The study investigated how preoperative history and the therapeutic approach relate to endometriosis recurrence in 201 women, with particular emphasis on patients’ subjective pain ratings before and after “sanierende” endometriosis surgery. Recurrence was defined as reemergence of complaints, and recurrence differences were analyzed using Kaplan–Meier estimates and Cox regression. Over five years, the recurrence rate was 38.7%, and women with higher preoperative pain levels and those with greater postoperative pain reduction experienced more frequent recurrences; abnormal bleeding duration, preoperative dyspareunia, and preoperative analgesic use were additional multivariable risk factors, while continuous oral contraceptive use was associated with reduced recurrence risk.
