A sua etiologia permanece incerta, uma vez que a menstruação retrógrada, o mecanismo habitualmente proposto, não justifica na totalidade o processo de fixação dos implantes. No entanto, a apoptose e a proliferação celular, processos fundamentais à homeostasia do tecido endometrial, apresentam alterações no endométrio de mulheres que sofrem desta patologia, o que poderá ajudar a explicar a sua etiopatogenia. A laparoscopia é o único método de diagnóstico definitivo para a endometriose. Uma vez que se trata de um procedimento invasivo, existe a necessidade de avaliar outros métodos mais inócuos que permitam o diagnóstico desta doença. Deste modo, o CA-125 tem sido proposto como o biomarcador mais promissor. No entanto, isolado ou em associação, este biomarcador carece de validação. O tratamento hormonal constitui a base do tratamento da endometriose, demonstrando eficácia e segurança no controlo dos sintomas associados a esta doença. Os seus efeitos estendem-se ao nível subcelular, interferindo com a regulação da apoptose e da proliferação celular. Neste sentido, os objectivos deste artigo de revisão são determinar as principais alterações no ambiente subcelular na endometriose, referentes aos factores que regulam a apoptose e a proliferação celular, a avaliação do CA-125 quanto ao seu valor diagnóstico, e de que modo todos estes factores evoluem com o tratamento hormonal. Endometriosis is a disease caracterized by the existence of endometrial glands and stroma in ectopic localization, which can compromise significantly the quality of life of women in childbearing age. Its etiology remains uncertain, since the retrograde menstruation, the commonly proposed mechanism, does not completely explain the implants fixation process. However, apoptosis and cellular proliferation, fundamental processes to endometrial tissue homeostasis, present changes in the endometrium of women who suffer from this disease, which may help explain its pathogenesis. Laparoscopy is the only definitive method of diagnosis for endometriosis. Since it is a invasive procedure, there is a need to evaluate other more innocuous methods for the diagnosis of endometriosis. Thus, CA-125 has been proposed as the most promising biomarker. However, either alone or in combination, this biomarker lacks validation. Hormonal treatment is the basis of endometriosis treatment, showing efficacy and safety in controlling the symptoms associated with this disease. Its effects extend to the subcellular level, interfering with the regulation of apoptosis and cellular proliferation.
