Laboratory investigations revealed the following: Complete blood count (CBC): White blood cell count (WBC) 10.68 × 10 9 /L, Neutrophil count 7.43 × 10 9 /L; High-sensitivity C-reactive protein (hs-CRP) 5.49 mg/L. Cardiac enzyme panel: Creatine kinase (CK) 211 U/L, Creatine kinase MB isoenzyme (CK-MB) 17 U/L. No significant abnormalities were found in: Liver function tests (7 items), Cardiac troponin I and myoglobin, Renal function and glucose, N-terminal pro-B-type natriuretic peptide (NT-proBNP), Thyroid function tests (3 items), Tumor markers (3 items), Rheumatoid factor, Anti-streptolysin O (ASO) titer, Blood culture for pathogens, Electrolytes (4 items), and D-dimer. Imaging studies: Chest CT showed bilateral pulmonary nodules, minimal inflammation in the right lower lobe, and aortic sclerosis. Echocardiography, lower extremity arterial and venous Doppler ultrasound, and abdominal plain film revealed no significant abnormalities. After screening for other etiologies based on auxiliary examinations and excluding adverse reactions to calcium channel blockers (CCBs), the presentation was attributed to an adverse drug reaction (ADR) to pramipexole. Due to personal reasons, the patient could not switch to an alternative oral medication for pramipexole. Consequently, the pramipexole dosage was reduced, and symptomatic treatments including diuresis for edema reduction, anti-infective therapy, and topical application were administered. Following treatment, the patient’s bilateral lower limb symptoms significantly improved.
