Objetivo Analizar si un programa de fisioterapia terapéutico-educativo mejora el dolor en la zona abdominopélvica, así como el control motor, la movilidad lumbar y la kinesofobia en una mujer con endometriosis. Métodos Este caso clínico se enmarca en un estudio más amplio del Departamento de Fisioterapia de la Universitat de València (UV) en colaboración con el Hospital Universitario La Fe (Valencia). Se realizó el seguimiento de una mujer de 35 años con endometriosis, migraña, ansiedad, antecedentes quirúrgicos de adenoides y un aborto previo (de gestación espontánea). Las variables de estudio evaluadas previamente (T0) y posterior a la intervención (T1) fueron: umbral de dolor a la presión (UDP) medido con algómetro (Wagner FDK-20), en el lado dominante a nivel supra e infraumbilical; movilidad de la pelvis y de la zona lumbar con el Test de Disociación Lumbopélvica y Toracolumbar (TDLT) con medición pre y post indicación; flexibilidad lumbar (Test de Schober); kinesofobia mediante la Escala de Tampa para Kinesofobia (TSK-11). La paciente participó en un programa de fisioterapia que incluía educación en dolor y ejercicio terapéutico de baja intensidad, 3 sesiones/semana durante 8 semanas, 16 sesiones, siendo la primera sesión de cada mes presencial (en laboratorios de la Facultad de Fisioterapia, UV) y el resto con seguimiento mediante aplicación (https://fisiotrack.com/). El caso clínico presentado sigue el Care Checklist y cuenta con el Comité de Ética de la Investigación con Medicamentos (CEIM - Hospital Universitario y Politécnico La Fe, registro 2023-249-1) y con el registro en clinicaltrials: NCT06212349. Resultados Todas las variables obtuvieron resultados positivos en T1. La UDP aumentó 0'1 kg/cm2 de T0 a T1 en el punto infraumbilical y 0'3 kg/cm2 en el punto supraumbilical. En el TDLT en T0 pre-indicaciones la puntuación fue de 2 puntos y post-indicaciones de 5 puntos, mientras que en T1 aumentó a 7'5 pre-indicaciones y 10 post-indicaciones. En el test de Schober mejoró 1 cm en T1. En la TSK-11 mostró menor kinesofobia tras intervención (T0: 32 puntos; T1: 23 puntos). Conclusiones Tras el análisis de este caso, el programa de fisioterapia terapéutico-educativa ha mejorado el dolor abdominopélvico, el control motor y la flexibilidad lumbar, reduciendo además la kinesofobia. Introduction

Endometriosis is a chronic pathology characterized by the presence of ectopic endometrial tissue situated outside the uterine cavity. Its main symptoms are a high level of pelvic and general pain, depression, fatigue, and gastrointestinal problems.

Objective

To analyze whether a therapeutic-educational physiotherapy program improves pain in the abdominopelvic area, as well as motor control, lumbar mobility, and kinesophobia in a woman with endometriosis.

Methods

This clinical case is part of a broader study conducted by the Department of Physiotherapy of the Universitat de València (UV) in collaboration with La Fe University Hospital (Valencia). A 35-year-old woman with endometriosis, migraine, anxiety, a history of adenoid surgery, and a previous abortion (spontaneous pregnancy) was assessed. The study variables assessed at baseline (T0) and after the treatment (T1) were the following: pressure pain threshold (PPT) measured with an algometer (Wagner FDK-20), on the dominant side at the supra- and infraumbilical level; mobility of the pelvis and lumbar area with the Lumbopelvic and Thoracolumbar Dissociation Test (LTDT) with pre- and post-indication measurement; lumbar flexibility (Schober Test); and kinesophobia using the Tampa Scale for Kinesophobia (TSK-11). The patient participated in a physiotherapy program that included pain education and low-intensity therapeutic exercise, 3 sessions/week for 8 weeks, 16 sessions, with the first session of each month in person (in laboratories of the Faculty of Physiotherapy, UV) and the rest applied through application (https://fisiotrack.com/). The clinical case presented follows the Care Checklist and has been evaluated by the Committee on Ethics for Drug Research (CEIM - Hospital Universitario y Politécnico La Fe, registro 2023-249-1) and registered in clinicaltrials: NCT06212349.

Results

All variables had positive results at T1. The PPT increased 0.1 kg/cm2 from T0 to T1 at the infraumbilical point and 0.3 kg/cm2 at the supraumbilical point. In LTDT at T0 the score was 2 points in the pre-indication measurement and 5 points in the post-indication one, while at T1 it increased to 7.5 pre-indications and 10 post-indications. In the Schober test the score improved 1 cm at T1. In the TSK-11 the patient showed less kinesiphobia after the intervention (T0: 32 points; T1: 23 points).

Conclusions

The therapeutic-educational physiotherapy program has improved abdominopelvic pain, motor control and lumbar flexibility, while also reducing kinesophobia in a woman with endometriosis.
