Rwanda’s successful efforts in malaria treatment reduced nationwide cases, but high mortality persists in the Eastern and Southern provinces. This study aimed to investigate the clinical features of severe malaria and to identify the death risk factors among under-five children in Eastern province Hospitals. Method This cross-sectional study assessed severe malaria mortality and associated factors among children aged under-five years in hospitals of Eastern Province, Rwanda, from 2017 to 2021. Data were collected from hospital records, and descriptive statistics and logistic regression were used for data analysis. Results Malaria mortality was 3.37%, mostly occurring at night (53.06%) and within 24 hours of admission (66.67%). Factors associated with severe malaria mortality included illness duration ≥ 4 days without treatment (OR = 29.22 [95% CI: 14.13–60.44]), coma (OR = 8.73 [95% CI: 8.08–38.07]), having three or more severe symptoms (OR = 5.95 [95% CI: 3.26–10.84]), high-level parasitaemia (OR = 3.71 [95% CI: 1.16–11.89]), and multi-organ complications (OR = 3.66 [95% CI: 1.04–12.84]), respiratory distress (OR = 3.33 [95% CI: 1.85-6.00]), vomiting (OR = 2.79 [95% CI: 1.49–5.20]), severe anaemia (OR = 1.85 [95% CI: 1.02–3.36]), Four day and more with illness without consultation (aOR = 38.45 [95% CI: 9.91-149.19]) and Coma (aOR = 9.61 [95% CI: 1.2-77.21]) were had greater odds of death in multivariate analysis. Conclusion In conclusion, targeted interventions are crucial to address - factors, improve healthcare-seeking behaviour, and ensure timely treatment for severe malaria in children under-five in Eastern Province, Rwanda. severe malaria mortality children under-five Rwanda factors hospital-based study Figures Figure 1 Figure 2 Figure 3 Introduction Malaria is endemic mainly in the tropical regions and subtropical regions of the world ( 1 ). Among 3.2 billion people living in poorest countries of sub-Sahara Africa, Southeast Asia, Western Pacific and Latin America, 10–15% are at risk of malaria with up to one million deaths annually, mostly children under age five ( 2 ). The percentage of malaria deaths among children under 5 years old has declined over the past two decades, from 87.3% in 2000 to 76.8% in 2015, but has since plateaued. In 2021, Africa accounted for 593,000 malaria deaths, with 78.9% of these deaths occurring in children under 5 years. Globally, there were over 2.4 million malaria cases in 2020, leading to more than 600,000 deaths. African countries were particularly affected, accounting for 93% of these deaths, with children under 5 representing about 80% of the fatalities. In Rwanda specifically, 2020 saw 2,986,047 malaria cases and 149 deaths ( 2 ). The factors associated with severe malaria mortality among under five were severe anemia or hypoglycemia, diarrheal disease, cerebral malaria and respiratory infection lack of health insurance, age of the patient, delayed diagnosis, coma, proximity and access to healthcare services, and weather conditions coma/seizures, hyperlactatemia and hypoglycaemia ( 3 – 6 ). Patients with severe malaria encounter several short and long-term health consequences, such as anemia, coma, metabolic acidosis, seizures, hypoglycemia, convulsions, and other neurological problems( 7 , 8 ). Today, malaria continues to be a significant threat to public health and remains one of the leading causes of death among children under-five years of age in Rwanda ( 9 , 10 ). Since 2012, malaria incidence dramatically increased in all 30 districts of Rwanda and triple increases in the Eastern province, and the prevalence of malaria among children under-five years old has increased by 6% from 2010 to 2017 ( 6 , 7 ) and to the best of our knowledge there was no information on severe malaria mortality and associated factors among under-five children ( 8 , 9 ). This study aimed to investigate the clinical features of severe malaria and to identify the death risk factors among under-five children in Eastern province Hospitals. Methods Study design and study area This cross-sectional study aimed to assess the factors associated with mortality caused by malaria in children under the age of five who were admitted for severe malaria between January 2017 and December 2021 in the Pediatric wards of nine hospitals located in the Eastern province of Rwanda that include Nyagatare DH in Nyagatare District is located in Northen eastern bordering Uganda and Tanzania in east, Ngarama DH, Kiziguro DH in Gatsibo District, Gahini DH, Rwinkwavu DH in Kayonza District, Kibungo RH in Ngoma District, Kirehe DH in Kirehe District. These hospitals of eastern provinces were reported to have an increased malaria mortality rate among under five ( 2 ). Sampling methodology and sample size determination A total of 2799 severe malaria cases were recorded in nine hospitals for the period of 2017–2021, systematic stratified sampling method was used to select the participants in the register and the sampling interval was 15th. The sample size was calculated using a single population proportion method, assuming a 50% case fatality rate for maximum representation, with a 95% confidence interval. This resulted in an estimated sample size of 422 patient files, calculated using Kish Leslie’s formula. ( 11 , 12 ). First, information that include sociodemographic details (age, sex, insurance status) for the selected participant were from register collected using Excel then Files of selected participants were retrieved from the hospital archive then used to complete other information such as clinical indicators (respiratory distress, convulsions, etc.), and management factors (blood transfusion, antimalarial drugs, etc.). Inclusion criteria encompassed children aged 2 to 59 months with positive malaria tests and severe symptoms. Patients outside this age range or lacking severe symptoms were excluded. Data analysis procedures Data were analyzed using STATA software version 16.0, descriptive statistics were used to summarize socio-demographic and clinical characteristics, as well as case management. Bivariate and multivariate logistic regression analyses were performed to assess factors associated with mortality among children diagnosed with severe malaria, the results of all logistic regression analyses were reported as odds ratios (OR) with 95% confidence intervals (CIs). Ethical considerations The ethical approval (No: CMHS/IRB/119/2023) was guaranteed by the University of Rwanda, College of Medicine, and Health Sciences, and the permission to access patients’ files was given by the Hospital administration and the research team ensured adherence to ethical guidelines and standards throughout the study by respecting the privacy and confidentiality of individuals whose data was utilized, managing the secondary data effectively, accurately reporting findings, and appropriately acknowledging the sources." and no consent or ascent was required as we used secondary data. Results Socio-demographic and clinical characteristics of study participants A total of 422 participants were included in the study (Table 2 ), male were 53.32%(n = 225), half of the participants 50.00% (n = 221) were aged from 2 to 24 months and the median age was 24.5 months (IQR:13–40 months) and 93.13% had health insurance. The common symptoms of severe malaria among study participants were convulsion 66.59% (n = 281) and 23.46% (n = 99) were presenting more than three severe symptoms at once, cerebral malaria was the most presented from 64.76% ( n = 261) the most complication of severe malaria developed was severe anaemia 27.25% (n = 115) followed by hypoglycemia 4.74% (n = 20) and the renal failure 0.47% (n = 2) whereas most presented comorbidity was severe malnutrition was presented among 3.08% (n = 13), most of the patients 84.65% (n = 353) spent less than three days with illness onset before seeking medical care then 70.38%( n = 297) had parasitemia level + + on microscopy (Table 2 ). Case management and environmental characteristics Most of the cases were treated using Artesunate 96.92% (409), a total of 30.09% (127/422) were transfused with blood whereas among 115 severe cases, 97.39% (n = 112) were transfused and 55.5% (n = 116) were transfused in less than 3 hours after admission and 2.61% (n = 3) died before transfusion, most of the patients hospitalized for less than one week 75.59% (n = 319) with the average time of hospitalization of 2.33 days (extremes:1–5 days). Most of the patients were admitted in the rainy season 60.19% (n = 254) (Table 2 ). Environmental characteristics Seasonality Dry season 168 39.81 Rainy season 254 60.19 Severe malaria treatment outcome The proportion of malaria mortality among children was 12.8% (n = 54) whereas 87.2% (n = 368) had a favourable evolution, most of the deaths occurred during the night 53.06% (n = 26) and 66.67% (n = 36) died within 24 hours of admission at the hospital. The figure below ( Fig. 2 ) shows that the trend of severe malaria cases decreased gradually from 2017 to 2019 from 55.88 to 40.53% then raised to 51.62% in 2021 in the Eastern province. Analysis of the trend of severe malaria mortality among children under five in Hospitals situated in the Eastern Province of Rwanda 2017–2021 The figure presented below ( Fig. 3 ) illustrates the trend of severe malaria mortality among children under five in hospitals located in the Eastern Province of Rwanda from 2017 to 2021. The data reveals an upward trend in the mortality rate, rising from 2.7 in 2018 to 3.9 in 2021. The overall case fatality rate for the period was calculated to be 3.37%. Bivariate analysis of factors associated with mortality among children with severe malaria. The table below ( Table 4 ) shows a bivariate analysis of the association between severe malaria mortality and clinical characteristics, socio-demographics, and case management. Among the clinical characteristics analyzed, several factors were found to be significantly associated with mortality risk among children with severe malaria. The following associations with the risk of death were observed at a 95% confidence interval: coma (OR = 8.73 [95% CI: 8.08–38.07] p-value < 0.001), respiratory distress (OR = 3.33 [95% CI: 1.85-6.00] p-value < 0.001), vomiting (OR = 2.79 [95% CI: 1.49–5.20] p-value = 0.001), severe anaemia (OR = 1.85 [95% CI: 1.02–3.36] p-value < 0.04), the patient presenting more than three number of severe symptoms (OR = 5.95[95% CI: 3.26–10.84] p-value < 0.001), high-level parasitemia (parasitemia level ++++) (OR = 3.71[95%CI: 1.16–11.89] p-value = 0.02), having severe anaemia along with cerebral complication (OR = 3.66 [95%CI: 1.04–12.84] p-value < 0.04), illness onset that lasts four days and more without treatment (OR = 29.22 [95%CI: 14.1314-60.43636] p-value < 0.001). The table below ( Table 5 ) shows the multivariate analysis of factors associated with mortality among children with severe malaria, two factors were found to have significant association with the risk of death. These factors are coma and a delay of four days or more from illness onset to receiving treatment. Coma was found to have an adjusted odds ratio (AOR) of 9.61 (95% confidence interval: 1.20, 77.21) with a p-value less than 0.03. This suggests that children with severe malaria who experience coma are at a significantly higher risk of mortality compared to those without coma. The delay of four days or more from the onset of illness to receiving treatment was also identified as a significant predictor. It had an adjusted odds ratio of 38.44 (95% confidence interval: 9.90-149.19) with a p-value less than 0.001. This indicates that children who had a longer delay in receiving treatment after the onset of severe malaria were at a significantly higher risk of death. These findings highlight the importance of early recognition and prompt treatment of severe malaria, as well as the need to closely monitor and manage cases involving coma. Timely and appropriate medical interventions are crucial in improving the outcomes and reducing mortality among children with severe malaria. This study aimed to investigate severe malaria mortality and associated factors in children under-five years in hospitals of Eastern Province-Rwanda from 2017 to 2021. Our study found that the severe malaria proportion of mortality among children admitted in eastern province hospitals during the study was 3.37%, this proportion in our study was quite similar to findings reported by other studies( 13 – 17 ) but this was inferior to that found in other studies done elsewhere in Africa ( 18 – 22 ). The low severe malaria proportion of mortality in our study can be to be related to early diagnosis and treatment of simple malaria by community health workers along with the implementation of effective malaria prevention and control measures and good management of severe malaria at the Hospitals in Eastern province which are well equipped to manage complications caused by the diseases. Our study showed that staying four days and more with illness without consultation was 38 times risk of death compared to patients who did early consultation, this was similar to the study done by Félicien Ilunga-Ilunga et al.( 23 ) and Muoneke V et al.( 19 ) this differs from the findings of Kalinga et al in Tanzania( 14 ) found that severe acidosis and severe anaemia as major predictors of mortality. This is similar to findings from a study done by Mathew M. Ippolito in Zambia ( 24 ) by Faith Mutsigiri-Murewanhema et al in Zimbabwe ( 20 ) The delay in the consultation might be attributed to home management of fever involving sponging or washing with warm water at the household level, which was widely practiced by caregivers and consulted traditional health practitioners. Limitation As this was cross sectional study did not identify specific factors associated with a delayed presentation to healthcare facilities. Malaria mortality could be overestimated as the result of misdiagnosis of severe malaria cases yet there could be concomitant parasitaemia especially bacterial sepsis Conclusion In conclusion, delayed initiation to anti malaria drugs can results in coma status and were the leading factors associated with death among children at admission. These findings highlight the importance of prompt treatment of severe malaria and early recognition, as well as the need to closely monitor and manage cases involving coma. Timely and appropriate medical interventions are crucial in improving the outcomes and reducing mortality among children with severe malaria. This highlights the importance of educational interventions targeted towards guardians to enhance their ability to recognize the signs of severe malaria.
