Multidisziplinäres Behandlungskonzept der Endometriose Grad III und IV bei Sterilität

The paper studies a multidisciplinary treatment approach for stage III/IV deep infiltrating endometriosis in women with infertility and/or significant abdominal and defecation-associated pain, all with rectum/anterior rectal wall involvement. Between 2007 and 2011, 19 patients were treated at a single university center using laparoscopic radical surgery, including complete excision of pelvic endometriosis and, depending on extent, anterior rectal resection with primary anastomosis or segmental resection; ureter visualization and endometriosis resection were performed in all cases. Reported outcomes include that all 19 underwent laparoscopic treatment, with initial complications including anastomotic insufficiency and a rectovaginal fistula, and in cases with protective ileostomy both were successfully reversed. This paper is centrally about endometriosis — specifically the feasibility and morbidity of fully laparoscopic radical surgical management for stage III/IV deep infiltrating rectovaginal endometriosis associated with infertility.
