This case report describes a 34-year-old woman presenting with severe dysmenorrhea and primary infertility, who was found to have an isolated adenomyotic macrocyst in the anterior uterine wall. Diagnostic imaging via transvaginal ultrasound and MRI revealed a distinct cystic structure, which was subsequently confirmed through intraoperative excision and histopathological analysis as a rare variant of adenomyosis. The patient experienced significant symptom relief six weeks after the minimally invasive surgical removal of the lesion, highlighting this approach as a potential treatment option for such localized presentations. This paper is centrally about adenomyosis — specifically detailing a rare case of an isolated adenomyotic macrocyst in the uterine anterior wall and its successful surgical management.
