient settings in Nigeria remains limited. This study assessed the burden and QoL of caregivers of psychiatric inpatients at the Federal Neuropsychiatric Hospital Enugu (FNHE) in Southeast Nigeria.\u003c/p\u003e\u003ch2\u003eMethods\u003c/h2\u003e \u003cp\u003eA descriptive cross-sectional study was conducted among 214 caregivers of psychiatric inpatients selected through consecutive sampling. Caregiver burden and QoL were assesses using structured questionnaires adapted from the Zarit Burden Interview and the WHOQoL-BREF. Data were analyzed using IBM SPSS Statistics version 20. Frequencies and percentages were used to summarize categorical responses, while the chi-square test was used to assess associations between caregiver burden, quality of life, and socio-demographic variables. Statistical significance was set at p\u0026thinsp;\u0026lt;\u0026thinsp;0.05.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e \u003cp\u003eMost caregivers were aged 25\u0026ndash;44 years (43.9%), female (57.0%), married (53.3%), and self-employed (63.1%). Nearly half had tertiary education (43.9%), and siblings constituted the largest proportion of caregivers (44.9%). Approximately 40.7% had provided care for more than three years. Overall, 45.8% reported little or no burden, 37.9% experienced mild to moderate burden, and 0.9% reported severe burden. Financial strain (43.9%) and emotional exhaustion (29.9%) were commonly reported challenges. Regarding QoL, 43.9% of respondents reported moderate QoL, 33.6% reported good QoL, and 11.2% reported poor QoL. Satisfaction was relatively high in some domains, with 76.2% satisfied with their ability to perform daily activities and 58.9% satisfied with personal relationships; however, 43.9% expressed dissatisfaction with their financial situation. Caregiver burden was significantly associated with QoL (p\u0026thinsp;\u0026lt;\u0026thinsp;0.001), with higher burden linked to poorer QoL. Among socio-demographic factors, only caregiving duration was significantly associated with caregiver burden (p\u0026thinsp;=\u0026thinsp;0.001).\u003c/p\u003e\u003ch2\u003eConclusions\u003c/h2\u003e \u003cp\u003eCaregivers of psychiatric inpatients demonstrated notable resilience despite experiencing financial and emotional challenges.
