Abstract
Introduction: The frequency of endometriosis is 5-10% of the total female population. In the treatment of endometriosis, medical and surgical treatment are used. Surgical treatment is applied in the treatment of the endometrial ovarian cyst larger than 3 cm. Objective: To assess how successful laparoscopic cystectomy and ovarian cystotomy are in the treatment of endometrial ovarian cyst. Furthermore, to assess whether laparoscopic cystectomy reduces the ovarian reserve. Material and methods: This prospective study included 60 patients operated on for the endometrial ovarian cyst. 30 patients underwent laparoscopic cystectomy. These patients represented the study group. The control group consisted of 30 patients who had undergone laparoscopic ovarian cystotomy. Results: The average age of patients, length of menstrual cycle, duration of menstrual bleeding, dysmenorrhoea and the presence of dyspareunia were similar in both groups of patients. The percentage of recurrence of the endometrial ovarian cyst after cystectomy was 6.66% and 16.66% after ovarian cystotomy. The differences were statistically significant. Six months after surgery, no statistically significant differences were revealed in the residual ovarian volume and the number of antral follicles between the two groups of patients. Conclusion: Laparoscopic cystectomy does not lead to significant reduction in ovarian reserve compared to ovarian cystotomy. It has shown more success in reducing the percentage of recurrence after surgery.
