This case report describes a 16-year-old girl with severe cyclical dysmenorrhea whose imaging and laparotomy found a unicornuate uterus with a non-communicating distended rudimentary horn and ipsilateral renal agenesis, complicated by right haematosalpinx and later endometriotic spots on the right ovary. After an initial attempt to connect the horn to the main uterine cavity followed by short-course danazol, her pain recurred and a cystic right adnexal mass was again seen on ultrasound, leading to hysterectomy of the non-communicating horn and haematosalpinx excision with ureteric stenting for identification. The authors explicitly note that their first drainage attempt was unconventional and that an artificial opening might heal with fibrosis and reclose, reflecting the limitation of a single-patient experience without comparative outcomes.
