Abstract

今回われわれは月経随伴性気胸の稀な1例を経験したので報告する.症例は39歳,女性.平成15年3月5日呼吸困難を訴え,右気胸の診断で近医より紹介入院した. 2月にも他院で気胸の診断をうけ,保存的に治療された. 2回とも月経周期に一致している事より月経随伴性気胸が強く疑われたが,胸部CT上右肺尖部に気腫性変化もみられたため,胸腔鏡下手術を施行した.肺尖部に黒褐色嚢胞様隆起がありこれを切除し,また横隔膜に多数の小孔を広範囲に見たため,切除は断念し縫合閉鎖した.病理組織的には子宮内膜組織はなかったが,ヘモジデリンの沈着と小出血病変がみられ,間接的に内膜組織からの出血を示唆するものと考えられた.再発予防のため酢酸ブセレリンのホルモン療法を開始した.現在のところ再発はなく,経過良好である.
