Study of relationship between effectiveness of mifepristone and progestin recepter in endometriosis after conservative surgery

Abstract

[Objective] To study the relationship between effectiveness of mifepristone and progestin receptor (PR) after conservative surgery in the treatment of patients with endometriosis. [Method] 62 cases of endometriosis were immunihistochemically investigated. 20 cases which were positive PR and 20 cases of negative PR were treated with mifepristone in low daily doses for 3 to 6 months, other 22 cases hadn′t taken any hormone drugs. Recurrence rate, pregnancy rate and side effect were compared among these group. [Results] During treatment with mifepristone, symptom and sign were remarkably relieved in both positive PR and negative PR group. The total effectiveness rate was 97.5% whereas 68.2% in single surgical operation group. The recurrence rate of endometriosis with mifepristone in positive PR group and negative PR group was 5% and 30% respectively. There were no difference between positive PR and negative PR group in pregnancy rate. Irregular vaginal bleeding and menstruation disorder were commonly saw in group with negative PR, compared with positive PR group. Serum E2 levels remained in the range of follicular phase after treatment 3 months with mifepristone while side effects were mainly hot flushes and vaginal bleeding. [Conclusion] The effectiveness of mifepristone afer conserved surgery in the management of endometriosis was related with PR. There was low recurrence rate in those of positive PR.
