Influência do uso de contraceptivo hormonal sobre o perfil inflamatório do peritônio com endometriose

Abstract
Endometriosis results from the abnormal implantation of endometrial tissue outside the
uterine cavity and has an uncertain etiology. It mainly occurs with pain and infertility, the first
choice for pain control is hormonal contraceptives (HC), although their action is not well
established in the literature. The present study analyzed the influence of the use of HC on the
inflammatory profile of the peritoneum of patients with endometriosis. This was a crosssectional
study with laboratory analysis. The study took place in tertiary hospitals in southern
Brazil, with sample collection from 65 patients, 42 with endometriosis (26 without HC and 16
with HC) and 23 without endometriosis. An interview was conducted with a questionnaire for
clinical-demographic information, peritoneal lavage and biopsy of the peritoneum were
collected. The samples were analyzed at the neuroscience laboratory to quantify the
concentration of IL-10 (using ELISA’s method), the averages obtained were analyzed by
ANOVA. The samples of peritoneal lavage with endometriosis showed a higher concentration
of IL-10 than the control group (p ≤ 0.05) and statistical significance was obtained for the
reduction of IL-10 in the peritoneal lavage of patients with endometriosis using HC (8.1 ± 7.4
pg/mL), compared to women with endometriosis without using HC (17.9 ± 10.5 pg/mL).
Considering the stage of the disease, a higher level of IL-10 (p ≤ 0.05) was observed both in
the initial stages (12.1 ± 10.5 pg/mL) and in the advanced stages (15.4 ± 9.7 pg/mL), in
relation to the control (1.7 ± 1.8 pg/mL); being significantly higher in advanced
endometriosis. This study contributes to fill gaps about the real influence of HC use in
peritoneum affected by endometriosis and the behavior of inflammatory system in
peritoneum’s profile in mild and minimal degree. Therefore, it reinforces the hypotheses of a
higher concentration of IL-10 in moderate and severe stages.
