Remote Monitoring in Automated Peritoneal Dialysis Modifies the Clinical Course of Peritonitis: A Secondary Analysis of a Cluster Randomized Trial

Abstract

Abstract Background: Peritonitis remains the most serious complication of peritoneal dialysis (PD), often resulting in hospitalization, technique failure, and increased mortality. Remote monitoring (RM) in automated peritoneal dialysis (APD) provides fast and opportune access to treatment data, potentially enabling earlier clinical intervention. Aim: To assess whether RM modifies the clinical course of peritonitis in APD patients by evaluating the time to first episode, hospitalization outcomes, and ultrafiltration (UF) dynamics. Methods: This secondary analysis of a cluster randomized controlled trial included 801 APD patients (403 with RM, 398 with conventional APD) followed from 2018 to 2020. A total of 95 patients with confirmed peritonitis were analyzed. Clinical variables and UF trends from the Sharesource® platform were assessed for the 10 days before and after the peritonitis diagnosis. Survival was evaluated using Kaplan–Meier curves and multivariable Cox regression models. Results: RM-APD patients experienced a significantly longer time to first peritonitis episode (13.3 vs. 11.7 months; p < 0.001). Although there were no significant differences in overall peritonitis incidence, hospitalization, or mortality rates, RM-APD patients showed improved post-hospitalization survival (p = 0.003) and lower therapy dropout after infection (p = 0.001). UF trends suggested a pre-infection decline, though not statistically significant. Conclusion: RM does not reduce peritonitis incidence but appears to enable earlier detection and improved outcomes, supporting a shift toward proactive management in APD. Future research should explore predictive algorithms based on RM-derived data to enhance peritonitis care.

It is the direct cause of death in >15% of PD deceased patients and the cause of approximately 16-20% of PD technique failure 1–3 . Peritonitis induces structural and functional damage in the peritoneal membrane even after a single episode, and severe or repeated infections can lead to membrane failure expressed by ultrafiltration (UF) failure 4,5 . Peritonitis has been a risk factor for mortality for a long time since it is an inductor of systemic inflammation and fluid overload secondary to derangement of UF, both associated with cardiovascular events 6,7 . Peritonitis rates vary widely, even up to 20 times, depending on non-modifiable factors such as age, diabetes and other comorbidities, education, and economic levels. Modifiable factors, such as the size of the dialysis program and attending population, staff training, and the success of the training programs for patients or their caregivers, also influence peritonitis rates 8 , 9 . Remote monitoring (RM), a recent advance in PD technology, allows the PD team to have closer surveillance of the patients; it allows the follow-up of prescribed treatment, as well as effective dialysis time, UF volume, inflow and drain of PD solutions patterns and patient’s skill in the management of the device. This information enables the early detection of clinically relevant issues, such as catheter permeability, peritonitis, treatment adherence, and UF problems. The correct use of this information enables the establishment of proactive actions that can decrease adverse events and hospitalizations 8 , 10–12 . The objective of this study was to evaluate the utility of remote monitoring in the clinical follow-up of peritonitis episodes in patients undergoing automated peritoneal dialysis by comparing clinical outcomes, ultrafiltration trends, and treatment response between those managed with or without remote monitoring. METHODOLOGY A secondary study was conducted among patients who participated in a cluster-randomized clinical trial 13 comparing Remote Monitoring of Automated Peritoneal Dialysis (RM-APD) with conventional Automated Peritoneal Dialysis (APD). The protocol of the original study was approved by the National Commission of Scientific Research and the local Research Committees of each hospital. Data from patients with a diagnosis of infectious peritonitis during the peritonitis phase (from 2018 to April 2020) were extracted from the central database of the two arms of the original study. The extracted information contains data on socio-demographic characteristics, baseline clinical data, as well as records of comorbidities and adverse events. The devices used in the original study were Homechoice Claria®, Baxter, S.A. de C.V., equipped with a device for RM (Sharesource® connectivity platform, Baxter Healthcare, Deerfield, IL, USA). Selected patients met the diagnostic criteria of peritonitis according to the ISPD, which include characteristic clinical data, such as abdominal pain, cloudy dialysis effluent, 3,14 leukocyte counts in dialysis effluent >100/µl (with a minimum permanence of 2 hours), >50% polymorph nuclear cells, and positive effluent culture 4 . The antibiotic prescription was based on the recommendations of the ISPD Clinical Guidelines. To evaluate the impact of peritonitis on ultrafiltration (UF), the date of clinical diagnosis was defined as the reference point. UF data were extracted from the Sharesource® platform over a 30-day post-diagnosis period. UF patterns were compared between patients who achieved clinical resolution and those who did not respond to antibiotic therapy. Statistical analysis Qualitative variables are expressed as simple frequencies and percentages. Quantitative continuous variables are expressed through their mean and standard deviation. Differences between groups were analyzed by Student´s “t” test or Squared Chi test according variable characteristics. To assess the effect of RM-APD on peritonitis episodes, incidence rates were analyzed at both the cluster and patient levels. Time-to-event outcomes, including time to first peritonitis episode, first hospitalization, and death, were evaluated using Kaplan–Meier survival curves and compared with the log-rank test. Multivariable Cox proportional hazards models were used to adjust for potential confounders, with results reported as hazard ratios (HR) and 95% confidence intervals (CI). UF dynamics during peritonitis episodes were analyzed using data extracted from the Sharesource® platform. Comparisons were made between patients who responded to antibiotic therapy and those who did not, as well as between patients with and without catheter dysfunction. Additional analyses included treatment adherence and the amount of dialysis time lost during the episode. RESULTS In the original study, 801 patients were enrolled: 398 in the APD group and 403 in the RM-APD group. Baseline clinical and biochemical characteristics were similar between groups. Figure 1 shows the patient selection and follow-up process. A total of 95 patients developed peritonitis and were included in the analysis. Follow-up continued until clinical recovery was achieved or, in the event of management needs, leave from the study was recommended. Several patients in the conventional APD group were shifted to CAPD to optimize antibiotic management, which limited the availability of Sharesource data for some cases. There were no significant differences between groups in the number of peritonitis episodes or their incidence rates. Table 1 presents the socio-demographic data for all patients with peritonitis and divided by study group. Except for age, there was no difference. The RM-APD group was ten years younger than the APD group. Table 2 presents the clinical parameters of both groups analyzed (APD and RM-APD). Most variables showed no difference except for neutrophils. The time (in months) from baseline until the first episode of peritonitis was significantly longer in the RM-APD group (14.81 months) compared to the APD group (11.20 months) (p <0.001), as shown in Figure 2. During the follow-up period, there were no differences between groups in the rates of first and subsequent peritonitis, hospitalization rates or days of hospitalization for peritonitis, or deaths secondary to peritonitis (Table 3). However, patient-level analyses revealed that the time from baseline to first hospitalization for peritonitis (p = 0.003) and dropout due to peritonitis (p = 0.001) were significantly shorter in the APD group, as shown in Figures 3 and 4, respectively. The data provided by the Sharesource platform is shown in Table 4. There were no significant differences between the groups. No relationship was found between episodes of peritonitis and exit site infections or tunnel infections, nor between peritonitis and mechanical problems with the catheter. No statistical significance was found regarding the variation of UF data provided by the Sharesource platform (β: 0.999; 95% CI: 0.998-1.001) during and after recovery from peritonitis. Finally, for the logistic regression analysis of factors associated with peritonitis recovery, no statistically significant associations were found. Factors include empirical monotherapy (β = 2.571; 95% CI: 0.721–9.167), empirical therapy covering both Gram-positive and Gram-negative bacteria (β = 0.400; 95% CI: 0.040–3.955), and targeted therapy (β = 0.727; 95% CI: 0.258–2.053). Additionally, ultrafiltration data from the Sharesource® platform showed no significant association (β = 0.999; 95% CI: 0.998–1.001). DISCUSSION The RM did not alter the rates of incidence of peritonitis, mortality, or hospitalization; however, peritonitis episodes occurred later among patients on RM-APD, and dropouts following infection were lower in this group. These findings suggest that RM may alter the clinical course of peritonitis by facilitating earlier diagnosis and more timely intervention. The longer time to the first peritonitis episode observed in the RM-APD group aligns with prior studies 15 , 16 , which reported indirect benefits of RM on treatment continuity and adherence in patients undergoing APD. While no significant differences were noted in subsequent peritonitis episodes, hospitalization, or peritonitis-related mortality, the reduced rate of technique failure indicates a potential role of RM in preserving long-term dialysis viability. From a pathophysiological standpoint, continuous monitoring of ultrafiltration (UF), dwell dynamics, and alert patterns enables the clinical team to detect subtle, early alterations in treatment performance. While no statistically significant association was found between UF trends and treatment response, a consistent decline in UF was noted during peritonitis follow-up. This observation aligns with the findings of Rojas-Díaz and Ramos, who reported similar UF variations preceding peritonitis onset, suggesting that remote monitoring may function as an early warning system 17 . Additionally, it may help identify non-responders sooner, allowing for timely interventions such as antibiotic adjustment, catheter reassessment, or a shift to alternative dialysis modalities 18 . Organizationally, RM represents a transformative opportunity for high-volume healthcare systems such as the Mexican Institute of Social Security (IMSS). Previous reports 10 19 have shown that remote oversight through platforms like Sharesource® reduces the need for in-person consultations, enhances team responsiveness, and improves institutional resource utilization without compromising patient safety 12,15,20 . Additionally, RM may empower patients by providing real-time feedback, potentially improving engagement and self-care. This analysis stems from a cluster randomized controlled trial, a design that inherently carries the risk of inter-center variability. However, no significant differences were observed between groups at the cluster level in patient enrollment rates, suggesting balanced recruitment. Furthermore, patient-level analyses demonstrated consistent trends favoring RM-APD, which reinforces the internal validity of our findings. The relatively small sample size for UF-related analyses may have limited statistical power to detect associations with stronger significance. This study provides evidence that may inform the development of predictive models using data derived from RM in peritoneal dialysis. Previous works have demonstrated the feasibility of applying artificial intelligence to RM data for the early identification of clinical complications such as peritonitis or suboptimal treatment response 21 , 22 . These emerging approaches reflect a growing interest in incorporating RM into clinical decision-making frameworks, aligning with the principles of precision nephrology and anticipatory care. Clinical Implications for Practice The implementation of RM in APD programs offers a scalable, strategic innovation for IMSS. By enabling real-time surveillance, RM facilitates earlier identification of peritonitis, supports prompt clinical decisions, and potentially reduces unnecessary hospitalizations. Objective data provided by RM platforms can improve the efficiency and responsiveness of clinical teams while promoting patient-centered care. Additionally, RM may enhance treatment adherence and reduce technique dropout, ultimately improving long-term clinical outcomes. In high-demand health systems, this translates into more sustainable care models and optimized use of institutional resources. Study Limitations The original cluster design may introduce heterogeneity between participating centers regarding peritonitis incidence. However, patient-level analyses yielded consistent results, reinforcing the reliability of the findings. Moreover, the limited number of patients with complete UF data may have constrained the ability to detect statistically significant trends in treatment response. A second concern is the difference in age distribution between groups; however, it cannot be attributed to inappropriate randomization, as age did not differ in the original study. Advanced age is associated with immunosenescence, which can lead to a diminished response to infection (as lower leucocyte count suggests), slower clinical recovery, an increased risk of technique failure, and a lower skill level in patients from the APD group in the management of PD. Although age was adjusted for in multivariate models, this imbalance may still have influenced the observed outcomes and should be considered when interpreting the results. CONCLUSION While RM-APD does not reduce the overall incidence of peritonitis, it alters the clinical trajectory of these episodes by facilitating continuous treatment monitoring, timely clinical interventions, and sustained technique viability. These results advocate for the integration of RM into institutional dialysis programs. Future studies should focus on developing predictive models using RM data to anticipate adverse events and tailor peritonitis management strategies accordingly. Remote monitoring (RM) in automated peritoneal dialysis (APD) provides fast and opportune access to treatment data, potentially enabling earlier clinical intervention.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eAim: \u003c/strong\u003eTo assess whether RM modifies the clinical course of peritonitis in APD patients by evaluating the time to first episode, hospitalization outcomes, and ultrafiltration (UF) dynamics.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethods: \u003c/strong\u003eThis secondary analysis of a cluster randomized controlled trial included 801 APD patients (403 with RM, 398 with conventional APD) followed from 2018 to 2020. A total of 95 patients with confirmed peritonitis were analyzed. Clinical variables and UF trends from the Sharesource® platform were assessed for the 10 days before and after the peritonitis diagnosis. Survival was evaluated using Kaplan–Meier curves and multivariable Cox regression models.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults: \u003c/strong\u003eRM-APD patients experienced a significantly longer time to first peritonitis episode (13.3 vs. 11.7 months; p \u0026lt; 0.001). Although there were no significant differences in overall peritonitis incidence, hospitalization, or mortality rates, RM-APD patients showed improved post-hospitalization survival (p = 0.003) and lower therapy dropout after infection (p = 0.001). UF trends suggested a pre-infection decline, though not statistically significant.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion: \u003c/strong\u003eRM does not reduce peritonitis incidence but appears to enable earlier detection and improved outcomes, supporting a shift toward proactive management in APD.
