Transcatheter Aortic Valve Implantation (TAVI) versus Surgical Aortic Valve Replacement (SAVR): A Retrospective Analysis from a Tertiary Care Hospital

Abstract Background: Since Pakistan is adapting to the technological advancements in the procedure, and shifting to Transcatheter Aortic Valve Implantation , more studies are required to evaluate the safe and efficacy of the procedure. The data on the topic is scarce, and our study will aim to fill in the literature gap. Furthermore, by looking at the burden of cardiac diseases, a policy implementation could be put in place to guide prevention and treatment in the country. We aim to evaluate the efficacy of Transcatheter Aortic Valve Implantation procedure and decide whether its a sustainable, cost effective enough of an option when compared to standard open heart surgery in the setting of low and middle income country Results: A higher Postoperative Complication Rate was observed with Surgical Aortic Valve Replacement compared with Transcatheter Aortic Valve Implantation. Among Surgical Aortic Valve Replacement patients the most common complications were postoperative bleeding, infection and cardiac tamponade. Patients who underwent Transcatheter Aortic Valve Implantation had higher incidence of other postoperative complications which were vascular complications and need for pacemaker implantation. On comparison the two groups did show significant differences in the mortality rate. Conclusions Transcatheter Aortic Valve Implantation has Low Postoperative Complication Rate and Mortality Rate compared to Surgical Aortic Valve Replacement. Our Data confirm that Transcatheter Aortic Valve Implantation is a safer and more reliable procedure for Patients suffering from Aortic Stenosis.
