This case report describes a 21-year-old woman with extremely high CA 125 levels who was diagnosed with a ruptured ovarian endometrioma, demonstrating that such marker elevations can mimic malignancy in this condition.

This paper reports a case of a 21-year-old nulliparous woman who presented with an extremely high acute rise in CA-125 and was suspected of having malignancy. Laparoscopy identified a ruptured ovarian endometrioma, and histopathology confirmed the diagnosis, with the authors discussing possible pathogenesis and the evaluation/management approach for ruptured endometriotic cysts. The main limitation is that it is a single case report, so the conclusions about mechanisms and evaluation are not based on a broader patient cohort. This paper is centrally about endometriosis—specifically, CA-125 upsurge following rupture of an ovarian endometrioma that initially mimicked malignancy.

Abstract

CA 125, in gynaecological practice, is seen to be elevated in both benign and malignant conditions. Higher values, usually raise the suspicion of malignancy. Here we report a case of a 21-year-old nulliparous female, with extremely high evels of CA 125, suspected malignancy, but laparoscopy revealed a ruptured ovarian endometrioma, which was confirmed by histopathology. We discuss the pathogenesis of the same, evaluation and approach to management of ruptured endometriotic cysts.
