Abstract

An 85 year old female patient had a colonoscopy for bright red blood per rectum. She underwent a hysterectomy with pelvic lymphadenectomy for low grade endometrioid adenocarcinoma, stage Ic, 15 years ago. During sigmoidoscopy internal hemorrhoids were found and treated with sclerotherapy. In addition, a submucosal bulging was seen 5cm above the anal sphincter in the ventral wall of the rectum. The patient was referred to the gynecology unit for a transvaginal ultrasound. This showed a hypoechogenic focally irregular mass in the anterior wall of the rectum at 5.5cm from the anus. The muscularis of the rectum seemed to be contiguous with the mass and the lesion caused stenosis of the lumen. There was no free fluid in the Douglas space. On colour Doppler examination the mass was strongly vascularised.
