Design: Retrospective, single-arm cohort. Setting: German tertiary referral urogynaecology hospital. Population: 334 women with apical POP-Q ≥2 treated between 2016-2023. Methods: Patient records review for collecting perioperative data, NPRS-11 pain, Pelvic Floor Questionnaire. Pre-post differences were analysed with Wilcoxon tests; effect sizes reported as robust Cohen’s d with bootstrapped 95% CIs. Main Outcome Measures: Combined anatomical and surgical success (apical POP-Q 0–1 and no reoperation), patient satisfaction, pelvic-floor function, and adverse events. Results: Follow-up was 11.3 weeks (IQR 8.86-13.82). Success rate was 98.80% (n=330/334). Apical POP-Q improved from median 3 to 0 (d=5.45, 95%CI 5.13–5.92; p<0.0001), with concomitant improvements in anterior and posterior compartments. Pelvic floor function improved 7.70 points (d=1.91, 95%CI 1.47–2.33; p<0.0001), with prolapse symptoms reduction (d=2.70, 95%CI 1.97–3.84), improved bladder (d=0.84, 95%CI 0.66–1.06), bowel (d=0.44, 95%CI 0.25–0.67), and sexual function (d=0.39, 95%CI 0.15–0.54). Patient satisfaction was 97.90% (n=327/334). Median surgery duration was 19 minutes (IQR 16–24). Adverse events included: 0.30% intraoperative blood loss >200 mL (n=1/334), 0.90% partial removals due to mesh exposure (n=2/334) and infection (n=1/334); 1.20% residual urine >100 mL (n=4/334), 0.60% moderate pain (n=2/334), and 7.19% mild pain (n=24/344). Conclusions: Splentis showed high short-term anatomical success and functional improvement with low morbidity, supporting it as a uterus-preserving option for apical POP.
