Abstract

We present in this study our first experience with intravaginal culture (IVC) of human oocytes as a modified technique for in-vitro fertilization. Up to 4 oocytes and washed, pre-incubated spermatozoa (final concentration: 0.1-0.2 x 10(5)/ml) are pipetted into a plastic capsule. The capsule is closed, avoiding any air inclusion. It is placed into the maternal vagina for up to 50 hours, held in place by a diaphragm. The first 15 patients treated by IVC achieved a fertilization rate of 58% (35/60). Following transfer of the cleavage stages, 3 intact clinical pregnancies were established: 2 in the group with tubal sterility factors and 1 in the group of patients with endometriosis. In the group with andrological causes of sterility, one beta-hCG-positive reaction was detected. However, no intact clinical pregnancy was achieved. Our preliminary results show IVC to be a promising alternative in particular cases of sterility treatment. In contrast to the conventional technique, IVC minimizes exposure of gametes to factors which may affect their early development, such as light or low temperature. Furthermore, there is an important psychological factor to be considered, as the patient actively participates in the incubation period of in-vitro fertilization.

and Progress with Intravaginal Fertilization and Culture of Human OocytesPublication History Publication Date: 19 March 2008 (online) Zusammenfassung In dieser Studie berichten wir über die ersten eigenen Erfahrungen und Erfolge mit der intravaginalen Kultur (IVC) menschlicher Eizellen. Das Prinzip dieser Methode besteht darin, daß bis zu 4 Eizellen im Anschluß an die Follikelpunktion zusammen mit gewaschenen, vorinkubierten Spermatozoen (Endkonzentration: 0,1 - 0,2 × 105/ml) in eine Plastikkapsel gegeben werden. Diese Kapsel wird nach luftfreiem Verschluß in die Vagina der Patientin eingelegt und bis zu 50 h dort belassen. Ein Verlust der Kapsel wird durch ein Diaphragma verhindert. Bei den ersten 15 Patientinnen, die wir mit IVC behandelten, konnten wir so eine Befruchtungsrate von 58% (35/60) erzielen. Nach dem Transfer der Furchungsstadien wurden 3 intakte Schwangerschaften etabliert: 2 in der Gruppe mit tubarer Sterilität und 1 bei den Patientinnen, die aufgrund einer Endometriose in unser IVF-Programm aufgenommen wurden. In der Gruppe mit andrologischer Sterilitätsursache erfolgte bei einer Patientin ein β-hCG-Anstieg, jedoch wurde keine intakte Schwangerschaft etabliert. Diese vorläufigen Ergebnisse deuten darauf hin, daß die IVC eine vielversprechende Alternative in der Behandlung verschiedener Sterilitätsursachen darstellt. Im Gegensatz zum herkömmlichen Verfahren vermindert die IVC zudem die Exposition der Keimzellen gegenüber Faktoren, die sich nachteilig auf deren Entwicklung auswirken können, wie z. B. Licht und niedrige Umgebungstemperatur. Wichtig erscheint auch ein psychologischer Faktor, da die Patientin aktiv in den Vorgang der In-vitro-Fertilisation einbezogen wird.
