Resumo
A endometriose é uma condição ginecológica crônica caracterizada pela presença de tecido endometrial funcional fora da cavidade uterina, cuja manifestação clínica mais recorrente é a dor pélvica persistente. Embora tradicionalmente tratada sob uma perspectiva anatômica e reprodutiva, estudos recentes têm evidenciado que a dor associada à endometriose decorre de processos fisiopatológicos sistêmicos, envolvendo inflamação contínua, desregulação hormonal e alterações neuroendócrinas. Tais mecanismos não apenas agravam o sofrimento das pacientes, como também dificultam o manejo clínico e comprometem sua qualidade de vida de forma global. Diante dessa complexidade, o presente estudo teve por objetivo analisar os impactos inflamatórios, hormonais e neuroendócrinos da dor crônica associada à endometriose, correlacionando-os às estratégias farmacológicas emergentes descritas na literatura científica da última década. Trata-se de uma Revisão Integrativa da Literatura, realizada nas bases PubMed, Scopus, SciELO, Web of Science e ScienceDirect, por meio de critérios sistematizados de seleção, análise crítica e categorização segundo os níveis de evidência do Instituto Joanna Briggs (JBI). Os resultados evidenciaram que a dor na endometriose se mantém por vias de sensibilização central, ativação do eixo hipotálamo-hipófise-adrenal, resistência aos glicocorticoides e inflamação de baixo grau. Além disso, verificou-se que os antagonistas de GnRH de ação oral representam uma alternativa terapêutica promissora, com eficácia significativa na supressão da dor e perfil mais tolerável de efeitos adversos. Conclui-se que o avanço da compreensão biomédica sobre os mecanismos da dor crônica tem impulsionado uma reformulação nos paradigmas terapêuticos da endometriose, aproximando a prática clínica dos princípios da medicina personalizada.
